Spillway structures serve as a vital protective element in regulating flood discharge to mitigate the risk of catastrophic dam failure. As an infrastructure featuring a distinct circular morning glory architecture, the Ir. H. Djuanda (Jatiluhur) Dam requires elevated structural safety, yet it currently undergoes cumulative concrete deterioration after exceeding five decades of operation. This study was conducted to analyze structural integrity and evaluate the maintenance management of the spillway components at Jatiluhur Dam. The methodology relies on a systematic content analysis of historical technical logs and secondary safety audit files spanning a 16-year timeline (2008–2024), quantified using a weighted Condition Index (CI) and a Failure Mode and Effects Analysis (FMEA) matrix. The evaluation reveals that the spillway's final Condition Index stands at 79.00%, indicating a marginal performance zone. Furthermore, the FMEA model indicates that the most critical operational risks are situated within the horizontal conduit and vertical shaft, both yielding a Risk Priority Number (RPN) of 32 due to visual inspection constraints. The engineering implication of this research urges dam authorities to transform routine maintenance into well-planned preventive strategies by integrating periodic non-destructive testing to detect deep-seated internal damage.
