Abstract

Background: Emesis gravidarum is one of the most common complaints experienced by women during the first trimester of pregnancy due to hormonal changes. If not properly managed, this condition may reduce appetite, impair nutritional status, and negatively affect both maternal and fetal health. Lemon essential oil aromatherapy has been reported as an effective non-pharmacological therapy to relieve nausea and vomiting. Family involvement also plays an important role in improving maternal knowledge regarding the management of emesis gravidarum. Objective: To describe family nursing care for first-trimester pregnant women with emesis gravidarum through the implementation of lemon essential oil aromatherapy in the working area of Puskesmas Perumnas, Rejang Lebong Regency, in 2026. Methods: This study employed a descriptive case study design involving two families with first-trimester pregnant women experiencing emesis gravidarum. Data were collected through interviews, observation, physical examination, documentation, and assessment of nausea and vomiting severity using the Pregnancy Unique Quantification of Emesis and Nausea (PUQE-24). Nursing interventions consisted of health education and lemon essential oil aromatherapy. Results: The implementation of family nursing care improved family knowledge regarding emesis gravidarum management. PUQE scores decreased after aromatherapy intervention, indicating reduced nausea and vomiting. Families were able to recognize health problems, make appropriate decisions, provide home care, utilize health services, and support pregnant women in managing their symptoms. Conclusion: Lemon essential oil aromatherapy combined with family-based health education effectively reduced emesis gravidarum symptoms while improving family knowledge and participation in maternal care.
