EFFECT OF A DIGITAL REMINDER SYSTEM ON ANTENATAL CARE VISIT COMPLIANCE AND MIDWIFERY SERVICE QUALITY IN THE RIAU ISLANDS: A QUASI-EXPERIMENTAL STUDY

Abstract

ANC visit compliance in archipelagic regions remains suboptimal due to geographical, transportation, and service monitoring barriers. To analyze the effect of a WhatsApp- and Google Form–based Digital Reminder System on ANC compliance and midwifery service quality among pregnant women in the Riau Islands, Indonesia. A quasi experimental, non-equivalent control group pretest–posttest study was conducted on 128 pregnant women (64 intervention, 64 control) at Puskesmas Sambau over 8–12 weeks. Outcomes were measured using a validated 12T checklist and SERVQUAL-adapted instrument, analyzed via Wilcoxon, Mann–Whitney, and logistic regression tests. ANC compliance increased significantly in the intervention group (3.92 ± 1.21 to 5.73±0.61; p < 0.001, r = 0.60) but not in controls (p = 0.214). Service quality improved significantly in the intervention group (p < 0.001), although a large baseline imbalance between groups means this finding is exploratory rather than confirmatory. Digital reminder use was the strongest compliance predictor (OR = 8.88; 95% CI: 3.21–24.57). The digital reminder system was associated with improved ANC compliance and may contribute to enhanced perceived service quality; however, these findings require confirmation through randomized studies.
