PDAM Surya Sembada is a local company that is responsible for providing clean water in the city of Surabaya. As a local company that is responsible for serving the community, then PDAM Surya Sembada seeks to monitor the development of enterprise organizations that are useful to improve the quality of the company.To see the development of an organization is to assess the performance of Human Resources (HR), its called the assessment of performance based employee performance. Performance appraisal results can indicate whether HR (employees) on stretcher organization has met the objectives / targets as requested by the organization, both in quantity and quality, how the behavior of employees in carrying out their work, how the use of working time and so on.The benefits obtained by the application of employee performance evaluation based on job performance, among other things: work units can take into account the results of the work program achievement compared with the results achieved, the expected performance of employees will be more measurable, their feedback to improve performance, and the monitoring and evaluation of leadership in order to improve employee performance.

One of the main causes for low level filing of promotion on the functional position librarian at the Library and Archives of East Java Province is evidence record activites after activities have been carried out, haven’t been stored properly. So happen discrepancy between the evidence activites with activities that have been carried out, when needed in the calculation of credit score. Than problem and analysis of the librarian data that is available, they need applications that can store evidence of activities in accordance with the activities they have done. In additional they also need applications that can be used to plan their activities beginning of each year as they have to achieve targets. While on the side of the official appraiser, needed an application that could be used to calculate the activity evidence of librarian accordance with ministerial decree number 9 in 2014 and accompanied the supervision of the official appraiser. The trial results of the application shows that the system is able to work according to function, such as: can be used to assist librarians in terms of storage and documentation of evidence of activities that have been carried out in accordance with the SKP that had been developed at the beginning of the period, can propose activities and displays the target the number of credits that must be achieved librarians, and can present the required reports related parties

An Administration Document is necessary for sales vehicle and must be completed by customers. This Administrative document collection is done at the one of Astra isuzu branch because this process involving cooperation between customers, sales units, Sales leasing, An Administration document for vehicle sales also serves as one of the absolute requirement on leasing demand credit application chosen by the applicant vehicle loans and determine the vehicle's credit application is accepted or not. There is a problem that has occurred during this time, where there are no regulation about deadline for collection of Administrative document that make delays in the all process of ordering vehicles. With the implementation of the Administrative Workflow Systems in this Monitoring system can help the customer to accelerate the collection of administration documents of sales vechicle and help the sales unit to immediately process the customer booking request and help sales leasing to immediately process subcribers vehicle if the loan application is accepted or not.

Abstract: PT. Jaya Darmabakti Arthagraha (JDA) is a company engaged in construction services Mechanical Electrical (ME). In general the projects it undertakes a project worth more than ten billion and the project implementation period of more than one year. Current conditions, PT. Jaya Darmabakti Arthagraha is the process of recording the recognition of revenue based on the progress of projects in the company still has not done. During this time, the recording of revenues are recorded based on terms of income and expense are recorded based on exit / entry of project costs, so profit / loss is based on the actual terms of income and expenses to the project. To overcome these problems, we need an information system that generates a revenue recognition by percentage of completion method of work. From the results of information systems, will be a calculation of revenue recognition based on project progress using the percentage of completion of the work. From the test results and evaluation, has generated an information system revenue recognition percentage of completion method of work that is integrated with project planning in the company, so when planning the project has included planning of revenue recognition.

PT. IGLAS (Persero) is a manufacturing company of glass packaging (bottles). The company currently often late in a customer demand fulfillment. This problem due to several factors such as conventional way of schedulling and inneficient production process. Based on these problems, it would require a production scheduling information system using the priority rules to solve. This information system can provide production schedule more effectively according to choosen criteria from five methods that have been used. The criterias used are average processing time, utilization, average number of jobs in the system, and average number of delayed jobs. Meanwhile five methods used are, First Come First Serve (FCFS), Earliest Due Date (EDD), Shortest Processing Time (SPT), Longest Processing Time (LPT), dan Critical Ratio (CR). Based on the information system created and series of trials performed, company has a more effective queued schedule in accordance with desired company criterias, so that all of customer demand can be fulfilled and timely completion.

Medical records are factual documents which related to patient condition, past and present treatment which is written by a doctor who treat the patient. (Huffman, 1999).RSAM have a diffcult situation to search, provide, and study patient medical records at Outpatient Unit because all of them are still in document format in archive room which could make it take a long time to deliver it to the admin room.To help solve this difficulty, therefore the hospital needs a medical records application at outake patient unit. This application is made to help doctors to decide what action to the patient so it will produce accurate and easy to search medical records. With the help of this application in Outpatient Unit, so Outpatient Unit at RSAM will become better and faster to give a service to the patient according to hospital service standard.With design and development of medical records application at outpatient unit in RSAM will produce a report and also medical record information about patient visitation, medical record registration, and income. This system is used by doctor to consider what action should be taken and trail access of patient medical records.

ABSTRAKDinas Perdagangan dan Perindustrian (Disperdagin) Kota Surabaya mempunyai tugas menyelenggarakan urusan pemerintahan dan pelayanan umum seperti pembuatan ijin perusahaan. Proses pembuatan ijin saat ini terlalu lama karena kurangnya informasi bagi perusahaan saat akan mendaftar. Pihak perusahaan harus membawa kembali dokumen formulir untuk mempersiapkan dokumen yang dibutuhkan dan kembali lagi untuk menyerahkan dokumen ke Disperdagin. Masalah lain yang timbul yaitu belum adanya sistem yang memberikan informasi untuk Disperdagin mengenai status perijinan yang dibuat.Berdasarkan permasalahan di atas, dibutuhkan sebuah sistem yang mampu mempermudah proses pembuatan ijin secara online bagi perusahaan. Perusahaan tidak perlu berulangkali datang ke kantor Disperdagin untuk melengkapi dokumen. Selain itu juga dibutuhkan sistem yang membantu pihak Disperdagin untuk memonitoring status ijin perusahaan. Sistem yang dibuat dikembangkan menggunakan metode Waterfall.Berdasarkan hasil uji coba yang telah dilakukan, sistem ini dapat memenuhi kebutuhan pada Dinas Perdagangan dan Perindustrian Kota Surabaya dalam proses pendaftaran ijin, persetujuan ijin, penerbitan ijin, pemantauan posisi dokumen, pembuatan laporan ijin usaha, dan ijin industri serta memberikan peringatan apabila ada ijin yang habis melalui sms gateway.Kata Kunci: Monitoring Perijinan, Perusahaan, Industri.

PT. Geo Given Visi Mandiri (PT. GGVM) is an innovative company that is engaged in the field of human resources services. PT. GGVM offer the provision of manpower to a sales force of the company. PT. GGVM has a vision to be the best as providers of employment services company to provide services and qualified human resources to the client.Training is one of the means to improve the competence of the workforce. At the PT. Geo Given Visi Mandiri, the training has not run optimally because the existing training planning is done without any training needs analysis in advance. This can result in a mismatch between the needs of competence in performing a job with competence possessed by today's workforce.Based on the results of the test and evaluation of training needs analysis application, indicating that the application can help PT. Geo Given Visi Mandiri in terms of manpower assessment, planning training needs, and provide reports in accordance with the company's needs.

Abstract : PT. Surapita Unitrans constitute a service company that handles process Kawasaki motorcycle. In handling process motorcycle vehicle maintenance there are some problems that occur in the company, namely the customer can't information scedule queue service, no booking service for customers who have a solid rushing, there is no information available for customer, and in preparing reports to the section of After Sale still using MS Office.From the above problems, then be made a system design aplication administration service motorcycle in PT. Surapita Unitrans which can be set queue service customer automatically by using method Shortest Job First, there is booking service system available for customer, and system can provide information service form history service available for customer, and there is a reporting system for the division after sale.Based on trial results application system administration services made This application can help companies in addressing the queue automatically, generating system booking services for customer, produce history service so that customers can obtain the required information service, and there is reporting period for division After Sale.

Cooperative Employees PT. Mega Utama Indah is a business entity in the form of savings and credit cooperatives whose members are employees of PT. Mega Utama Indah. The main constraints faced by the Cooperative Employees PT. Main Mega Indah now is the process of savings is still done manually as the process of loan application, loan approval and repayment of the loan installments. All these processes are still done by recording into a book.These constraints resulted members have to wait to find out whether its proposed loan and the company experienced difficulty in obtaining information and reports - reports required by the cooperative, the required savings and loan applications which includes member registration, loan applications, loan approvals and repayment of the loan installments.Based on trials that have been done, the application can generate reports related to the savings and loan in accordance with the needs of cooperatives, for example, statements about the expected revenues loans, loan report approved or rejected, reports about employees who pay the installments of this month and others reports required cooperative.
