Acne is a chronic inflammatory condition involving the pilosebaceous unit, which may be influenced by the presence of Cutibacterium acnes. Its treatment includes the use of antibiotics administered orally or applied topically. Nutmeg seeds (Myristica fragrans Houtt.) have potential as an active ingredient in anti-acne preparations because they contain secondary metabolites, including alkaloids, flavonoids, tannins, and saponins. This study aimed to formulate an anti-acne serum containing ethanolic extract of nutmeg seeds and evaluate its physical characteristics and antibacterial activity against C. acnes. The extract was obtained through maceration using 96% ethanol and subsequently formulated into serum preparations containing 4% (F1), 6% (F2), and 8% (F3) extract. The formulations were evaluated for organoleptic properties, homogeneity, pH, viscosity, spreadability, specific gravity, skin moisture, stability using a cycling test, and skin irritation. Antibacterial activity was assessed by measuring the diameter of the inhibition zone. The results demonstrated that nutmeg seed extract could be incorporated into serum formulations whose physical and chemical characteristics were evaluated using the selected parameters. The inhibition zone diameters produced by F1, F2, and F3 were 8.2 mm, 13.6 mm, and 14.5 mm, respectively. Among the three formulations, F3 exhibited the largest inhibition zone against C. acnes. Nevertheless, further studies are required to improve formulation stability and obtain more comprehensive evidence regarding the safety and efficacy of the preparation
