Pregnancy in women aged over 35 years is classified as a high-risk obstetric condition because of the increased likelihood of preeclampsia, gestational diabetes, preterm birth, and low birth weight. In multigravida women, irregular attendance at antenatal care services may further increase these risks. This report was prepared using a case report approach involving a multigravida woman aged >35 years whose frequency of ANC visits had not met the recommended standard at TPMB Siti Amina Tanah Merah. Case information was obtained through anamnesis, physical examination, supporting examinations including hemoglobin, KSPR, and ultrasound, as well as documentation using the SOAP format. The case involved a multigravida woman in the second trimester aged ≥35 years with a KSPR score of 6, placing her in the high-risk pregnancy category. Inadequate ANC visits and the absence of ultrasound examination were factors that aggravated the condition. The interventions provided included outreach services, guidance for early referral to a primary health center, and an approach involving the mother, husband, and family members. This case demonstrates that irregular ANC attendance among multigravida women aged >35 years may increase the potential for obstetric complications. Comprehensive midwifery care through counseling, early referral planning, and family involvement is necessary to prevent delayed management while maintaining maternal and neonatal safety.
