Abstract

One of male contraception methods in family planning program is vasectomy; however, its achievement has been still far behind due to several obstacles in its implementation. In Bantul, Dlingo is a rural sub-district with the highest achievement of vasectomy participants during the last three years (2007-2009). Such a condition is contrary to that found in Sewon as an urban sub-district. This study aimed to identify factors influencing the differences in the achievement, and its results would be beneficial in improving the participation level of vasectomy male contraception. This was a qualitative study comparing the conditions of the two sub-districts through the use of primary and secondary data. Subjects in the research considered information accuracy and validation; hence, its number was subject to the expected results. Data validity testing was performed by utilizing triangulation. Data analysis techniques involved data reduction, presentation, and verification, or tentative conclusion drawing possibly answered the research problem. The result of the research which influenced vasectomy male participation involved un-wealthy or limited economic conditions. This was supported by officers’ attitude and behavior, peer support, public/ religious leaders and community association that actively provided greater contribution in improving participation, and association as the media of socialization and motivation. The results recommended data updating, dialogues with public leaders/religious leaders, and also socialization concerning vasectomy. Tomaintain and increase the male contraception participation level, it is necessary to perform establishment, activation, supervision on the association of male contraception participants, with regard to welfare improvement. Recognition awarded to vasectomy participants and Family Planning Field Officer for their achievement is also necessary.Keywords: Vasectomy, Socialization, Recognition

Abstract

Medicines with a narrow therapeutic range such as phenytoin require individual dose, monitoring, and strict assessment of the patient to maintain the safety of the patient. It is because small changes in systemic concentration can lead significant changes in the pharmacodynamic responses, such as subtherapeutic or toxic. This study aimed to determine the relationship between dose and phenytoin levels in the blood included clinical outcome of patients with epilepsy based on the duration of seizure-free at the Sleman Regional General Hospital, Yogyakarta. This study was a descriptive observational study. The sampling method was purposive sampling. The data was collected retrospectively on epilepsy patients with phenytoin treatment both inpatient and outpatient from January 2010 to December 2012, male or female who were routinely controlled for at least 6 consecutive months. Patients were grouped into monotherapy and combination therapy groups. Clinical outcome was observed through the duration of the seizure-free and devided into 2 categories i.e. < 6 months (poor outcome) and ≥ 6 months (a good outcome). The results showed that male (53.6%) have a greater percentage than female (46.4%). Estimated blood levels of phenytoin was in the average 5.30±4.03 mg/L. Based on the duration of seizure free, the results showed that the percentage of patients in monotherapy group had good and poor outcomes with 71.83% and 28.17% respectively, while only 50% patients in combination therapy group had good outcome.Keywords: phenytoin, epilepsy, nonlinear, clinical outcome

Abstract

Health is one of human wealth indicators so it becomes a priority in developing of a nation. Pharmaceutical industry is important to support that goal. This research was aimed to measure the influence of machinery and work force on production of Epexol tablet in PT. Sanbe Farma Bandung and to determine the most dominant variable. Data were collected retrospectively from production record batch of Epexol tablet in 2012. Data were analysed using a multiple linear regression followed transformation using Cobb-Douglass of production functional equation. Data were processed using SPSS software. A free variable influence towards dependent variables was analysed using an F-test and t-test with a confidence level of 95% (α = 0.05). The result showed that significant value of F-test was 0.0001, while significant value of t-test were 0.028 and 0.010 for machinery and work force variable respectively. An R determination coefficient value was 0.789, and R square was 0.623. It means that machinery and work force variables simultaneously influenced 62.3% of production of Epexol tablet. Cobb-Douglass equation function was Q =304979,55X10,039X20.059. In conclusion, there was a simultaneous and partial significant influence between machinery and work force variables towards total production of Epexol tablet. A dominant variable that affected in the number of Epexol production was a work force variable.Keywords: Machinery, Work force, Production, Epexol

Abstract

Although the number of pharmacists in primary health care is limited, counseling is very important. The Government obligates primary health care to use poster as a health promotion method. In this study, counseling and poster were used to inform patients about hypertension and its therapy. The objective of this study was to compare the influence of counseling and poster towards adherence and therapeutic outcome on hypertensive patients in Bakti Jaya Primary Care, Depok. Research was conducted by quasi experimental using non-equivalent control group design. Samples were hypertensive patients at Bakti Jaya Public Health Care Depok collected from March to May 2012. Samples were divided into two groups i.e. counseling and poster groups. In the first group, pharmacist gave counseling to the patients and in the other group, the patients were only given poster. Patient’s adherence and blood pressure as therapeutic outcome were measured before (pre test) and after intervention (post test). Patient’s adherence was measured using Morisky Medication Adherence Scale (MMAS-8) questionnaire. This study showed that counseling enhanced adherence (P=0.000) and decreased systolic and diastolic blood pressure (P=0.010 and P=0.018), whereas poster only increased adherence (P=0.028). The differences of MMAS-8 score between counseling and poster group were significantly different (P=0.017), and the decreasing of systolic and diastolic blood pressure between counseling and poster group were not significantly different (P=0.170 and P=0.410).Keywords : Adherence, Hypertension, Counseling, Poster

Abstract

The prevalence of epilepsy in the world reachs 5-20 per 1000 population. In Indonesia, the prevalence of epilepsy is approximately at 0.5-1.2%. Antiepilepsy drug has an important role in the epilepsy treatment. This study was aimed to know the effectiveness and safety of monotherapy treatment compared to polytherapy in terms of seizure frequency, seizure-free duration, and adverse drug reaction. This study was the observational research. The data were collected prospectively using instruments i.e.: seizure diaries and APS (Adverse Drug Reaction Probability Scale). Subject of this study were epileptic patient with idiopathic generalized tonic clonic who receive either polytherapy or monotherapy in neurological clinic of Dr. Sardjito Hospital on May-June 2012 period, which fulfilled the inclusion criteria. The data from monotherapy and polytherapy group was analysed in term of seizure frequency, seizure-free duration and adverse drug reaction. Result showed that monotherapy treatment more effective in controlling seizure (reduce the frequency of seizure) compared to polytherapy (<0.05). Percentage of seizure free was 83% in monotherapy compared to 48% in polytherapy. The prevalence of adverse drug reaction was about 20% in monotherapy and 14% in polytherapy. Based on the interview, although monotherapy caused more adverse drug reaction than that in polytherapy, the drug adverse reaction in monotherapy was milder and did not annoy patient convenience.Keywords: Epilepsy, Idiopathic Generalised Tonic Clonic, Monotherapy, Polytherapy, Adverse Drug Reaction

Abstract

Pharmacy Department is the highest revenue of Panembahan Senopati Bantul Regional General Hospital, but its performance is never measured, so the success of intangible and tangible asset management in achieving the organisational vision and goals is still unknown. This research was conducted to measure and evaluate the performance of Pharmacy Department at Panembahan Senopati Bantul Regional General Hospital using the balanced scorecard. This research was a descriptive non-experimental study. Data were obtained retrospectively and prospectively. Qualitative data were acquired through questionnaires and in-depth interview with Head of the Pharmacy Department. Quantitative data were attained through direct observation, prescription survey, and financial reports. Quantitative data were statistically analyzed using a statistic, while qualitative data were analysed and presented in descriptive form. Performance of learning and growth perspective showed that from human capital view, the proportion of pharmacists and bed was not ideal (1:36); the percentage of employee training had not reached 100%; and the average of employee morale was high (3.18). The percentage of Organization Capital was 75.71% which showed the high culture, leadership, teamwork, and alignment. Meanwhile, Information System Capabilities needed the addition of expired date and drugs dosage databases, internet, and the development of management information system. The performance of internal business process perspective showed that the average of drug availability was 98%; the average dispensing time was 37 minutes for compounded prescription and 29 minutes for non compounded prescription; the average SOP suitability with accreditation standards of hospital was 74%; and there was no documentation for medication error. Performance of the customer’s perspective showed the percentage of patients encountered was 80.97%; the rate of customer satisfaction was 86%; and the rate of customer growth was 1% per month. Performance of the financial perspective found that TOR value was 9 times per year in 2011 and 7 times per year in 2012; the rate of service revenue in 2011 was 35% and 28.35% in 2012; Gross Profit Margin was 26.29% in 2011 and 24.83% in 2012.Keywords: Performance, Balanced Scorecard, Pharmacy Department

Abstract

Pharmacy department at Yukum Medical Centre Hospital (IFRS YMC) Central Lampung can not avoid the effects of tight business competition, so it needs to improve the services quality continuously. The purpose of this research was to analyse stakeholder satisfaction of pharmacy department services, and to formulate strategies to increase the stakeholder satisfaction. This research was a descriptive study with qualitative and quantitative data. Qualitative data were obtained by in-depth interviews, whereas quantitative data were acquired using questionnaires to measure satisfaction of stakeholder and pharmacy department staffs. The internal and external environments were analyzed, followed by SWOT (Strengths, Weaknesses, Opportunities, and Threats) analysis. The result was used as data to formulate strategies followed by Quantitative Strategic Planning Matrix (QSPM) test to sequence priorities of the chosen strategies as strategy development of IF RSYMC Central Lampung. The result stated that IFRS YMC Central Lampung had bigger opportunities (average score=1.36) than threats (1.27), and had smaller strengths (1.69) than weaknesses (1.73). The priority of alternative strategies based on QSPM test was developing technologies to support services, following national or international accreditation for hospitals, improving the capacity and quality of human resources in IFRS, and formulating a written standard operating prosedure to whole service in IFRS YMC Central Lampung.Keywords : SWOT analysis, QSPM test, Pharmacy Department,

Abstract

Bupivacaine is used as a local anesthetic with Epidural Lumbal Anasthesia (ELA) for Caesarean Section (CS) in Indonesia. During pregnancy, there are changes in pharmacokinetic parameter values, as a result of changes in the physiology of the body. This study was designed to determine the pharmacokinetic parameters of isobaric bupivacaine 0.5% with non-compartment model and to monitor its effectiveness. The study was conducted on 7 normotensive pregnant patients who treated with 0.5% bupivacaine doses 75 mg at Integrated Central Surgery Building in Dr. Sardjito Hospital. Blood samples (3 mL) from each patients were drawn at the following intervals: 0, 15, 20, 25, 30, 45, 60, 75 and 90 minutes, then the plasma bupivacaine consentration were measured using HPLC and pharmacokinetic parameters were calculated with non-compartment models, i.e. AUC(0-~), AUMC(0-~) and the MRT. Monitoring the effectiveness was conducted by Pinprick test. Relationship of AUC(0-~) with maximum sensory block was analyzed by Pearson correlation. The results of pharmacokinetic parameters of isobaric bupivacaine 0.5% for 7 normotensive pregnant patients with non-compartment model were AUC(0-~) 101,25 ± 15,07 μg.mL-1.min; AUMC(0-~) 6.431,92 ± 2.212,96 μg.mL-1.min2 and MRT 63,74 ± 20,52 minutes. Bupivacaine 0,5% doses 75 mg as an anesthetic for CS with ELA in 7 normotensive pregnant patients was effective for pain blockade, with maximum sensory block was T6. Abstract

Epilepsy is a chronic disease in neurology and the second most common disorder after stroke. Valproic acid is categorised to the medicine with a narrow therapeutic index which requires monitoring of plasma drug levels and dose adjustments to prevent toxic effects. This study aimed to determine the estimated value of valproic acid concentration in serum and clinical outcomes in paediatric patients with epilepsy observed from seizure-free duration at Sleman Regional General Hopital, Yogyakarta. This study was a retrospective study with descriptive observational design. Subjects were paediatric patients with epilepsy who received valproic acid treatment over the period of January 2010-December 2012, 1 to 18 years old, boys or girls who were routinely controlled for at least 6 consecutive months. Clinical outcomes were observed through the duration of the seizure-free which were devided into 2 categories i.e. <6 months (unfavorable outcome), and ≥ 6 months (a good outcome). The proportion of 99 patients who met the inclusion criteria were 27.28% less than or equal to 5 years old, whereas 5-12 years and 12-18 years age groups had the same percentage which was 36.36%. From the calculation, average estimated value of valproic acid concentrations in serum for girls group were as much as 86.32±49.25 mg/L and 33.26±22.06 mg/L, while in boys group were as much as 94.91±44.72 mg/L and 38.57±20.72 mg/L. Based on the duration of seizure free, clinical outcome showed that the percentage of patients in monotherapy group had good and poor outcomes with 52.33% and 46.67% respectively, while 61.54% and 38.46% of patients in polytherapy group had good and poor outcome respectively.Keywords: Epilepsy, Valproic Acid, Clinical Outcome, Paediatric

Abstract

The Hospital of Army Semarang is a hospital with a strategic location and its Pharmacy Department Swadana has competitive pharmaceutical prices. But, patients from the surrounding communities visiting for treatment in the hospital is less than those in its competitor. The aims of this research were to analyze internal and external conditions and also to evaluate the quality of service delivery in self financing Pharmacy Departement. This research was non experimental research, used quantitative and qualitative methods. Quantitative research measured patient’s satisfaction measured using SERVQUAL questionnaire. Dispensing time was calculated from duration of receipt service, and compare to standard procedures. Qualitative research analyzed internal and external conditions. Result of the research was formulated in a SWOT matrix used as input for the self financing strategic planning of improving the quality of service delivery in the Pharmacy Department in the Hospital of Army. Results of the research indicated that based on all the three indicators, there were unsatisfaction among outpatients, and medical and paramedical staffs in the Hospital of Army, which could be seen from negative gap in all the dimensions of questionnaires. The duration of receipt service was in accordance with public service standards, and the Pharmacy Department did not have a written service standard procedures. From the SWOT analysis, it can be known that the hospital has strenghts, namely, a strategic location and competitive prices, and weaknesses such as the lack of quality and quantity of human resources, opportunity for government to provide public service, and threats such as an increasingly tight business competition and progression of science and technology. It is recommended that the Pharmacy Department of the hospital should increase the coverage and scope of service delivery through promotion and counseling, improve the quality of service delivery, equip the department with adequate physical infrastructures and improve the quality of human resources through education and training.Keywords: SWOT, SERVQUAL, Pharmacy Department
