Case Report Penile Fracture With Corpus Cavernosum Rupture Without Urethral Disruption Due to Sexual Intercourse

Objective: We reported a case of penile fracture with unilateral corpus cavernosum rupture that underwent penile exploration and reconstruction measures. Case presentation: We reported a 45-year-old male patient complaining of penile pain, swelling, loss of erectile ability and a "pop" sound after bumping during sexual intercourse. The patient can still urinate. Physical examination found a swollen penis, hematom, eggplant-like deformity, and angulation to the right accompanied by tenderness. Routine blood support examination within normal limits. Based on data obtained from the anamnesis, physical examination, diagnosis of penile fracture was established and the patient was treated with exploration and reconstruction of the penis <24 hours. Discussion: Patients carried out exploratory actions carried out with circumferential and degloving subcoronal incisions, 4 cc blood clots were evacuated and ruptures were seen in the tunica albuginea corpus cavernosum sinistra along 4 cm. Reconstruction is carried out with primary sutures on the torn corpus cavernosum. The patient is carried out treatment for 5 days. At the postoperative 7-day evaluation, the surgical wound was good and urination was good. At the evaluation of 1 month the patient can have a painless erection with an erection hardness score of 4, there is no penile deformity. Conclusion: Penile fracture is a urological emergency that shows a good prognosis if you get surgical intervention immediately (<24 hours) and can reduce the number of complications

Objective: We reported a case of priapism fracture in chronic myelocytic leukemia performed winter procedure Case presentation: We reported a 22-year-old male patient with complaints of erect penis continuously for 3 days without being affected by sexual stimulation. The patient feels pain in his penis and can still urinate. The patient denied any previous history of penile trauma, drug use, and similar complaints. Physical examination found an erect penis with EHS 4 and palpable corpus cavernosum stiff and soft. Routine blood support tests show an increase in leukocytes up to 367,000/μL. Examination of peripheral blood images shows an increase in the number of all types of granulocytes from young cells to blast cells with the effect of a chronic leukemia. Intracavernous blood gas analysis showed pH 7.2, pO2 25 mmHg, and pCO2 75 mmHg. Based on data obtained from the anamnesis, physical examination, and supporting examinations, a diagnosis of ischemic or low flow priapism was established. The patient was treated with cavernous blood aspiration using the winter procedure and combined with intracavernous irrigation using the α phenyleprine agonist. Discussion: The patient performed a winter procedure using two 18G needles for cavernous blood aspiration and combined with intracaevernose irrigation using phenylephrine 1 mg diluted saline. Intracavernous blood aspiration gets 40 mL of blood. The action was performed in the operating room for 2 hours with the result of the penis being detumescence with EHS 2. The patient is observed for 2 days and then referred to a hospital with internal medicine specialist facilities, hemato-oncology consultants for further management related to CML. Conclusion: Ischemic type priapism or low flow is a urological emergency that can be managed with winter procedures and shows good outcomes.

Background: Mycobacterium tuberculosis, which causes TB, is a deadly disease that mostly affects the lungs. Surveys revealed that although there was significant variance, the estimated incidence of bacteriologically proven pulmonary TB per 100 000 people aged 15 or older was high in several countries. With multisectoral action and responsibility to address the larger causes that influence the TB epidemic and the socioeconomic effect of TB, knowing and comprehending the sociodemographic features of tuberculosis can serve as a milestone in the End TB Strategy by 2035. Methods: Descriptive cross-sectional methodology was employed in this study to collect data. 154 samples were collected over the course of eight months in accordance with the inclusion criteria; the findings are displayed as a distribution table. Results: The study's results showed that of 154 samples, patients with positive bacteriological TB were more likely to be men than women, to be in their prime working years, to be HIV-negative and to be medication sensitive. RSUP Prof.Dr.I.G.N.G.Ngoerah Denpasar is an advanced service facility which is part of supporting the running of national insurance programs processed by the BPJS agency. The BPJS Health wasin audit aims to ensure that companies or participants have fulfilled their obligations regarding health insurance provided by BPJS Health, where reimbursement for advanced health services costs uses the INA-CGBs E-Claim application. INA-CBG E-Claim is an application specifically designed to assist hospitals in the national health insurance claim process. INA-CBG's is the amount of claim payment by BPJS to health facilities where the INA-CBG's rate uses a coding system with ICD-10 for diagnosis and ICD 9CM for procedures or actions. The aim of this research is to obtain the suitability of INA-CBGs claim coding on BPJS Health wasin at Prof. Dr. IGNGNgoerah Denpasar Hospital. Empirical legal research used in this research, with a quantitative approach. which exists in fact. The results of this research are that the impact of losses from discrepancies in the coding of clinical data affects the financing of health services, so there is a need for a BPJS Health data processing process regarding re-confirmation regarding the advantages and disadvantages of claims that will be paid. In August 2023, there was confirmation that there were 638 inpatient data processing data, 2339 outpatient data. Then action was taken by the hospital to provide training to coding officers and continue to regularly monitor and check the hospital's internal verification.

Background: Neuropathy is a major cause of morbidity and mortality in type 1 and 2 diabetes worldwide. The development of neuropathy and its severity increases with increasing duration of DM and poor glycemic control. Early detection of neuropathy is essential to prevent worse disease complications. Objective: to identify a simple instrument MNSI in detecting peripheral neuropathy of diabetic foot. Methods: Scoping review protocol using 5 steps of Aksey and O'Malley Framework. Inclusion criteria were published in 2016-2023, full text, English language and discussed the use of MNSI in detecting peripheral neuropathy. This article used 4 databases (Pubmed, ProQuest, and ScienceDirect) with the keywords "Michigan screening instrument AND diabetic neuropathy AND early detection". Results: There were 10 articles that fit the inclusion criteria that mentioned that MNSI is a simple tool in detecting peripheral neuropathy in diabetic foot. Conclusion: MNSI is a reliable instrument, easy to do and can be used by doctors, nurses, and health workers involved in the treatment of patients with diabetes mellitus

Work accidents are one of the problems that often occur in the workplace. Work accidents can cause material and non-material losses, both for workers, companies, and society. One of the factors that can cause work accidents is workers' non-compliance in using personal protective equipment (PPE). The purpose of this study was to analyze the effect of compliance with the use of PPE in accordance with the risk of exposure to the incidence of work accidents. The research method used in this research is quantitative research. The data collection techniques used in this study were questionnaires and literature studies. The data that has been collected is then analyzed using the help of the SPSS program. The results showed that compliance with the use of PPE in accordance with the risk of exposure had a significant effect on the incidence of work accidents. Workers who are obedient in using PPE have a lower risk of work accidents compared to workers who are not obedient in using PPE

Breast cancer has a large number of new cases and deaths in the world and Indonesia. Chemotherapy is one of the treatments chosen to treat breast cancer patients. Physical side effects can include Chemotherapy-induced Induced Nausea and Vomiting (CINV), Chemotherapy-induced Induced Peripheral Neuropathy (CIPN) and anxiety. Standard hospital care is care in which nursing actions are in accordance with the Standard Operating Procedure (SOP) that the hospital has established for each existing service. The research method used was quasi-experimental (quasi-experiment), which was carried out with an unequal control group design. The pre-test was carried out during chemotherapy when the researcher met the respondent, and the post-test was carried out during the respondent's following chemotherapy schedule. The population in this study were breast cancer patients undergoing chemotherapy at the chemotherapy clinic at RSD, Dr. Soebandi Jember and Rumkit Kindergarten. III Baladhika Husada Jember . The sampling method used was simple random sampling with a minimum sample size of 60 people. The results of the Paired Samples Test can be concluded that standard treatment in hospitals does not affect the level of Chemotherapy Induced Nausea and Vomiting (CINV) but does influence the incidence rate of chemotherapy-induced peripheral neuropathy (CIPN) and the level of anxiety in breast cancer patients with chemotherapy.

A 16-year-old male patient came to a private dental practice presenting chief complaints about frequent gum bleeding when brushing his teeth. The patient came without his parents. Anamnesis and clinical examination result shows that the patient was suspected of having gingivitis associated with sexually transmitted disease. No medical treatment was performed on the patient because the patient was not accompanied by parents while the patient was not legally able to sign informed consent. The patient was educated about sexually transmitted diseases and asked to come back after taking blood and urine tests, with his parents to accompany him during the examination because the patient. The patient then came back with the results of the blood and urine tests, but without his parents. The dentist cannot provide dental treatment because underage patients must be accompanied by their parents in performing a non-emergency medical treatment. Dentists have the right to postpone the non-emergency medical procedures in private practices for these patients. Dentists are obliged to provide education and refer patients to the hospital for further examination and treatment in accordance with government programs and applicable legal provisions in Indonesia.

The rapid development of science and technology and education also brings changes in human life. These changes have the effect of higher demands on each individual to further improve their performance. In order for their existence to be maintained, each individual will experience stress and frustration, especially for individuals who are less able to adjust to these developments. If you pay attention to people in everyday life, you will see various things that happen among the community. There are also those who often complain and grieve, despair, give up, do not fit in with other people and their work. This makes a person experience a health problem. This happens because of the lack of community for something to maintain harmony in society.
