Dry skin is a common issue experienced by many people, often causing discomfort and emotional stress. Hydrating serum formulations help retain skin moisture, resulting in smoother and more radiant skin. Antioxidants can prevent damage to essential molecules in the body. One of the plants widely found in Indonesia that contains antioxidant compounds is guava (Psidium guajava L.). This study aimed to formulate a hydrating serum preparation using guava leaf ethanolic extract, perform a stability evaluation of the formulation, and determine the IC₅₀ value using the DPPH method. The extraction was conducted by maceration using 96% ethanol. The extract was then formulated into a lotion preparation with varying concentrations of guava leaf extract: F1, F2, and F3. The formulations were evaluated for organoleptic properties, pH, homogeneity, viscosity, and hedonic quality, and the antioxidant activity was tested at a wavelength of 516 nm. The results showed that the guava leaf extract lotion met the requirements for organoleptic, pH, homogeneity, and viscosity tests. The IC₅₀ value of the ethanolic guava leaf extract showed a very strong antioxidant category, with the F1 formulation having an IC₅₀ value of 11,30 µg/mL.
