Abstract

In general, diseases experienced by older adults are degenerative or age-related non-communicable diseases, such as heart disease, diabetes, stroke, rheumatic diseases, and injuries that commonly affect quality of life in later stages of aging. The increasing prevalence of non-communicable diseases (NCDs) poses a significant challenge, particularly for healthcare professionals and community-based health services in providing appropriate preventive and promotive care. This community service activity was conducted using a participatory approach to actively involve elderly participants in health screening and education. The activity was carried out on November 26, 2025, at Saint Joseph Church, Kediri, involving 60 elderly participants. The program was implemented in three stages: preparation, implementation, and evaluation to ensure systematic and effective execution. The results showed that the most prevalent condition identified among participants was hypertension (88.33%). Other conditions included hyperuricemia (51.67%), hypercholesterolemia (40%), joint diseases (13.33%), and hyperglycemia or diabetes mellitus (10%). In conclusion, this community service activity revealed a high prevalence of hypertension among the elderly. Therefore, continuous and sustainable community health programs are essential to maintain and improve the overall health status of elderly parish members.
