Abstract

Objective: Reporting a comprehensive management of physical medicine and rehabilitation of post operative high S curve-idiopathic scoliosis in adolescent.Methods: Patient was assessed for balance contraction of back muscles with biofeedback (BF), chest expansion and endurance capacity with conversed 6 minute walking test (6 MWT) results to metabolicequivalents (METs), pre and post spinal corrective surgery with Harrington’s rod and pedicle screw.Results: Pre and post surgical and rehabilitation integration programs showed improvements in Cobb’s angle (thoracal/lumbar angle was 64o/32o became 20o/5o), chest expansion, spinal muscular balance andstrength, and endurance capacity reflected by METS (5.5; 5.78; and 6.2 respectively).Conclusion: There were improvements in Cobb’s angle, chest expansion, spinal muscular balance and strength, and endurance capacity 6 months post therapy.Keywords: scoliosis, Cobb’s angle, biofeedback, chest expansion, METs

Abstract

Objective: to compare the effectiveness of Tendon and Nerve Gliding Technique (TNGT) exercise with ultrasound diathermy.Methods: Twenty Carpal Tunnel Syndrome (CTS) hands meet the inclusion criteria and divided into2 groups: TNGT exercise and Ultrasound group. The subjects’ characteristics in the two groups are homogenous.Results: The statistical analysis results using Friedman test on pain/paresthesia complaint and hand functional status in each group is significant. Using the ANOVA, results of pain/paresthesia complaintbetween two groups shows a significant difference in the 4th measurement and results of functional status between two groups show a significant difference in the 9th measurement.Conclusions: Our study emphasized the TNGT exercise combined with splinting will be a good alternative treatment for CTS beside ultrasound diathermy combined with splinting, since TNGTexercise group has the same effectiveness with ultrasound diathermy group in decreasing VAS and has better effectiveness in improvement of FSS.Keywords: CTS, TNGT, Ultrasound Diathermy, VAS, FSS.

Abstract

Objectives: Knee osteoarthritis patient has the risk for ambulation disability as a result of knee pain. Nuhonni-Tulaar rheumatic exercise was designed to prevent complications of rheumatic diseasesincluding knee osteoarthritis. The purpose of this study is to investigate the effectivity of Nuhonni-Tulaar rheumatic exercise to reduce pain of knee osteoarthritis patient.Methods: Twenty two subjects did the rheumatic exercise thrice a week in the gymnasium of medicalrehabilitation department Cipto Mangunkusumo hospital. Twenty other subjects, the control group, didthe standard hamstring stretching exercise at home. Each group performed the exercise for 8 weeks.Pain was measured using Visual Analog Scale (VAS) each week and analyzed before and after theintervention.Results: After 8 weeks VAS was reduced 1.5 in the intervention group while in the control group the reduction was 0.9. VAS was then transformed into no pain, mild pain, moderate pain and severepain. VAS was significantly reduced in the intervention group (p=0.002) but not in the control group (p=0.059). VAS changes between the two groups was significantly different (p=0.004).Conclusions: Rheumatic exercise has greater benefit in reducing pain than standard stretching exercise.Warming up, core exercise II and cooling down is safe and comfortable for knee osteoarthritis patientwith grade I-II Kellgren Lawrence radiographic grading.Keywords: Nuhonni-Tulaar Rheumatic exercise, Knee osteoarthritis, Visual Analog Scale

Abstract

Objectives: to evaluate the efficacy of medial patellar taping in addition to exercise for pain relief and 15 meters walking time, and to find any correlation between the pain improvement and functional outcome.Methods: This study was a randomized controlled trial of knee patellar taping in combination with the isometric and stretching exercise in comparison to taping free- exercise. The inclusion criteria includesknee osteoarthritis patients with VAS score ranges four to eight of each knee, with or without tolerable pain on the other knee. The treatment group received knee tapping and isometric and stretching exercise of thequadriceps, iliotibial band, hamstring and gastrocnemius muscles. Both treatment and control group were assessed for about three weeks (day 1/week 0, day 2, day 28/week 4, and day 49/week 7). A visual analoguescale (VAS) was used to assess knee pain severity whereas 15 m walking time assessed to measure functional gait performance. On day 2, both measurements were done for study group only, with assumption the resultwould be the same with 1 day exercise only. A Lequesne index measured functional outcome.Results: There were 24 participants, twelve patients were in the tapped group while the remaining twelve were in the control group. Significant pain differences were visible during the treatment period at day 1, day2 and at the beginning of week 7. At day 2 and week 4, the study group showed significant pain reduction in comparison to the control group (p < 0.007).The walking time also showed significant improvement in the study group at day 2 (p < 0.000) but there were no significant difference at week 4 and week 7. At week 7, the study group experienced pain (p < 0.037) andoutcome improvement significantly. In addition, for about 75% of them had none to moderate disability post intervention. Meanwhile, from the total of eight patients in the control group who had severe to very severedisability, four of them had moderate (50%) and the rest had mild disability (50%). There was significant correlation between the pain and functional outcome on day two.Conclusions: Medial Patellar Taping in addition to isometric and stretching exercise of quadriceps, iliotibial band, hamstrings and gastrocnemius muscles showed significant improvement of disability superior to theuntapped group. Abstract

Objectives: Obtaining the standard normal value of 6 years old children aged six year, and testing our hypothesis if girls’ dexterity is higher than boys.Methods: The design of this study was a cross sectional study of six years old students of public elementary school in South Jakarta. Inclusion criteria included age six years old, no neurological andorthopedic problem, normal nutritional status and right handed. Location based-random sampling was performed. Dependent variables comprised of age, sex and nutritional status. Independent variable wasthe result of ‘16-Holes Three-Jaw Chuck Manual Dexterity Pegboard’ test. Statistical analysis was done by descriptive analysis and Mann Whitney Test.Results: There were 191 girls and 191 boys who fulfilled the criteria. The ‘16-Holes Three-Jaw Chuck Manual Dexterity Pegboard’ test allowed subjects to be assessed for their ability to put numbers of pegwithin 15 seconds. The mean number of pegs placed for girls was 6.92 pegs (CI 95% 6.76 – 7.08) and boys was 6.49 pegs (CI 95% 6.33 – 6,65)Conclusions: Girls are able to place more pegs than boys in a given time. Furthermore, girls’ manual dexterity is superior to boys (p 0,000).Keywords: Manual dexterity, standard normal value, pegboard, sex differences.

Abstract

Objectives: to know the standard value of lower extremity muscle strength of eight year old children and furthermore to explore the correlation of the muscle strength and body height and weight.Methods: The study design is cross sectional. The target is eight year old children in public elementary school in Jakarta Pusat. The subjects’ characteristics are normal nutritional state, and no neurological normusculoskeletal disorders. Sampling was done by cluster randomization to determine the location and simple randomization on site to determine subjects. There were 171 boys and 180 girls in this tudy. Independentvariables are age, sex, body weight, body height and nutritional state that was determine with Z-score of body mass index. Dependent variables are lower extremity muscle strength that classify as torque. This study usedhand-held dynamometer for muscle strength measurement. Statistical analysis was done with descriptive statistic and Pearson and Spearman correlation test.Results: Standard values of eight year old boy’s lower extremity muscle strength are: right hip flexor 21.86 Nm (SD 3.40), left hip flexor 19.64 Nm (SD 3.19), right hip extensor 17.05 Nm (SD 3.66), left hip extensor16.08 Nm (SD 3.56), right knee extensor 18.19 Nm (SD 3.60), left knee extensor 16.09 Nm (SD 3.55), right knee flexor 15.18 Nm (SD 4.23), left knee flexor 14.48 Nm (SD 3.97), right ankle dorsiflexor 6.58 Nm (SD1.53), left ankle dorsiflexor 6.05 Nm (SD 1.42), right ankle plantarflexor 10.08 Nm (SD 1.69), left ankle plantar flexor 9.13 Nm (SD 1.90).Standard values of eight year old girl’s lower extremity muscle strength are: right hip flexor 21.60 Nm (SD 3.62), left hip flexor 19.62 Nm (SD 3.37), right hip extensor 16.66 Nm (SD 4.06), left hip extensor 15.81 Nm(SD 3.94), right knee extensor 17.43 Nm (SD 3.79), left knee extensor 15.20 Nm (SD 3.38), right knee flexor 14.61 Nm (SD 4.28), left knee flexor 13.51 Nm (SD 4.00), right ankle dorsiflexor 6.34 Nm (SD 1.45), leftankle dorsiflexor 5.97 Nm (SD 1.52), right ankle plantarflexor 9.55 Nm (SD 1.98), left ankle plantar flexor 8.69 Nm (SD 1.83). The boy’s lower extremity muscle strength are stronger than the girl’s in left knee extensor,left knee flexor, right ankle plantarflexor and left ankle plantarflexor. The boy’s muscle strength are moderately correlated to body height except for right hip extensor, left hip extensor and right ankle dorsiflexorthat weakly correlated. The boy’s muscle strength are moderately correlated to body weight except for left hip extensor that weakly correlated. The girl’s muscle strength are moderately correlated to body height. Thegirl’s muscle strength are moderately correlated to body weight except for left hip flexor and left hip extensor that weakly correlated.Conclusions: The muscle strength pattern of boys and girls is similar; the strongest are right hip flexor and the weakest are left ankle dorsiflexor.Keywords: Muscle strength, standard values of eight year old children, torque, hand-held dynamometer

Abstract

Objectives: to determine whether the incentive spirometry respiratory muscle training can increase the maximum inspiratory capacity, decrease difficulty of breathing, functional lung capacity and quality oflife according to St. George’s Respiratory Questionnaire on Chronic Obstructive Pulmonary Disease patients.Methods: an intervention pre-post and case control group of twenty subjects with mild COPD who came to Medical Rehabilitation Department. They divided into two groups: control group (10 subjects)and study group (10 subjects). All subjects underwent pre interventional test which are : maximum inspiratory capacity, dyspnoea rating scale, pulmonary functional test (PFT) and questionnaire forquality of life. The study group were given respiratory muscle training with incentive spirometry and breathing control exercise while the control group only given the breathing control exercise. After 8weeks, all participant underwent post interventional test. Every subject still using the basic medication.Results: There are statistically improvement of maximum inspiratory capacity, dyspnoea rating scale and quality of life in study group compare with the control group (p<0,05). Mean difference of SGRQbetween pre and post intervention shows significant results in activity, impact and total component (p<0,05) and there is no significant results on symptoms components (p>0,05). No changes were foundin FEV1% value with the study or control group (p>0,05).Conclusions: Combination between incentive spirometry respiratory muscle training and breathing control exercise can improve maximum inspiratory capacity, dyspnoea rating scale and quality of life inCOPD patients within 8 weeks.Keywords: COPD, Incentive spirometry muscle training, Maximum inspiratory capacity, Quality of life.

Abstract

Objectives: To investigate the validity and reliability of Modified Picture Vocabulary Test IV (PPVTIV) in 48-59 months old child. Methods 228 items in 19 sets of PPVT IV were translated in Indonesian language and revised according to Indonesian culture. One hundred and five 48-59 months old children were tested. The results wereanalyzed for its construct validity and reliability. The construct validity was valued by the correlation coefficient between the set score and the total score and the correlation coefficient between the itemscore and the set score. The reliability was valued by intra-class correlation coefficient, standard error of measurement (SEM) and Cronbach alpha.Results: The construct validity of Modified PPVT IV was good (r>0.256; α=0.01 n=100) and the reliability was high (internal consistency with Cronbach alpha= 0.938, inter-rater reliability= 0.957-0,985 and standard error of measurement (SEM) = 6.4).Conclusions: Modified PPVT IV was a valid instrument with satisfactory reliability. Abstract

Objective: To investigate the effectiveness of dual-task-based ball exercise program to improve walking ability for executing a dual task.Methods: Experimental research, pretest – posttest design. Sixteen subjects, 1 woman and fifteen men, meanage 61 years old that come in Rehabilitation outpatient clinic BLU. RSUP. Prof. dr.R. D. Kandou Manado. Subjects were intervened with dual task-exercise using one or two balls while walking for about 10 m in length within 30 minutes. The frequency of exercise was three times a weekfor about four weeks. Subjects were assessed throught their walking velocity changes in single task and dual task exercises. The evaluation scores were noted on the questionnaire of walking ability.Results: The walking ability value in single task condition has significant change with p<0.001, and the walking ability in dual task condition has significant change also with p<0.001. These indicatethat subjects’ walking ability after intervention is faster than walking ability before the intervention. Walking ability based on walking ability questionnaire has significant change with p<0.001, showswalking ability after intervention has significant change.Conclusions: Dual task-exercise can improve walking ability of stroke chronic patient in singla task condition and dual task condition.Keywords: Chronic stroke, Dual task-exercise.

Abstract

Objective: to evaluate walking performance in female with type 2 diabetes mellitus (DM2) with the 400-meters walk test (400-MWT) compared to healthy individuals.Methods: two groups of female subjects with DM2 and healthy individuals were matched by the age. The 400-MWT parameters to be compared were walking speed (WS) and predicted maximum oxygenconsumption (pVO2max). Baseline examination included body mass index (BMI), random blood glucose (RBG), and ankle-brachial index (ABI). All subjects performed 2 minutes warm up before thetest. Heart rate (HR) was recorded every 30 seconds, and blood pressure (BP) was measured before warm up and within 60 seconds after test. The test was performed twice on a different day.Results: Nineteen subjects on each group participated in the study. The mean WS was significantly different (p<0.0001) between the study group (1.26 + 0.19 and 1.31 + 0.17 m/s) and the control group(1.70 + 0.20 and 1.78 + 0.24 m/s) for the first and second tests respectively. There was a significant difference of mean pVO2max (p<0.0001) between the study group (17.22 + 2.94 and 17.99 + 2.36 ml/kg/min) and the control group (23.68 + 3.79 and 24.44 + 3.74 ml/kg/min).Conclusion: the 400-MWT demonstrated lower walking performance in female subjects with DM2 compared to healthy individuals.Keywords: Type 2 DM, healthy individuals, 400 meter walk test, walking speed, VO2max.
