Abstract

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare, life-threatening conditions with high morbidity and mortality characterized by necrosis and extensive shedding of the epidermis in the vesiculobullous skin, orifice mucosa, and eyes as well, accompanied by severe general symptoms in the patient. SJS/TEN supportive care management varies widely. The difference is in the severity found based on the area of the affected skin surface. The most common cause is a hypersensitivity reaction to a drug. A 50-year-old man went to the emergency room (ER) with complaints of developing blisters, a rash accompanied by extensive peeling of the skin on the face, eyes, lips, chest, back, and genitals. 2 days before the onset of the complaint, the patient had a history of taking medication suggested by a nurse tetracycline and paracetamol. On physical examination, while in the emergency room, epidermolysis in the patient's body was found to be around 40% marked with a positive Nikolsky sign, and the SCORTEN found in this patient was 4. The management of this patient was in the form of keeping the patient's airway clear, administering adequate intravenous fluids, high doses of intravenous steroids, wound care, close patient observation, and care from other multidisciplinary disciplines such as internal medicine, anesthesia, ophthalmology, and surgery.

Abstract

Perineal wounds are tears that occur when the baby is born either spontaneously or by using a tool or action. Administration of amoxicillin and mefenamic acid as pharmacological treatment is not enough for perineal wound healing. An alternative way to heal perineal wounds is using green betel leaf gel, which has the advantage of cold gel properties, so it can reduce pain. The gel has benefits as an anti-inflammatory, antioxidant, antiseptic, and antibacterial, so it can be used as a perineal wound treatment that can help accelerate perineal wound healing. This study aimed to prove the administration of green betel leaf gel (Piper betle) effect on perineal wound healing in postpartum mothers. This study is a quasy experiment with a pretest and posttest with a control group design. Through the technique of purposive sampling, it was found that 32 respondents were divided into two groups, the control group with clean, dry wound care and the intervention group, which was given green betel leaf gel twice a day for 7 days as measured using the REEDA score. Green betel leaf gel is effective for perineal wound healing on the 7th day compared to dry clean treatment.

Abstract

Diabetes mellitus is an endocrine, metabolic disease that can occur in all ages and socioeconomic groups. Uncontrolled diabetes mellitus can cause various complications. One of them is the manifestation of the skin. Chronic lichen simplex in diabetes mellitus patients was reported in cases with uncontrolled blood sugar levels greatly affecting chronic skin lesions in patients. Controlled sugar levels and appropriate topical therapy and psychological management in patients are expected to provide better output and prognosis. The patient came with complaints of itchy, red and painful wounds on both legs, accompanied by a fever for 7 days, Previously the patient often had numbness and tingling in the upper and lower extremities. Patients often scratch their skin, resulting in wounds that then dry out and thicken. The patient also has the comorbid disease of Diabetes mellitus. Complaints have been recurring frequently and have been felt for more than the last 2 years. Itching often recurs, especially when you think about something a lot and when you are resting. Based on the dermatological examination, the results showed that on both lower limbs, there were non-layered smooth white scaly lesions, lichenification accompanied by hyperpigmentation, the size of the plaques had indistinct boundaries, there was slight edema and erythema, multiple erosions with black crusts on the surface of the lesion were not easily removed. Diabetes mellitus is associated with dermatologist disorders. Early recognition of initial cutaneous changes such as xerosis, hyperkeratosis, or skin infections by appropriate management could help reduce late or severe complications of diabetes. Although most dermatological conditions require specific treatment, the most important rule is to improve blood glucose levels control in patients. Education in treatment, hygiene, emotional stress and lifestyle changes are important for improving the quality of life for patients with diabetes mellitus.
