Type 2 Diabetes Mellitus (T2DM) is a chronic metabolic disorder whose global burden continues to rise, placing considerable pressure on patients and healthcare systems. Successful therapeutic outcomes in Type 2 Diabetes Mellitus (T2DM) are largely contingent upon patient compliance with prescribed regimens and robust self-management practices. However, suboptimal medication adherence remains a persistent barrier to achieving glycemic targets and preventing complications. To enhance medication adherence and clinical indices, diverse pharmaceutical strategies encompassing telepharmacy, structured patient counseling, health education, and ongoing medication therapy monitoring are frequently implemented by pharmacists. A systematic literature review was conducted to evaluate the influence of pharmaceutical interventions on medication adherence and clinical outcomes in patients diagnosed with Type 2 Diabetes Mellitus (T2DM). Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, this study examined peer-reviewed articles published from 2019 to 2026. A rigorous bibliographic search was executed across prominent national and international electronic repositories, namely PubMed, ScienceDirect, Google Scholar, and SINTA. Identified records were screened by title and abstract, followed by full-text assessment against predefined eligibility criteria, and data from the included studies were extracted and synthesized qualitatively. The findings indicate that pharmaceutical strategies such as pharmaceutical care, patient counseling, home pharmacy care, telepharmacy, and health education consistently enhance glycemic regulation, therapeutic compliance, and overall quality of life, while simultaneously mitigating the frequency of drug-related problems (DRPs). Ultimately, current literature validates the effectiveness of pharmaceutical interventions in improving clinical parameters and medication adherence among individuals diagnosed with Type 2 Diabetes Mellitus (T2DM), underscoring the necessity for the broader integration of patient-centered pharmaceutical care to secure long-term clinical success.
