Chronic Kidney Disease (CKD) stage V is the final stage of kidney disease that requires kidney replacement therapy in the form of hemodialysis. Hemodialysis is known to trigger a systemic inflammatory response that potentially affects hematological parameters, including the Platelet-to-Lymphocyte Ratio (PLR). This study aims to determine the effect of hemodialysis on changes in platelet count, lymphocyte count, and PLR in hospitalized patients with stage V CKD at Purwokerto Islamic Hospital. This analytical study used a retrospective observational design with a paired pre–post design and involved 45 adult patients (≥18 years) with stage V CKD who underwent routine hemodialysis twice per week for 4-5 hours per session at RSI Purwokerto. Data on platelet count, lymphocyte count, and PLR values before and after one hemodialysis session were obtained from medical records. The normality of the data was tested using the Shapiro– Wilk test. A paired t-test was used for data that were normally distributed, while the signed-rank Wilcoxon test was used for data that were not normally distributed, with a significance level of p <0.05. A total of 45 patients met the study criteria. The average platelet count changed from 264.53±145.10 ×10³/μL before hemodialysis to 264.33±125.48 ×10³/μL after hemodialysis (p=0.133). The average lymphocyte count increased from 11.42±7.93 to 12.50±13.56 (p=0.488). The average PLR increased from 39.99±55.88 before hemodialysis to 44.90±53.24 after hemodialysis, but the difference was not statistically significant (p=0.692). In conclusion, one hemodialysis session did not have a significant effect on platelet count, lymphocyte count, or PLR values in stage V CKD patients. These findings suggest that PLR more reflected chronic inflammatory processes than acute hematological changes after one hemodialysis session.
