Abstract

Breast cancer still become a major health problem in Indonesia and worldwide until today. Based on WHO 2012, breast cancer incidence is reported as 1.67 million cases with 90% of mortality rate in the metastasis stage. Chemoresistant is one cause of this increased mortality and morbidity. Nowadays, there are many treatment choices for cancer, but 90% incident of chemoresistant breast cancer occur even with prior chemotherapy. This review aimed to describe the potential of microsphere combinations fucoidan and miRNA-200b as a treatment for chemoresistant in breast cancer. Literature review were derived from scientific journals using www.pubmed.com and scholar.google.com database with “chemoresistant breast cancer, Fucoidan, microRNA-200b” as keyword. Fucoidan can induce apoptosis through the extrinsic pathway involving apoptotic receptor, or intrinsic pathway involving changes in mitochondrial membrane potential (MMP) to release cytochrome C and activating the apoptotic signal. Meanwhile, miRNA-200b expression, will decrease Sp1 expression and decrease histone-3 acetylation level in a miRNA-200b promoter, resulting in decreased cancer cell migration and invasion. However, no studies have evaluated this combination clinically. So, further studies are needed to confirm the potential of microsphere combination fucoidan and miRNA-200b in chemoresistant breast cancer. Keywords: chemoresistant breast cancer; Fucoidan; miRNA-200b.

Abstract

Ovarian cancer is one of the major health problems in gynecology worldwide. Globocan 2012 data shows that ovarian cancer occurs in 239.000 cases, and is placed fourth as the most malignancy occurred in Indonesia. So far, cisplatin is the most effective therapeutic agent, but the high dosage of administration often caused side effects. A new alternative therapy to overcome the problems by using withaferin-A and nanoparticle PEGylated liposome. WFA has the potential effect to reduce cisplatin resistance but has low bioavailability thus, it needs to be encapsulated. This review’s goal is to depict the potency of withaferin-A, cisplatin, and PEGylated liposome combination as a therapeutic agent to treat ovarian cancer. The combination of Cisplatin-Withaferin PEGylated Liposome is constructed by the modified thin lipid film hydration method then administered orally. This combination suppresses 70-80% of the ovarian cancer cell growth in vivo and inhibits NGFR from binding to TRKA, prevents cell migration as much as 50%. Nano-Cisferin-Liposome, inhibits migration of ovarian cancer cell significantly through ?-catenin signaling through ALDH1 that disturb the spheroid formation, and increase apoptosis event as much 70-80% by increasing ROS production up to 2,5 times. Thus, Nano-Cisferin-Liposome (NCL) has potential as a therapeutic agent for treating ovarian cancer. Keywords: Cisplatin; Ovarian Cancer; PEGylated Liposome; Withaferin A

Abstract

Knee Osteoarthritis (KOA) is characterized by knee cartilage degradation. Knee Osteoarthritis symptoms are joint pain, stiffness, quadriceps muscle weakness and daily activities difficulties. The use of Kinesio Taping (KT) in Quadriceps muscle strengthening exercises (QSE) is known to increase quadriceps muscle strength, reduce pain and improve functional abilities in KOA. This study aimed to determine the effect of KT with QSE on quadriceps muscle strength and functional status in KOA. This is a double-blind randomized controlled trial, involving twenty-six KOA women that were divided equally into treatment and control groups. The treatment group was given KT and QSE, the control group received sham taping and QSE. Quadriceps muscle strength and WOMAC scores were measured before and after treatment at 4, 6 and 8 weeks. There was a significant difference in quadriceps muscle strength at the 6th week (p = 0.019) between treatment and control groups and no significant differences (p> 0.05) in the strength of quadriceps muscle at 4th and 8th weeks and WOMAC values ??at 4th, 6th and 8th weeks. We concluded that a combination of KT with QSE offers advantages for improvement of quadriceps muscle strength at 6th week but not for functional status in KOA. Abstract

Lupus vulgaris is a variant of cutaneous tuberculosis in a previously sensitized individual that has a high degree of delayed-type hypersensitivity to tuberculin. Absolute criteria for the diagnosis of cutaneous tuberculosis is a positive M. tuberculosis culture on guinea pig or polymerase chain reaction (PCR) examination. We reported a case of lupus vulgaris in a 32 years old female. A diagnosis of lupus vulgaris was made by clinical manifestation, therapeutic trial, interferon-gamma release assay (IGRA) test, and other laboratory tests. The patient presented with an erythematous scaly plaque on the face and left hand. Diascopy test was positive, consistent with the “apple jelly” sign. Regression of lesion was observed after 1 month of therapy with anti-tuberculosis drugs. Culture, PCR, and histopathological examination were not established due to patient’s refusal. The patient was allergic to pyrazinamide and ethambutol, therefore she was initially treated with a daily dose of 450 mg Rifampicin, 300 mg isoniazid (INH), and 500 mg levofloxacin for 2 months and continued with maintenance phase, three times weekly of 450 mg Rifampicin and 450 mg INH until one year. Treatment provided a good result. Keywords: tuberculosis; lupus vulgaris; therapeutic trial

Abstract

The prevalence of dental caries in Indonesia primary school is 60-80%. Dental caries is a continuous dynamic process involving repeating periods of demineralization by organic acids of microbial origin and subsequent remineralization by salivary components, but in which the overall environment is imbalanced towards demineralization. Rampant caries is a sign of gross dietary inadequacy, complete absence of oral hygiene practice or systemic illness. Rampant caries is suddenly appearing, widespread, rapidly burrowing type of caries, resulting in early involvement of the pulp and affecting those teeth usually regarded as immune to ordinary decay. This case report presents a case of rampant caries in an 8-year-old boy and the treatment. The cooperative behavior of patients and parents is needed for optimal results. Abstract

Noise-Induced Hearing Loss (NIHL) is a hearing disorder caused by noise. Noise with the intensity of 85 decibels (dB)or higher may damage the corti hearing receptor. Pilot is one of the riskiest occupations due to frequent noise exposure from aircraft machine. This research aimed to identify the influence of aircraft noise levels on hearing disorder of both helicopter and casa pilots. This is observational analytical research with cross-sectional design, involving 30 helicopter pilots and 30 casa pilots. The diagnosis of hearing disorder was done using secondary data from medical records. The aircraft noise was measured by sound level meter, the other risk factors were observed with questionnaire. Data were analyzed by chi-square with ?=0,05. The results showed that aircraft noise (p=0,009; OR=4,929), duration of working period (p=0,026; OR=8,308), and flight hours (p=0,006; OR=3,222) were related with hearing disorder, but not age (p=0,093; OR=3,222). We concluded aircraft noise, duration of the working period, and flight hours are related to hearing disorder of the pilots. Keywords: hearing disorder; pilot; noise; noise-induced hearing loss

Abstract

Cataract is the leading cause of blindness in the world, including Indonesia. Senile cataract is the most common type of cataract, caused by the aging process, and involving many factors such as genetic, ultraviolet radiation, dietary factors, dehydration, and smoking. Smoking is one of the causes involved in the maturity of cataract release due to the accumulation of the 3-hydroxykhynurine molecules and chromophores that have roles in changing the color of the lens. This research aimed to find the relationship between smoking levels and senile cataract severity. This is descriptive correlative research with a cross-sectional approach. The subjects were senile cataract patients with smoking habits at the cataract and refractive surgery clinic of Cicendo Eye Hospital Bandung, Indonesia. We recruited 337 patients with purposive sampling technique. Data were collected through interviews and medical record data. Data were analyzed using Chi-square statistical analysis. We found that 61.7% of the patients were light smokers and 66.2% had immature senile cataracts, and the statistical analysis found a significant correlation between smoking level and senile cataract stadium (p=0.000. We concluded that there is a correlation between smoking level and senile cataract severity. Keywords: cataract; senile cataract severity; smoking level

Abstract

The previous study shows ethanol extract of Filicium decipiens leaf possesses a strong antibacterial activity against Staphylococcus aureus, but there have been no studies of the antibacterial activity with different solvents. The purpose of this study was to determine the antibacterial activities of methanol, n-hexane fraction of methanol and n-hexane extract of F. decipiens leaves against S. aureus and Bacillus subtilis bacteria. Extraction was carried out by soxletation method using methanol and n-hexane solvent. The antibacterial activity test was done by measuring the minimum inhibitory concentration (MIC) and Zone of Inhibition. The results showed that methanol extract did not have activity either against S. aureus bacteria or against B. subtilis. N-hexane extract which was fractionated from methanol extract had no activity against B. subtilis but showed moderate activity against S. aureus with 9 mm inhibition diameter at an extract concentration of 40%. N-hexane extract has very strong antibacterial activity against S. aureus with inhibition diameter reaching 21 mm at extract concentration of 30% and showing strong activity against B. subtilis bacteria with inhibition diameter of 10 mm at extract concentration of 30%. It can be concluded that n-hexane extract from F. decipiens has a potent as natural antibiotic material.
