Abstract

Background: CPT is a disorder characterized by nonunion tibial fracture that occurs spontaneously or after trivial trauma. Therapy usually includes surgical management and aims to gain lifetime bony union, avoid leg length discrepancy, and to prevent mechanical axis deviation, surrounding soft tissue lesion, joint stiffness, and pathological fracture. CPT remains to be a challenging orthopedic issue due to the difficulty to gain and to maintain the union and functional limb. The combined VFG and ESIN is one of our preferred method in Surabaya. Case: A 13-year-old boy complained about crooked right lower leg since 1-year old, and fractured at 11 years old. In January 2014, the patient underwent ORIF ESIN combined with free VFG. The patient got bony union 24 months after the ORIF ESIN and VFG surgery. RUST modified scoring system showed a score out of 3, which signified that there was a radiographically significant union at the site of tibial pseudoarthrosis. Discussion: The combined VFG and ESIN is to maintain bony union and stability. Intramedullary fixation offers a good tibial alignment and prevents refracture. Hypertrophy and the vitality of VFG support the bony union. The level of success rate in both primary and secondary union is a benefit shown after VFG therapy. Conclusion: Combination of VFG after complete resection of tibial pseudoarthrosis and ESIN in this case report showed a success in achieving and maintaining the bony union, thus this therapy became one of the preferences as a suitable standard therapy that we use in our hospital institution.

Abstract

Background: Peripheral nerve injuries (PNI) are quite common, with an incidence of 13.9% per 100,000 people per year. The most cause is direct penetrating trauma. PNI will cause a significant decrease in the quality of life of patients. Case: A 21-years-old woman complained of the inability to move her right leg and numbness. She had been injured in the of the right thigh due to broken glass. The patient underwent emergency surgery to repair the right Femoral artery and the Sciatic nerve. Discussion: After six months since the first operation, flail foot and anesthesia were persisting with a FADI score of 47.1. We decided to perform re-exploration, from the exploration we found neuroma on the sciatic nerve. Excision was performed on the Neuroma then an end-to-end anastomosis was performed on both sciatic nerve components (tibial and peroneal) and augmented using amniotic membrane composites and mesenchymal stem cells. After 6 months since the second surgery, the patient could perform ankle dorsal flexion. After 12 months postoperatively, the patient could move his right leg functionally, sensory function also improved. Patients can undergo daily activities without difficulty, with a FADI score of 96.2. Conclusion: There is a significant improvement in the patient's foot function after surgery using the method we used, more research is needed to optimize the use of amniotic membrane and mesenchymal stem cells as augmentation in nerve repair.

Abstract

Background: In recent years, Freeze-Dried Scaffold Bovine Cartilage has been widely used as an alternative therapy for joint cartilage defects. This study aims to determine the biocompatibility of scaffold without involving implantation which provides clinical reports as expected through the evaluation of post-implantation chondrocytes regeneration, biocompatibility markers of the scaffold, and biocompatibility of sponge cartilage scaffold involving cartilage defects New Zealand White Rabbit. Methods: This experimental in-vivo study was conducted for four weeks. Rabbits were divided into 4 treatment groups: microfracture defect group with DFLP sponge cartilage scaffold (P1) implantation; Microfracture defect group with DFLP sponge cartilage scaffold-secretome implantation (P2); Microfracture defect group with DFLP sponge cartilage scaffold-adipose derived Mesenchymal Stem Cells (ADMSCs) (P3); Microfracture defect group without implantation (control). The evaluations of basophil, eosinophil, neutrophil, and polymorphonuclear (PMN) cells were done in the first 24 hours, 3 days, and 1 week after the treatment. The collected data will be analyzed statistically. Results: Research observations performed three times in the first, third, and seventh days. The results showed a small number of average Neutrophil (Neutrophil granulated) and PMN (segmented Neutrophils) cells both in the P2 and P3 groups compared with the control and the P1 group. Conclusion: In general, biocompatibility is not included on the cytotoxic effects including inflammatory reactions and post-cartilage scaffold sponge implantation (DFLP) with or without the addition of ADMSC and secretome in the white rabbit New Zealand cartilage defect associated with differences seen in eosinophils, basophils, neutrophils, also total PMN cells in four groups.

Abstract

Background: Bone grafting is a surgical procedure that involves transplanting material to enhance bone healing through osteogenesis, osteoconduction, and osteoinduction. While autografting has long been considered the gold standard, it has limitations, including a restricted supply and donor site morbidity. These limitations, along with those associated with allografts, have led to the development of bovine hydroxyapatite (BHA) as a bone graft substitute. This study aimed to evaluate the clinical and radiological outcomes of BHA.Methods: This descriptive study used a consecutive sampling design, including all trauma patients who received BHA at our hospital between 2016 and 2018. The BHA was obtained from the tissue bank at our institution. The clinical and radiological outcomes were evaluated, and the data was tabulated and analyzed descriptively.Results: Of the 56 patients who underwent surgery with BHA, most (80.36%) had excellent outcomes, 12.5% had good outcomes, 3.57% had fair outcomes, and 3.57% had poor outcomes.Conclusion: Bovine hydroxyapatite can be considered an alternative bone graft to support the bone healing process.

Abstract

Background: Cervical radiculopathy is a clinical condition characterized by unilateral arm pain, numbness and tingling in a dermatomal distribution in the hand, and weakness in specific muscle groups. This condition can be treated with nonsurgical or surgical methods. This study aims to evaluate the outcome of the stand-alone cervical cage surgical method for cervical radiculopathy at our hospital institution from 2013 to 2017.Methods: This is a retrospective observational study on every patient who underwent a stand-alone cervical cage procedure for cervical radiculopathy at our hospital institution from 2013 to 2017. The study was conducted from December 2017 until April 2018. We evaluated the clinical outcome with the Neck Disability Index (NDI). The data were collected from medical records, and postoperative follow-up was done by house visits, phone calls, and outpatient visits.Results: Five male and one female subject with a mean age of 58 years (range, 45–65 years) underwent the procedure; one patient passed away three years postoperatively due to events unrelated to the operation; one patient could no longer be reached. Four patients had an increasing NDI score postoperatively. There were no postoperative complications.Conclusion: Stand-alone cervical cage appears to be a safe and effective treatment, providing a favorable clinical outcome for cervical radiculopathy at our hospital institution from 2013 to 2017.
