Abstract

Introduction: Burns is a traumatic condition with a very high mortality rate. In an effort to prevent greater mortality in burn conditions, a predictor is needed to optimize the prediction of death in burn patients. This study aimed to determine the efficacy of red blood cell distribution width ratio (RDW) to platelets (PLT) as a predictor of mortality in burn patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: This study was a retrospective cohort analytic observational study. A total of 51 research subjects participated in this study. Observation of clinical data and sociodemographic and laboratory evaluations were carried out. Data analysis was carried out with SPSS in univariate, bivariate, and diagnostic tests. Results: The RPR diagnostic test on observation ≥ 7 days is more sensitive and specific than that on observation ≥ 3 days. This study shows that both observations ≥ 3 days and observations ≥ 7 days show similarity, where the RPR value is much higher in cases of mortality than in cases of alive. Conclusion: RPR value ≥ 7 days has prognostic value with better sensitivity and specificity in predicting mortality in burn patients.

Abstract

Introduction: Distal radius fracture is the most common fracture in an orthopedic emergency involving the upper limb. Management of patients with distal radius metaphyseal fractures greatly affects the function of the upper limb during recovery. Then the right choice of type of procedure will affect the function of the upper limb. This study aimed to compare the function of the upper limb in intraarticular distal radius metaphyseal fracture between operative and non-operative procedures with the clinical assessment of the QuickDASH score at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: This study was an analytic observational study with a retrospective cohort approach. A total of 55 research subjects consisting of 38 subjects belonging to the operative group and 17 subjects belonging to the non-operative group, participated in this study. Data analysis was carried out using univariate and bivariate to find out the differences between operative and non-operative procedures related to post-fracture upper limb performance. Results: Operative procedure are proven to improve upper limb performance better than non-operative procedure based on the QuickDASH score. Operative procedures provide improved upper limb performance 2.65 times better than non-operative procedure. Conclusion: Operative procedure that are better in maintaining upper limb performance after intraarticular radius distal metaphyseal fractures based on the QuickDASH score compared to non-operative procedure at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia.

Abstract

Introduction: Hemangiomas that have undergone involution, also have a risk for the formation of residual scar tissue or scar, as well as hemangiomas undergoing excision therapy. Scar, especially those that occur in areas that are visible or open, certainly has implications for the patient's quality of life. This study aimed to determine the level of satisfaction of hemangioma patients who received propranolol therapy with excision therapy based on the scar results at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: Cross-sectional analytic observational study. A total of 95 research subjects participated in this study. The level of satisfaction is based on the severity of the lesion scar. The family assessment was assessed using a Likert scale consisting of the following assessment items itching, pain, color, thickness, and shape. Data analysis was carried out with the help of SPSS version 25 in univariate and bivariate. Results: Research subjects who received propranolol therapy had different levels of satisfaction from research subjects who received surgical therapy. Research subjects who received propranolol therapy had a higher satisfaction level of 1.71 higher than those who received surgical therapy. Conclusion: There are differences in the level of satisfaction of hemangioma patients who received propranolol therapy with excision therapy based on the scar results at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia.

Abstract

Introduction: A free flap is part of reconstructive surgery. This technique takes tissue or bone flaps along with their original vascular supply and performs microvascular anastomosis of the pedicle arteries and veins at the recipient site. Several risk factors that influence the occurrence of flap failure are still a major concern, such as age, gender, defect size, duration of surgery, length of tissue ischemic time, nutritional status, etc. This is further associated with higher morbidity, the potential for flap failure, higher costs, and increased length of stay in the hospital. This study aimed to determine the factors associated with free flap viability at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: Cross-sectional analytic observational study. A total of 33 research subjects participated in this study. Observation of risk factors was carried out using SPSS univariate and bivariate. Results: Gender and ischemic time factors are related to free flap viability. Male gender and ischemic time < 60 minutes increase the success of free flap viability. Conclusion: Gender and length of ischemic time are associated with free flap viability at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia.

Abstract

Background: Vitamin D has a role in diabetes mellitus. Low levels of vitamin D are associated with the development of diabetic foot infections. Effects of vitamin D supplementation on wound healing, insulin resistance, inflammatory biomarkers, and oxidative stress are mediated by effects on induction of phagocytosis and bacterial killing by macrophages, suppression of interferon-gamma mediating macrophage activation, activation of insulin receptor expression, and downregulation from cytokines. This study aimed to determine the correlation between serum vitamin D levels and the severity of diabetic foot ulcers in Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: This study was an observational analytics study, where as many as 30 research subjects participated in this study. The study observed vitamin D levels and the severity of diabetic foot ulcers as assessed by the classification of the wound, ischemia, and foot infection (WIfI). Univariate and bivariate data analysis was carried out using SPSS. Results: The results of Sommer's correlation statistical test obtained a value of p = 0.001 (p <0.05) with a correlation strength of r = 0.564, which means sufficient strength (r = 0.40-0.70). This means that the higher the deficiency in serum Vitamin D levels, the higher the degree of WIfI classification experienced. Conclusion: There is a correlation between serum vitamin D levels and the severity of diabetic foot ulcers in Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia.

Abstract

Introduction: Proximal humeral fractures are one of the most common long bone fractures due to osteoporosis. The need for operative action can provide greater benefits than non-operative measures. However, the difference between the need for operative and non-operative measures in proximal fractures is still controversial. This study aimed to determine differences in DASH (disability of the arm, shoulder, and hand) scores as a function of assessing upper extremity patients with proximal humeral fractures after operative and non-operative management at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: Cross-sectional analytic observational study, in which 37 research subjects participated in this study. Univariate and bivariate analyses were carried out using SPSS software on the patient's sociodemographic data as well as the clinical data of the study subjects. Results: There was no statistically significant difference in upper extremity function (DASH score) between operative and non-operative procedures in patients with proximal humeral fractures, p> 0.05. Conclusion: There was no difference in upper extremity function as assessed by the DASH score in operative and non-operative procedures in patients with proximal humeral fractures at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia.

Abstract

Introduction: Planned mandibular reconstruction with the help of morphometry allows plastic surgeons to perform operations with greater precision. They can plan bone shifts and cuts more accurately, minimize damage to surrounding tissue, and achieve cosmetically and functionally better results. This study aimed to compare the morphometric variation of ramus length and mandibular corpus length in patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia, based on gender. Methods: This study was a cross-sectional analytic observational study. A total of 96 research subjects participated in this study, where the length of the ramus and corpus of the mandibular was obtained by assessing the 3D CT scan of the mandibular. Data analysis was carried out using SPSS in a bivariate. Results: The length of the mandibular ramus dextra and sinistra shows that the male gender is longer than the female gender, where the p-values are 0.000 and 0.001, respectively (p<0.05). The length of the mandibular corpus dextra and sinistra did not show a statistical difference in length, where the p-value was > 0.05. Conclusion: There is a difference in the length of the mandibular ramus between men and women, but there is no significant difference between the length of the corpus mandibular between men and women.

Abstract

Introduction: Closed thoracic trauma is frequently associated with rib fractures and pulmonary contusions. Thoracic trauma can be life-threatening if it is not treated immediately and properly. This study aimed to determine the relationship between thoracic trauma scoring and mortality in blunt thoracic trauma patients in sub-population Indonesia, especially at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: This study is a cross-sectional observational study. A total of 70 research subjects participated in this study. Data analysis was carried out using SPSS in univariate and bivariate to determine the relationship between thoracic trauma scoring and mortality. Results: There is a significant relationship between CTS scores and mortality (p = 0.000). Patients with a CTS score ≥ 5 were significantly more at risk of dying than patients with a CTS score < 5 (p = 0.000). Conclusion: There is a relationship between CTS score and blunt trauma patient mortality thorax at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia.

Abstract

Introduction: Breast cancer is the second most common cancer in the world and is the most common cancer among women. Cancer is a term used for malignant tumors, which are tumors that grow rapidly, infiltrate surrounding tissues, metastasize, and can cause death if they don't get proper treatment and therapy. One of the treatments for cancer is chemotherapy; the negative effects of chemotherapy treatment for patients can affect life. This study aimed to find out in depth the life experiences of women with breast cancer undergoing chemotherapy. Methods: The research design used is qualitative research using a phenomenological approach. The technique used is purposive sampling, with a total of 10 participants. This research was carried out at the Jayapura General Hospital in April 2023 using the in-depth interview method. Results: Subjects felt the side effects during chemotherapy, the impact during chemotherapy, adapting to reduce physical changes after chemotherapy, the support received during chemotherapy, and expectations of chemotherapy. Conclusion: Chemotherapy provides hope for patients but also causes uncomfortable side effects for patients. Efforts to educate and strengthen chemotherapy patients are important.

Abstract

Introduction: Maxillofacial trauma is an important public health problem, and understanding the relationship between injury severity and patient outcomes is critical for effective management. This study aimed to determine the relationship between the maxillofacial injury severity score (MFISS) and the outcomes of maxillofacial trauma patients at Dr. Mohammad Hoesin General Hospital, Palembang. Methods: An analytical observational study was conducted involving 121 maxillofacial trauma patients from April to May 2023. Data regarding patient characteristics, MFISS scores, type of management, length of stay, and mortality were collected and analyzed using statistical tests. Results: The majority of maxillofacial trauma patients were male (74.4%) and aged over 18 years (63.6%). Traffic accidents are the main cause of trauma (84.3%). The distribution of MFISS scores showed that 75% of patients had mild scores, 18.2% had moderate scores, and 5.8% had severe scores. Most patients underwent surgical management, with the majority having a length of stay of ≤7 days. The mortality rate was 2.5%. There was no significant relationship between the MFISS score and type of management (p > 0.05). However, there was a significant difference in the length of stay based on the MFISS score (p = 0.000). Patients with severe MFISS scores had significantly longer lengths of stay compared with patients with mild or moderate scores. In addition, there was a significant relationship between MFISS score and mortality (p = 0.000). Patients with severe MFISS scores had a higher risk of death compared with patients with mild to moderate scores. Conclusion: The MFISS score is associated with length of stay and mortality in maxillofacial trauma patients. However, there was no significant relationship between the MFISS score and the type of management.
