Abstract

Introduction: Meningiomas are common central nervous system tumors originating from meningothelial cells. Imaging examinations are widely used to characterize lesion features, while histopathological evaluation remains essential for definitive diagnosis and grading. Material and Methods: This study was a retrospective descriptive study conducted using secondary data from medical records of patients diagnosed with meningioma at RSUD Dr. Saiful Anwar Malang between 2020 and 2023. Data included demographic characteristics, imaging findings from CT and MRI examinations, and histopathological grading. Results: Most meningioma cases occurred in female patients (85%), with the highest proportion observed in the 40–60-year age group (65%). According to the WHO classification, grade I meningiomas were the most common (69%). The frontal region was the most common tumor location (25%), followed by involvement of multiple regions (35%). Solitary meningiomas accounted for 87% of cases. Most tumors had a maximum diameter of 4–6 cm (67%). Imaging findings commonly included dural tail (51%), hyperostosis (53%), and peritumoral vasogenic edema, which was qualitatively identified in all cases. Conclusion: This retrospective descriptive study demonstrates the demographic distribution, imaging characteristics, and histopathological grading of meningioma cases at a tertiary referral center. Imaging features such as dural tail, hyperostosis, and peritumoral edema were frequently observed; however, histopathological examination remains the gold standard for definitive diagnosis and grading of meningioma.

Abstract

Introduction: Post-classroom questionnaires (PCQ) in graduate medical education may serve as more than satisfaction surveys; when grounded in educational theory and analyzed semiquantitatively, they can be interpreted as tools for reflective practice, Continuous Quality Improvement (CQI), and Outcome-Based Education (OBE). Material and Methods: This study employed a descriptive semiquantitative program evaluation design within a Master Medical Education Program, analyzing routinely collected, anonymized learner feedback across multiple classroom sessions. Items addressed domains including program identity, onboarding, pedagogy, assessment, institutional support, and perceived value. Results: A 51% response rate yielded patterns of consistently high endorsement for program mission, orientation, student-centered learning, and assessment-as-learning, while flipped classroom implementation, holistic competency development, and research/publication support received comparatively lower ratings. Institutional support was positively perceived, though tuition value was rated less favorably. These contrasts may suggest interpretive hypotheses of systemic misalignment, such as a potential resource–expectation gap in scholarly mentorship, or a pedagogical implementation trap in innovative teaching strategies. By emphasizing pattern recognition and relative contrasts rather than inferential claims, this evaluation generates hypotheses that warrant further exploration and can inform curricular refinement, faculty development, and policy adjustment. Conclusion: Post-classroom questionnaires, when systematically embedded within CQI–OBE frameworks, may thus function as low-burden, high-impact instruments that foster reflective professionalism, evidence-informed decision-making, and sustainable excellence in postgraduate medical education.

Abstract

Introduction: Cervical cancer remains a significant health burden, particularly in developing countries where access to radiotherapy is limited. Neoadjuvant chemotherapy (NACT) followed by radical hysterectomy has emerged as an alternative treatment strategy for locally advanced cervical cancer. This study aims to evaluate the survival outcomes and histopathological characteristics of patients with stage IIB cervical cancer who underwent NACT followed by radical hysterectomy in Dr. Saiful Anwar General Hospital. Material and Methods: This retrospective study analyzed medical records of patients with stage IIB cervical cancer who received NACT followed by radical hysterectomy at Dr. Saiful Anwar General Hospital from January 2021 to July 2022. Patients with incomplete medical records or loss to follow-up were excluded. Data analysis included descriptive survival outcomes and histopathological findings. Associations between histopathological variables and mortality were analyzed using Fisher’s exact. A p-value ≤0.05 was considered significant. Results: A total of 51 patients met the inclusion criteria. During the follow-up period, 39 patients (76.5%) were alive, while 12 patients (23.5%) had died. The estimated overall survival probability at 24 months was 76.5% (95% CI: 65.7–89.0). Fisher’s exact test demonstrated no statistically significant association between histopathological subtype and mortality (p = 0.069). No statistically significant association was observed between histopathological subtype and mortality. Conclusion: Survival outcomes of stage IIB cervical cancer treated with neoadjuvant chemotherapy followed by radical hysterectomy in a tertiary setting. Histopathological subtype and absence of residual tumor were associated with improved survival. Further comparative studies are required to evaluate the effectiveness relative to standard treatments.

Abstract

Introduction: Diaphragmatic and hiatal hernias are clinically significant and may lead to serious complications if not promptly recognized and treated. Differentiating these entities on imaging can be particularly challenging when extensive visceral herniation distorts normal anatomical landmarks. Case Presentation: We report two rare cases illustrating distinct diagnostic challenges. The first involved a 49‑year‑old woman with paraesophageal hernia complicated by gastric volvulus. The second concerned a 1‑year‑old girl initially misdiagnosed with Bochdalek hernia, later identified intraoperatively as type III hiatal hernia, an unusual presentation in infancy. Results: Radiologic imaging was crucial for diagnosis and management. Contrast‑enhanced CT offered precise anatomical detail, enabling accurate identification of hernia type, extent, and complications, thereby guiding surgical planning. Notably, in the pediatric case, CT contributed to distinguishing a rare type III hiatal hernia from an initial Bochdalek diagnosis Conclusion: These cases emphasize the key role of CT in distinguishing hiatal from congenital diaphragmatic hernias. Understanding hernia anatomy and imaging features is vital for timely diagnosis, surgical planning, and preventing serious complications.

Abstract

Introduction: Condyloma acuminata is an anogenital wart caused by human papillomavirus (HPV), typically affecting sexually active individuals. However, atypical presentations in elderly patients without a history of sexual exposure are rare and may mimic malignant lesions, posing a diagnostic challenge. Case Presentation: A 72-year-old male patient presented to the dermatology and venerology outpatient clinic of Fatmawati Hospital with complaints of reddish spots on the penis accompanied by itching that had worsened over the past 2 weeks. The symptoms had been recurring intermittently for the past four years. The patient denied any risky sexual activity. On the dermatological examination at the initial visit, multiple, erythematous, verrucous, and nummular plaques were observed on the glans penis and sulcus. Acetowhite examination yielded positive results, and the histopathological examination suggested condyloma acuminata. The patient was treated with 25% podophyllin, 90% TCA, and glycyrrhizinic acid. Conclusion: This case underscores the importance of histopathological confirmation in elderly patients with atypical genital lesions mimicking malignancy. Careful diagnostic evaluation is essential to avoid overtreatment and misdiagnosis.

Abstract

Introduction: Brain and central nervous system tumors, particularly gliomas, represent a growing global burden with poor prognosis. Radiotherapy post–maximal resection is central to management, yet its success depends on precise target delineation to preserve healthy tissue. MRI, especially T2-weighted and FLAIR sequences, is standard for planning, but the optimal sequence remains unclear.    Material and Methods: A systematic review was conducted according to PRISMA guidelines. Literature searches were performed in PubMed, ScienceDirect, and EuropePMC databases up to 25 May 2025. Observational or comparative studies assessing T2-weighted and/or FLAIR MRI for radiotherapy target contouring were included. Study selection, data extraction, and quality assessment using ROBINS-I were independently performed by two reviewers.    Results: From 246 identified studies, four studies met the inclusion criteria. Overall, FLAIR-based delineation consistently produced larger clinical and planning target volumes compared with T2-weighted imaging, capturing additional regions of peritumoral abnormality. Recurrence pattern analyses suggested that FLAIR-defined volumes may better encompass infiltrative tumor margins. High interobserver agreement was reported for MRI-based target delineation, supporting the feasibility of MRI-only workflows, although selective CT integration remained important for certain organs at risk.    Conclusion: Although the evidence is limited, both T2-weighted and FLAIR MRI sequences play an important role in brain tumor radiotherapy planning. While T2-weighted imaging generates more conservative target volumes, FLAIR sequences better capture infiltrative edema, potentially lowering recurrence risks at the cost of increased healthy tissue irradiation. Careful, consensus-driven sequences selection is required to balance tumor coverage and normal tissue sparing.

Abstract

Introduction: Cleft lip with or without cleft palate requires timely surgical management to optimize feeding, speech development, and facial growth. In archipelagic regions, geographic isolation and limited healthcare infrastructure may delay access to specialized cleft care. Hospital ship–based outreach programs have emerged as an alternative strategy to deliver surgical services to underserved island communities. This report presents a descriptive case series of cleft lip repairs performed during a hospital ship mission in remote islands of South Sulawesi, Indonesia. Case Presentation: Two patients with untreated cleft lip deformities were identified during surgical screening conducted as part of a hospital ship outreach mission. Both presented in adolescence or adulthood after prolonged delays in accessing definitive treatment. Cleft lip repair using a modified Millard rotation–advancement technique was performed under general anesthesia. Early postoperative outcomes were assessed based on surgical complications and scar evaluation. Postoperative follow-up was limited to 7 days. Conclusion: Hospital ship–based cleft lip repair may represent a feasible approach to delivering surgical care in remote island settings. However, evidence remains limited to early postoperative outcomes, and further studies with larger cohorts and standardized long-term follow-up are needed to evaluate functional and aesthetic results.

Abstract

Introduction: Endometriosis is a chronic condition affecting about one in ten women of reproductive age, resulting in reduced quality of life. Non-invasive diagnostic methods based on single serum biomarkers or isolated clinical parameters have shown limited reliability. Consequently, integrating serum biomarkers with clinical variables has been proposed as a more effective diagnostic approach. This study aimed to assess the diagnostic accuracy of combined serum biomarkers and clinical variables for endometriosis diagnosis.    Methods: A systematic review and meta-analysis were carried out in line with PRISMA recommendations. Literature synthesize were conducted across PubMed, Cochrane, ScienceDirect, Epistemonikos, Scopus, ProQuest, and EBSCO to February 2025. Eligible studies included diagnostic investigations that compared combined serum biomarker and clinical-variable models with serum biomarker or clinical-variable only reference standards. Study quality was evaluated using the QUADAS-2 tool. Pooled effect analysis was generated as odds ratios (ORs) using 95% confidence intervals (CI).    Results: Seventeen studies met eligibility criteria. Combined diagnostic models demonstrated significantly higher sensitivity (OR 2.64; 95% CI 1.54–4.53) but non-significant specificity (OR 1.03; 95% CI 0.43–2.49) compared with single diagnostic approaches. However, the area under the curve (AUC) analysis significantly improved in combined models (OR 1.46; 95% CI 1.10–1.93). Moderate to high heterogeneity with publication bias was observed across studies.    Conclusion: Combined serum biomarkers and clinical variables provide better diagnostic performance than single tests and support the use of non-invasive multimodal strategies prior to surgical confirmation. Further prospective validation is required to ensure consistency and clinical applicability.

Abstract

Introduction: An imperforate hymen is a congenital condition in which the hymen completely obstructs the vaginal canal. Many patients remain asymptomatic until menarche, when they may present with primary amenorrhea, a pelvic mass, and cyclic abdominal pain. The traditional hymenectomy technique involves a cruciate (cross-shaped) incision with excision of the quadrants and approximation of the hymenal edges to the vestibule using absorbable sutures. However, this method can be associated with scar formation and subsequent reclosure of the hymen. Case Presentation: A 12-year-old girl presented with cyclic abdominal pain for three months, which worsened one week prior to admission. She complained of severe abdominal pain starting six hours before hospital admission. Physical examination revealed a palpable lower abdominal mass, tenderness, and no visible hymenal opening. Ultrasonography showed hematometra and hematocolpos, leading to the diagnosis of imperforate hymen. The patient underwent an annular hymenectomy using electrocautery to create a circumferential (ring-shaped) excision, aiming to prevent postoperative hymenal closure and scarred hymenal contour. Conclusion: Imperforate hymen should be considered in premenarcheal adolescent girls presenting with cyclic abdominal pain. Annular hymenectomy using electrocautery offers a modern surgical approach that may reduce postoperative hymenal reclosure and scarring compared with the traditional cruciate technique.
