Abstract

Background: Premature rupture of membranes (KPD) is the rupture of the amniotic membrane before there are signs of labor and after one hour is not followed by labor. Sectio caesarea (SC) or cesarean section is a surgical procedure to remove the fetus through an incision or action on the uterine and abdominal walls, provided that the fetus is healthy and weighs more than 500 grams. The purpose of this study was to determine the relationship between premature rupture of membranes and cesarean section delivery. Methods: This type of research is quantitative using observational analytic research design, with a cross sectional approach. The population in this study was the medical record data of patients with maternity pathology or with inpatient complication with simple random sampling. The results of the study were analyzed by the chi-square test. Results: There were 49 mothers diagnosed with premature rupture of membranes with sectio caesarean delivery with an expected value of (42.6%) with mothers who experienced premature rupture of membranes who performed sectio caesarean delivery and 23 mothers with an expected value of (20%) who experienced premature rupture of membranes and did not perform sectio caesarean delivery. The results of the study obtained a P-Value of 0.002 <0.05. Conclusion: There is a significant relationship between premature rupture of membranes and cesarean section delivery at Budi Kemuliaan Hospital, Batam City in 2022.

Abstract

ABSTRACT Background: Hypertension is a non-communicable disease with a high prevalence among women of reproductive age, particularly those using hormonal contraceptives. The use of hormonal contraceptives is suspected to be associated with increased blood pressure through activation of the Renin-Angiotensin-Aldosterone System (RAAS). This study aims to determine the relationship between hormonal contraceptive use and the incidence of hypertension among women of reproductive age at RS Hj. Bunda Halimah. Methods: This study employed an observational analytic design with a retrospective cross-sectional approach. The sample consisted of 323 women of reproductive age at Hj Bunda Halimah Hospital in 2024, selected using the Slovin formula. Data were collected through questionnaires and blood pressure measurements, and subsequently analyzed using the Chi-square test (p < 0.05). Results: The analysis results showed that most hormonal contraceptive users experienced hypertension (88.2%), whereas most non-hormonal contraceptive users did not experience hypertension (85.7%). Statistical analysis indicated a significant association between hormonal contraceptive use and the incidence of hypertension among women of reproductive age (p = 0.001). Conclusion: There was a statistically significant association between hormonal contraceptive use and the incidence of hypertension among women of reproductive age at Hj Bunda Halimah Hospital in 2024. Abstract

Introduction: An imperforate hymen is a congenital condition in which the hymen completely obstructs the vaginal canal. Many patients remain asymptomatic until menarche, when they may present with primary amenorrhea, a pelvic mass, and cyclic abdominal pain. The traditional hymenectomy technique involves a cruciate (cross-shaped) incision with excision of the quadrants and approximation of the hymenal edges to the vestibule using absorbable sutures. However, this method can be associated with scar formation and subsequent reclosure of the hymen. Case Presentation: A 12-year-old girl presented with cyclic abdominal pain for three months, which worsened one week prior to admission. She complained of severe abdominal pain starting six hours before hospital admission. Physical examination revealed a palpable lower abdominal mass, tenderness, and no visible hymenal opening. Ultrasonography showed hematometra and hematocolpos, leading to the diagnosis of imperforate hymen. The patient underwent an annular hymenectomy using electrocautery to create a circumferential (ring-shaped) excision, aiming to prevent postoperative hymenal closure and scarred hymenal contour. Conclusion: Imperforate hymen should be considered in premenarcheal adolescent girls presenting with cyclic abdominal pain. Annular hymenectomy using electrocautery offers a modern surgical approach that may reduce postoperative hymenal reclosure and scarring compared with the traditional cruciate technique.

Abstract

Introduction: Ovarian torsion during pregnancy is a rare but serious condition that requires prompt diagnosis and intervention to prevent maternal and fetal complications. Large benign ovarian tumors, particularly mature cystic teratomas, are the most common underlying cause. This case report highlights the management of a giant torsed ovarian dermoid cyst detected in a pregnant woman during the second trimester.Case Illustration: A 16-year-old woman, G1P0A0, presented at 17–18 weeks of gestation with severe abdominal pain for one day. Physical examination revealed a mobile cystic adnexal mass. Ultrasonography showed a single live intrauterine fetus corresponding to gestational age and a unilocular left adnexal cyst measuring 14.5 × 12.7 × 11.5 cm with a positive whirlpool sign, indicating ovarian torsion. An emergency laparotomy with left salpingo-oophorectomy was performed. Histopathological examination confirmed a mature cystic teratoma with extensive necrosis and hemorrhage consistent with torsion, without evidence of malignancy. Ascitic and cyst fluid cytology showed benign lesions. The pregnancy was preserved, and postoperative fetal condition remained stable.Conclusion: This case demonstrates that prompt surgical intervention for ovarian torsion during the second trimester of pregnancy can be performed safely, allowing preservation of pregnancy and favorable maternal and fetal outcomes.Manajemen Torsio Ovarium Akibat Kista Dermoid Raksasa pada Trimester Kedua Kehamilan: Sebuah Laporan KasusAbstrakPendahuluan: Torsio ovarium selama kehamilan merupakan kondisi yang jarang, namun berpotensi serius dan memerlukan diagnosis serta penatalaksanaan yang cepat untuk mencegah komplikasi pada ibu dan janin. Tumor ovarium jinak berukuran besar, terutama mature cystic teratoma, merupakan penyebab tersering torsio ovarium. Laporan kasus ini menyoroti penatalaksanaan torsio ovarium akibat kista dermoid besar pada trimester kedua kehamilan.Ilustrasi Kasus: Seorang wanita berusia 16 tahun, G1P0A0, datang pada usia kehamilan 17 – 18 minggu dengan keluhan nyeri perut hebat sejak satu hari. Pemeriksaan fisik menunjukkan massa adneksa kistik yang mobile. Ultrasonografi menunjukkan janin tunggal hidup intrauterin sesuai usia kehamilan dan massa kistik unilokular pada adneksa kiri berukuran 14,5 × 12,7 × 11,5 cm dengan whirlpool sign positif yang mengarah ke torsio ovarium. Pasien menjalani laparotomi dan salpingo-ooforektomi kiri secara emergensi. Pemeriksaan histopatologi menunjukkan mature cystic teratoma dengan nekrosis dan perdarahan luas sesuai dengan torsio, tanpa tanda keganasan. Sitologi cairan asites dan kista menunjukkan lesi jinak. Kehamilan dapat dipertahankan dan kondisi janin pascaoperasi stabil.Kesimpulan: Kasus ini menunjukkan bahwa intervensi bedah yang cepat pada torsio ovarium trimester kedua dapat dilakukan dengan aman serta memberikan luaran maternal dan janin yang baik.

Abstract

Introduction: Gastroesophageal reflux disease (GERD) is an upper gastrointestinal disorder caused by repeated reflux of gastric contents into the esophagus, leading to symptoms or complications. One of the risk factors for GERD is stress, which is commonly experienced by university students. This study aimed to determine the relationship between academic stress levels and the incidence of GERD among medical professional students of the Medical Study Program, Faculty of Medicine, Universitas Batam. Methods: This study employed a quantitative observational analytic design with a cross-sectional approach and utilized the Chi-square test. Total random sampling was applied based on inclusion and exclusion criteria. GERD was assessed using the GERD-Q questionnaire, while academic stress levels were measured using the Medical Student Stressor Questionnaire (MSSQ). Results: The results showed that a small proportion of students, 14.5% (9 students), experienced GERD symptoms. Nearly half of the respondents, 48.4% (30 students), experienced moderate stress, while 30.6% (13 students) experienced mild stress. A smaller proportion, 19.4% (12 students), experienced severe stress, and only 1.6% (1 student) experienced very severe stress. The Chi-square analysis revealed a p-value of 0.680 (p > 0.05) for the relationship between academic stress level and GERD incidence. Conclusion: There was no statistically significant relationship between academic stress level and the incidence of GERD among medical professional students. Abstract

Primary dysmenorrhea frequently disrupts the academic and daily activities of adolescent girls. This study evaluated the effectiveness of a 5-minute warm compress application in reducing menstrual pain intensity. A quasi-experimental study with a one-group pretest-posttest design was conducted on 42 female students of SMAN 28 Batam selected through purposive sampling. Participants applied a warm compress (40–45 °C) to the lower abdomen for 5 minutes. Pain intensity was measured using the Numeric Rating Scale (NRS) before and after the intervention. Data were analyzed using the Wilcoxon Signed-Rank test. Before the intervention, 42.9% of participants reported severe pain (NRS 7–10). After the 5-minute application, the severe pain category disappeared entirely (0%), with 57.1% of subjects shifting to the mild pain category (NRS 1–3). Statistical analysis showed a significant reduction in dysmenorrhea intensity (p < 0.001). A 5-minute warm compress application is a rapid and effective non-pharmacological intervention for relieving primary dysmenorrhea in adolescents. Its practicality makes this method a feasible self-care strategy to implement in school settings.
