Maternal and infant health is an important component in improving public health, so comprehensive and continuous midwifery services are needed through the Continuity of Care (CoC) approach. CoC allows for comprehensive assessment and assistance starting from pregnancy, childbirth, postpartum, newborn care, to family planning, so that changes in maternal and infant conditions can be monitored consistently and early risk detection can be carried out optimally. This study aims to describe the process and results of the implementation of continuous midwifery care for Mrs. O, 28 years old, G3P1A1 at PMB Istiqomah, S. Tr. Keb., Bdn. Pringsari Village, Pringapus District during the period of March 2, 2026 to April 19, 2026. The method used is a case study with a descriptive approach through data collection using in-depth interviews, observations, physical examinations, and document reviews. The results of the assistance show that during pregnancy, Mrs. O did not experience complications, fetal growth was according to gestational age, and third trimester complaints such as anxiety and back pain could be overcome through health education and prenatal yoga. The birth was spontaneous, safe, and without complications, with the baby having a strong cry, good muscle tone, and normal vital parameters. During the postpartum period, uterine involution proceeded physiologically, lactation was smooth, there were no signs of infection, and the mother was able to care for herself and her baby independently. Newborn care also showed good results, including adequate elimination, normal neonatal reflexes, and smooth umbilical cord care. During the family planning phase, the mother chose the 3-month postpartum injectable contraceptive method. Overall, the implementation of the CoC provided tangible benefits in improving maternal readiness for each stage of reproduction, ensuring standardized care, and strengthening the quality of maternal and infant health monitoring. These findings emphasize the need for continued strengthening of continuous midwifery care as a strategic effort to support the reduction of maternal and infant morbidity and mortality.
