Abstract

Abstract: Background: Therapy intensification (TI) is the most important factor in bloodpressure control among the adherent patients. The TI is the physician prescription behavior toadd the item(s) and/or the dosage of hypertensive medicine when the patients' BP was ?10mmHgabove the target. Comorbid patients have 10mmHg lower BP target. Aims: to evaluate the effectof comorbidity on TI score and blood pressure control; and to correlate the variables of TI and BPcontrol. Method: retrospective cohort study done in 4 hospitals in Yogyakarta for 5 months. Thesubjects of age 18 years, hypertensive out-patient covered with Askes insurance, and ?1 visitwith uncontrolled BP were included. Hemodialysis subjects were excluded. Subjects weregrouped into with/without comorbid. The BP profile was analyzed with T-test, repeatedmeasurement Anova, and odds ratio. Results: subjects consisted of without (WO) (n=268) vs.with comorbid (W) (n=401) patients. Comorbid subjects had older age, more male proportionand more visits (p0.05). The profiles of final SBP/DBP in WO vs. W subjects were as follow:148.9/89.1 (WO) vs. 143.8/86.1mmHg (W) (p0.05); TI score (-) 0.360.26 (WO) vs. (-)0.380.24 (W) (p0.05); the final SBP: worse BP control 20.9 (WO) vs.16.2% (W), notcontrolled in all visits 38.1 vs. 45.9%, improved 17.5 vs. 23.9%, and good controlled in all visit23.5 vs.13.2%; proportion of subjects reached BP target 40.7% (WO) vs. 37.4% (W) (p0.05);the different of final minus target SBP: (-)9.018.5 vs. (-)13.917.4mmHg (p0.05); correlationbetween TI and variables of SBP (p0.05) with the coefficient (r) at 0.4-0.6 (medium).Conclusion: comorbidity had no effect on TI score; but subjects with comorbid had worse BPcontrol (p0.05); TI score correlated in medium level with SBP.Keywords: Comorbidity Factor, Therapy Intensification, Blood Pressure Control

Abstract

Abstract: Acetogenin compound known for its activity as anticancer by inhibited the NADHubiquinone reductase contained in the mitochondria. These compounds with high liphophilicityproperties have a weakness in solubility with log P 8.44, so modifications to improve thesolubility by maintaining a core group of tetrahydrofuran and ?-lactone as well as shortening thealkyl chain is needed. The modified compounds was lactogenin or 3-(3- (tetrahydrofuran-3-yl)acryloyl) dihydrofuran-2(3H)-one) that have a log P 0.90 and is expected to have a bettersolubility properties than acetogenin. Lactogenin synthesis was based on cross-aldolcondensation reaction by reacting tetrahyrdofuran-3-carbaldehyde and 2-acetyl-?-butyrolactone using sodium methoxide as catalyst. The synthesized compounds were red solutionwith specific smell. The TLC result showed the new products at Rf 0.314. The purity test by gaschromatography showed 74.07% purity. The structure elucidation by infrared and massspectroscopy showed that the synthesized compound was ?-hydroxy lactogenin or 3-(3-hydroxy-3-(tetrahydrofuran-3-yl) propanoyl) dihydrofuran-2(3H)-one.Keywords: NADH-ubiquinone oksidoreduktase, lactogenin, cross-aldol condensationreaction, ?-hydroxy lactogenin

Abstract

Abstract: A urinary tract infections (UTI) is an infection that affects the urinary tract caused bybacteria (most often Escherichia coli). Antimicrobial are used to treat UTI. The sensitivity ofbacterial pattern toward antimicrobials and the bacterials pattern will affect the effectiveness ofUTI treatment. This research was conducted to evaluate the strains of bacterial and sensitivity ofbacterial pattern that caused UTI. A descriptive evaluation and retrospective study was done inthis research. In total 79 cases patient with UTI at Inpatient Unit X hospital in Yogyakarta2011, whose has sensitivity test and sprout up germ culture examination data were included.Patient with UTI at Inpatient Unit which has barren germ culture data and sprout up germ wereexcluded. The common microbes were gram negative bacteria including Escherichia col,Pseudomonas aeruginosa, Klebsiella pneumonia, and Staphylococcus coagulase negative.Gram negative microbes were sensitive to amikasin, imipenem, netilmicin, and fosfomicin. Grampositive microbes were sensitive to nitrofurantoin, vancomicin, imipenem and cefuroxime.Keywords: Urinary Tract Infection, antimicrobial, and sensitivity bacterial pattern.

Abstract

Abstract: Antioxidant has the ability to capture free radicals. These free radicals can oxidizenucleic acids, proteins, lipids or DNA, causing degenerative diseases. Antioxidants compoundfound in plants such as phenolic acids, polyphenols and flavanoids would capture free radicalssuch as peroxide, hydroperoxide or lipid peroxyl and also inhibit the oxidative mechanisms thatshow degenerative diseases. Quercetin and quercetin 3-o-rhamnosida as constituents ofmistletoes (Dendrophthoe pentandra (L.) Miq.), are able to provide antioxidant activity.Mistletoes dried and extracted with 70% ethanol, followed by fractionation with ethyl acetate.Antioxidant activity of ethyl acetate fraction from ethanol extract was calculated using DPPHmethod and obtained IC50 values. The results showed that the ethyl acetate fraction fromethanolic mistletoes leaf extract has antioxidant activity with IC50 value of (12.57 0.7) mg / mL.The content of total phenolic (13.760.9) mg gallic acid equivalents per gram of ethyl acetatefraction from ethanol extract of leaves of the mistletoes.Keywords: Antioxidant, misletoes leaf, Dendrophthoe pentandra, ethyl acetate fraction,ethanolic extract, DPPH, total phenolic..

Abstract

Abstract: Approximately 11.000 new child cancer cases per annum. An appropriate cytostaticdrug regimen can worse renal function. Aims of this research to determine the use of cytotoxic inchildren chemotherapy based on GFR according Schwartz and CB formulas in RSUP Dr.Sardjito Yogyakarta 2010 period. Cytotoxic drugs include antibiotic and non antibioticcytotoxics. An observational descriptive evaluative study with retrospective design was done.Inclusion criteria were patient 0 12 years with creatinine serum data and exclusion criteriawere acute or chronic kidney failure with creatinine serum 5 mg/dL. The data taken werepatient's age, height, weight, and creatinine serum. Adjustment doses was evaluated based on2GFR 60 mL/min/1.73 m for cytotoxic drugs. Dose adjustment was evaluated according to DIHand BSA guidelines. Using 171 patient's datas with 160 cases of antibiotics prescription and 704cases of non antibiotics prescription. 1,8%; 5,6% cases of antibiotics prescription and 1,0%;3,6% cases of non antibiotics prescription required adjusment doses according to the Schwartzand CB formulas. Unappropriate doses were found for doxorubicine and bleomycine as cytotoxicantibiotics usage and etoposide, L-asparginase, dan cyclophosphamid as non cytotoxicantibiotics usage.Keywords : cytotoxic drugs, children, glomerular filtration rate, Schwartz formula, CounahanBarratt formula.

Abstract

Abstract: Antibiotics will be used safely when it is used rationally, and rational use of antibioticsneed an appropriately of knowledge. In accordance to achieve appropriately knowledge aboutantibiotics, it is important to identify the level of knowledge about antibiotics to improve aneducational method and material about antibiotics. As a qualitative study using focus groupdiscussion (FGD) among residence of Four Sub-district in Yogyakarta City, e.g.: Mergangsan,Gondokusuman, Umbulhardjo and Kotagede. There was eight key person of each sub districtinvolved in the study. The inclusion criteria are: Residence of one of the four sub-districts withage above 17 years old, willing to attend the activities. The exclusion criteria are: residence withPharmacy education background, moving limited and mental disability. Results of the studyshow the characteristic demography of the participants are: 5 Female and 3 Male, age rangefrom 40 to 52 years old, with educational level: 5 participants are high school graduated and 3participants are University graduated. All of participants are key person of each sub districtwhose familiar with the residences and the environmental where they are living. All ofparticipants mention that they do not know about antibiotics et all including resistance and otherrisks of irrational use of antibiotics. All of participants asking for complete information aboutantibiotics which simple, accessible and could be socialized through the routine activities inevery sub district e.g.: PKK (mothers gathering). In conclusion, the level of participants'knowledge about antibiotics is still low, and need to be improved. Complete information aboutantibiotics in a simple form has to be developed as an educational material.Keywords: Focus Group Discussion, Antibiotics, Knowledge

Abstract

Abstract: Analog of curcumin in forms of enone and dienone aromatic is known for their activityas an NF-?B inhibitor. In this study, will be synthesize 2-(4'-N, N-dimethylamino benzilidine)cyclohexane-1,3-dione as an analog that predicted has an activity as an NF-?B inhibitor. Thisresearch was conducted based on the crossed aldol condensation reaction by reacting 3 mmole pN,N-dimethylamino benzaldehide and 6 mmole cyclohexane-1,3-dione with hydrochloric acid asthe catalyst using solid phase reaction method. Based on computational analysis, 2-(4'-N,Ndimethylamino benzilidine) cyclohexane-1,3-dione showed a better interaction with NF-?Bprotein with PLANTSPLP score was -69,7895. The outcome of the reaction was yellow coloredpowder, no odor and soluble in hydrochloric acid 3N. The yield value was 78.8%. Liquidchromatography showed 100% purity. The melting point range was 237.5-240.3C. The resultsof structure elucidation by 1H-NMR, infrared and mass spectroscopy tests indicated thecompound was 2-(4-(dimetilamino)benzilidena)-4-(3-oksosiklohex-1-enil) sikloheksana-1,3-dion.Key words: 2-(4'-N, N-dimethylamino benzilidine) cyclohexane-1,3-dione, crossed aldolcondensation, solid phase reaction.

Abstract

Abstract: This research aim to find out the effect of M. tanarius leaf methanol-water extract(MTME) whether it can lowering blood glucose levels. This research was experimental studywith one way-complete-random design using 25 male rats which divided into five groups. Therats in group I were given CMC 1% (negative control). The group II rats were given 0.45 g/kg BWdose of Glibenclamide (positive control). In group III, IV and V, the rats were given 0.43; 1.28;3.84 g/kg BW doses of MTME orally. The hypoglycemic effect of MTME was tested by using theOral Glucose Tolerance Test (OGTT) method. The blood-glucose contents were measured at the0, 15, 30, 60, 90, 180, and 240 after the OGTT and treatment. The AUC were statisticallyanalyzed using one way ANOVA and Scheffe test with 95 % convidence level. The result show thatMTME at dose 0.43; 1.28; and 3.84 g/kg BW was effective in improving oral glucose tolerance.Keywords : Blood glucose, Methanol-water extract, Macaranga tanarius
