Indonesian Journal of Cardiology (IJC) is a peer-reviewed and open-access journal established by Indonesian Heart Association (IHA)/Perhimpunan Dokter Spesialis Kardiovaskular Indonesia (PERKI) [www.inaheart.org] on the year 1979. This journal is published to meet the needs of physicians and other health professionals for scientific articles in the cardiovascular field. All articles (research, case report, review article, and others) should be original and has never been published in any magazine/journal. Prior to publication, every manuscript will be subjected to double-blind review by peer-reviewers. We consider articles on all aspects of the cardiovascular system including clinical, translational, epidemiological, and basic studies. Abstract

Background.Obesity is associated with increase arterial stiffness and el-evated plasma endothelin-1 concentration. However, there is still conflict-ing data regarding the effect of obesity on arterial stiffness and plasma endothelin-1 concentration.Objective.The purpose of the current study was to assess this effect and analyze whether there was correlation between arterial stiffness indices and plasma endothelin-1 concentration.Methods.The design of the study was cross sectional study that compare and correlate arterial stiffness and plasma endothelin-1 concentration be-tween obesity group (n=40) and normoweight group (n=40). Obesity was defined as body mass index = 25 kg/m2. Arterial stiffness was as-sessed by measuring carotid-femoral pulse wave velocity (PWV) dan ß stiffness index (ß) non-invasively using ultrasound method. Endothelin-1 was measured by ELISA.Results.There was no significant difference regarding PWV between obesity and normoweight group (mean ± SD: 809.44 ± 137.77 versus 850.96 ± 211.60 cm/s, p=NS), but ßwas significantly higher in obese group (8.79 ± 3.15 versus 7.28 ± 1.96, p=0.012). PWV and ßcorre-lated significantly with age (PWV: r=0.446, p<0.001, ß: r=0.354, p=0.001), but only ßcorrelated with body mass index (r=0.282, p=0.011) and waist circumference (r=0.312, p=0.005). There was no significant difference between obesity and normoweight group regard-ing plasma endothelin-1 concentration (0.94 ± 0.26 versus 0.95 ± 0.18 pg/dl, p=NS). There are no significant correlations between PWV/ßand plasma endothelin-1 concentration.Conclusions.These findings suggest that the effect of obesity on arterial stiffness is not uniformly seen throughout all arterial region. Obesity im-pact on carotid artery is greater than its impact on aorta, and this impact on arterial stiffness is not mediated by endothelin-1.

Abstract

Background.Hypertension is frequently found in obesity, although the mechanisms for obesity related hypertension are still unclear. Several studies showed the role of endothelin-1 in the development of hypertension asso-ciated with obesity. More recent studies showed the possibility for the role of leptin in obesity related hypertension. In vitro study with human umbilical vein endothelial cells (HUVECs) showed correlation between leptin and endothelin-1. The aim of this study was to clarify the relation-ship between leptin, body mass index (BMI), blood pressure (BP) and plasma endothelin-1 in obese hypertensive subjects.Methods.A cross sectional study was performed at National Cardiovas-cular Centre, Harapan Kita Jakarta, which included 80 Indonesian men. The subjects were divided into lean hypertensive and obese hypertensive. The plasma immunoreactive ET-1 and leptin were determined by ELISA method.Results.ET-1 level and leptin level were significantly higher in obese hy-pertensive subjects compared with lean hypertensive subjects. There are significant correlation between endothelin level with BMI (r = 0.32; p < 0.005). There are also significant correlation between leptin level with BMI (r = 0.533; p <0.01), but there are no correlation between plasma leptin and endothelin-1 level in obese hypertensive men.Conclusions.Leptin and ET-1 level were significantly increase in obese hypertensive subjects compared with lean hypertensive subjects. There is no correlation between leptin and ET-1 level in obese hypertensive subjects.

Abstract

Background. QRS duration has long been established as a strong predic-tor of sudden cardiac death in patients after surgical correction of Tetral-ogy of Fallot (TOF). Several factors has been correlated to QRS duration, i.e severity of pulmonal regurgitation and right ventricular volume after TOF correction. But there is no established relation between right ven-tricular function and QRS duration after surgical correction TOF.Objectives. We aimed to investigate the relation between QRS duration and right ventricular function and whether patients after TOF repair with right bundle branch block have left ventricular dissynchrony.Methods.50 patients with repaired TOF were studied. Right ventricular function were derived from tissue Doppler imaging Tei index. Left ven-tricular dissynchrony were derived from tissue Doppler imaging. The time interval between the onset of QRS complex and the onset of systolic ejec-tion was measured for six left ventricular basal wall, i.e septal, lateral, anteroseptal, posterior, anterior and inferior wall. Mechanical delay from septal to lateral wall, antero-septal to posterior wall and anterior to infe-rior wall was calculated. Measured data were correlated with QRS dura-tion.Results.There was relation between QRS duration and right ventricular function (P = 0.028, r = 0.44). Ten (20%) of the examined patients after TOF repair had LV dissnynchrony particularly in patients with RBBB and prolonged QRS duration (160 ± 9 ms).Conclusions.There is correlation between right ventricular function and QRS duration. Left ventricular dissynchrony may exist in patients after TOF repair with RBBB.

Abstract

Background. Inflammation has been linked to all stages of the develop-ment of vulnerable plaque which not only causes platelet activation but also proceeded by activation of polymorphonuclear neutrophils (PMNs). Myeloperoxidase (MPO), a hemoprotein abundantly expressed by PMNs and secreted during activation, possesses potent proinflammatory prop-erties and may contribute directly to tissue injury. Among predictors of major adverse cardiovascular events (MACE) that we have been widely used and known, such as hs-CRP and Troponin T, it still unknown whether MPO also provides prognostic information in patients with acute myocar-dial infarction (AMI). Objectives. The aim of our study is to search predictive value of MPO and to compare MPO with hs-CRP and Troponin T as a predictor of MACE in patients with AMI.Methods.Cohort-prospective study was done, conducted from April to June 2006 in the emergency department of National Cardiovascular Cen-tre - Harapan Kita, (NCVC-HK), Jakarta. MPO, hs-CRP, and troponin T serum levels were assessed in 93 patients with AMI. It recorded death, re-infarctions, angina, revascularization, and heart failure during 6 months of follow-up.Results.Patients with MPO serum level > 204,9 µg/l have increased risk of cardiovascular events (HR 6.76; 95% CI 3.37-13.56, P < 0.001). In statistical analysis, MPO (sensitivity 83.1%; specificity 82.4%) is a stronger independent predictor for subsequent cardiovascular events than Tropo-nin T (sensitivity 59.8%; specificity 71.6%) and hs-CRP (sensitivity 48.5%; specificity 46.1%).Conclusions.MPO serum levels powerfully predict an increased risk for subsequent cardiovascular events in patients with acute myocardial infarction.

Abstract

Background. Congenital heart disease (CHD) is a significant cause of mortality in pediatric patients. Meanwhile, malnutrition remains a public health problem in Indonesia. Combined with infection, these condition will increase mortality rate of patients with CHD.Objectives.The objective of this study is to describe the cause of mortal-ity among CHD patients who were hospitalized in pediatric ward, Doctor Soetomo General Hospital, Surabaya during period 2004-2006. Methods. This study is a descriptive evaluation base on Medical Record data.Results. Of patients hospitalized from 2004 to 2006, 189, 185, and 186 respectively grouped into neonatal, infant, and childhood age. Medical record data in 3 years was described into distributive frequency and cross table. Ventricular septal Defect (VSD), Tetralogy of Fallot (TOF), Atrial Septal Defect (ASD), and Patent Ductus Arteriosus (PDA) were the four major CHD. Hospital mortality rate was 11,64% (2004), 11,35% (2005), and 13,44% (2006), infant were the main portion 72,73% (2004), 76,19% (2005), 64% (2006). Respiratory and cardiac failure were two leading cause of death. Fifty percent of death case was underlied by pnemonia and bron-chiolitis. VSD, ASD and TOF were the most common CHD associated with mortality. Death rate was significantly high in malnourished patients, from 2004 to 2006: 45,45% (10/22), 66,67% (14/21), and 72% (18/25) respectively.Conclusions.High mortality rate in our patients with with CHD were associated with age (first year of life), respiratory infections (pnemonia, bronchiolitis), and malnutrition.

Abstract

Penyakit Kawasaki merupakan penyakit akut, vaskulitis yang sembuh sendiri (self-limited).Penyakit ini menyerang bayi dan anak-anak, penyebabnya tidak diketahui. Tomisaku Kawasaki menemukan kasus pertama tahun 1961 pada anak usia 4 tahun yang menderita demam dengan bercak kemerahan. Pada tahun 1967, Kawasaki melihat 50 pasien dengan gejala yang sama, yaitu demam akut disertai eksantema dan menegakkan diagnosisnya sebagai mucocutaneus lymph node syndrome.Penyakit tersebut mulai dikenal luas di luar Jepang sejak tahun 1974 setelah Kawasaki melaporkannya dalam suatu jurnal di Inggris.

Abstract

Cedera dan disfungsi endotel merupakan kejadian yang mendasari patogenesis proses aterosklerosis, sehingga pemahaman terhadap mekanisme yang dapat mem-pertahan-kan maupun mengembalikan fungsi endotel tersebut mempunyai implikasi klinis yang penting.Selain itu, tingkat kejadian gagal jantung kronis sebagai akibat penyakit jantung koroner masih tetap tinggi. Penyelamatan miokard dengan terapi perfusi dini terbukti dapat menurunkan angka mortalitas, namun gagal jantung pasca infark miokard akibat remodeling ventrikel masih menjadi masalah yang belum terpecahkan sepenuhnya.

Abstract

Trombosis pada katup prostetik jarang terjadi, namun dapat mengancam jiwa pasien dengan penggantian katup. Komplikasi ini lebih sering terjadi pada katup prostesis mekanik, meskipun kejadiannya hanya 0,3-4,3 % pertahun.Trombosis katup prostetik ditandai dengan adanya trombus, tanpa disertai infeksi, yang melekat pada atau dekat katup, yang menyumbat aliran darah atau mengganggu fungsi katup. Hal ini tergantung pada jenis katup (katup plat berdiri atau katup bola), struktur (katup mekanik), lokasi (mitral) dan kepatuhan pasien terhadap penggunaan obat anti koagulan.

Abstract

Thyroid heart disease is one of the many and frequent complication of thyrotoxicosis, which also manifest in extra cardiovascular system. Opthalmopathy is a well-known and well-studied complication of thyro-toxicosis, and another is dermopathy, which manifests as pretibial edema.This case presentation describes a patient of long standing thyrotoxicosis with thyroid heart disease. In this particular case, the patient developed unilateral pretibial edema which posed a diagnostic challenge.In dealing with that, it would be important to con-sider several differential diagnosis. Some of the differ-ential diagnosis of unilateral pretibial edema are actu-ally not proven due to limited hospital stay, but an analysis of differential diagnosis will be presented and hopefully will lead to better recognition of clinical symptoms and signs of thyrotoxicosis, its complica-tions and lead to appropriate diagnostic studies.
