Abstract

ABSTRACTTotal removal is difficult to be performed in skull base tumors because its location is surrounded by important structures such as nerves and blood vessels. Therefore, radiotherapy is one of treatment modalities that has been proven efficacy. Simultaneous with the development of imaging technology and advancement of radiobiology, radiosurgery is an emerging therapeutic modality. Radiosurgery is radiotherapy method which delivers high doseirradiation in single fraction. Rational use of stereotactic radiosurgery on benign skull base tumor is from radiobiology point of view; there is no advantage can be achieved from conventional dose fractionated radiotherapy compared with high dose. However, if we want to delivered high dose radiation, we must apply rigid immobilization, target definition using stereotactic navigation and image guidance verification. Radiosurgery can only be delivered in small intracranial lesion.ABSTRAKReseksi total kadang sulit dilakukan pada tumor yang terletak pada dasar tengkorak. Hal ini disebabkan lokasinya dikelilingi oleh struktur saraf dan pembuluh darah penting. Oleh karena itu, radioterapi merupakan salah satu modalitas terapi yang sudah terbukti maanfaatnya. Sejalan dengan perkembangan teknologi pencitraan dan kemajuan pengetahuan radiobiologi, radiosurgery merupakan modalitas terapi yang melejit penggunannya. Radiosurgery adalah metode pemberian radioterapi dengan dosis tinggi dan diberikan dalam fraksi tunggal. Rasional penggunaan stereotactic radiosurgery pada tumor jinak dasar tengkorak adalah karena dari sudut pandang radiobiologi, tidak ada kelebihan dariradioterapi dengan dosis konvensional dibandingkan dengan dosis tinggi. Namun, untuk pemberian dosis tinggi diwajibkan imobilisasi yang rigid dan lokalisasi yang akurat dengan menggunakan navigasi stereotaktik dan verifikasi dengan panduan pencitraan radiologi. Radiosurgery hanya dapat diberikan pada kelainan intrakranial yang berukuran kecil.

Abstract

ABSTRACTCancer is related to a deterioration of nutritional status and quality of life (Qol), but the extent of these conditions in patients with breast cancer has not been studied well. Malnutrition is prevalent among cancer patients and maybe correlated with altered quality of life. The aim of this study is to evaluate the association of QoL and nutritional status after breast cancer diagnosed. Nutritional status was evaluated with Patient Generated Subjective Global Assessment and QoL using Short form 36 (SF-36) and also with the specific module for breast cancer patients. A consecutive sampleof twenty two patients diagnosed with breast cancer was evaluated. The associations of QoL with stadium and nutrition status were evaluated using T-test analysis. The mean of body mass index was 21.3 kg/m2. Fifty percent patient have menopause. Most patients were stage II (77.3%), the others stage III (18.2%) and stage I (4.5%). Sixty eight point two percent had risk of malnutrition. The stadium of tumor was significantly related to physical functioning (p < 0.000), physical limitation (p < 0.024), emotional limitation (p < 0.013), well-being (p < 0.020), health changes (p < 0.010). Thestatus of nutrition was significantly related to physical functioning (p < 0.001), loss of energy (p < 0.010) and general health (p <0.005). For Conclusion, the status of nutrition breast cancer patients were related to QoL especially physical functioning, loss of energy and general health after they were diagnosed.ABSTRAKKanker sangat terkait dengan perburukan status nutrisi dan kualitas hidup. Namun demikian, belum banyak studi yang yang melaporkan masalah nutrisi dan kulitas hidup pada kanker payudara. Malnutrisi sering ditemukan pada kanker dan mungkin terkait dengan perubahan kulaitas hidup. Tujuan penelitian ini adalah untuk mengevaluasi hubungan antara kualitas hidup dengan status nutrisi setelah kanker payudara terdiagnosis. Evaluasi status nutrisi dilakukan dengan Patient Generated Subjective Global Assessment dan kualitas hidup dengan Short form 36 (SF-36) sertakarakteristik pada kanker payudara. Sampel diambil dengan teknik konsekutif terhadap 22 pasien yang terdiagnosis kanker payudara. Hubungan kualitas hidup dengan stadium kanker dan status nutrisi dinilai menggunakan analisis T-test. Indeks massa tubuh rata-rata adalah 21,3 kg/m2. Lima puluh pasien telah menopause. Terbanyak adalah stadium 2 (77,3%), stadium 3 (18,2%), dan stadium 1 (4,5%). Enam puluh dua koma dua persen berisiko malnutrisi. Stadium tumor secara bermakna berhubungan dengan fungsi fisik (p <0,000), keterbatasan fisik (p<0,024), keterbatasan emosi(p<0,013), rasa nyaman (p<0,020), dan perubahan kesehatan (p<0,000). Status nutrisi berhubungan secara bermakna dengan fungsi fisik (p<0,001), kehilangan energi (p<0,010), dan kesehatan secara umum (p<0,005). Simpulan, status nutrisi pasien kanker payudara berhubungan dengan kualitas hidup, terutama fungsi fisik, kehilangan energi, status kesehatan umum setelah mereka terdiagnosis.

Abstract

ABSTRACTThe aim of this study is to search relationship between using of hormonal contraception and breast cancer incidence. This study uses observational analytic method with case contol design. Group case is the women in POSA RSUD Dr. Soetomo diagnosed as breast cancer patient. Group control is the women in POSA RSUD Dr. Soetomo who are not diagnosed breast cancer. Then, calculate the Odds Ratio using 2x2 crosstabulation and p value using chi square statistictest with SPSS program. There are 96 women of case group sample and 96 women of control group sample are used in this study. Calculation of regression logistic multivariate analysis shows that there is significant relationship between using hormonal contraception and breast cancer incidence in POSA RSUD Dr. Soetomo with p=0.001, OR=2.990, woman who uses hormonal contraception has 2.990 times risk bigger than the woman who doesnt use hormonal contraception. There is significant relationship between using hormonal contraception and breast cancer incidence inPOSA RSUD Dr. Soetomo and woman who uses hormonal contraception has 2,304 times risk bigger than the woman who doesnt.ABSTRAKTujuan dari penelitian ini adalah untuk mengetahui hubungan antara pemakaian KB hormonal dengan kejadian kanker payudara. Penelitian ini menggunakan metode analisis dengan desain kasus kontrol. Kelompok kasus adalah wanita di POSA RSUD Dr. Soetomo yang menderita kanker payudara, dan kelompok kontrol adalah wanita di POSA RSUD Dr. Soetomo yang tidak menderita kanker payudara. Kemudian Rasio Odds dihitung menggunakan tabel 2x2 dan nilai p menggunakan tes chi square dengan program SPSS. Ada 96 kelompok kasus dan 96 kelompok kontrol yang ada dalampenelitian ini. Setelah dihitung dengan analisis multivariat regresi logistik menunjukkan bahwa ada hubungan bermakna antara penggunaan KB hormonal dengan kejadian kanker payudara pada wanita di POSA RSUD Dr. Soetomo dengan nilai p= 0,001 dan OR=2,990 yang berarti wanita yang menggunakan KB hormonal memiliki risiko 2,990 kali lebih besar terkena kanker payudara dibanding yang tidak menggunakan. Kesimpulannya, ada hubungan bermakna antara pemakaian KB hormonal dengan kejadian kanker payudara pada wanita di POSA RSUD Dr. Soetomo dan wanita yang menggunakan KB hormonal memiliki risiko 2,990 kali lebih besar terkena kanker payudara dibanding yang tidak menggunakan.

Abstract

ABSTRACTThere are some evidences that the metabolic changes in breast cancer patients with high body mass index (BMI) associated with insulin resistance and, in particular, the related alteration in cytokine production by adipose tissue which are major contributors to the aggressive behavior of breast cancer that develop through their effects in angiogenesis and stimulation of invasive capasity of cancer cells. Retrospective cohort study conducted at the Dharmais National Cancer Hospital aimed to determine the effect of BMI on five-year disease-free survival (DFS) breast cancer patients. This study was conducted from August to November 2014. The samples in this study were collected consecutively as many as 127 patients. From this study, it is known that the five-year DFS of breast cancerpatients was 70.0%. Based on the category of BMI, breast cancer patients with high BMI (>22.9 kg/m2) had the biggest DFS, followed by low BMI (<18.5 kg/m2) and normal BMI (18.5 22.9 kg/m2) that the precentages successively were 75.5%, 68.6%, and 60.4%. Multivariate analysis showed that BMI was not associated with the events of recurrence or metastases (HR 1.055; 95% CI 0.413-2.678) after being controlled by other variables, such as education, sosioeconomic, staging, lymph node involvement, histopathology, occupation, and biological subtypes.ABSTRAKBeberapa bukti menunjukkan perubahan metabolik pada pasien kanker payudara dengan indeks massa tubuh (IMT) tinggi berhubungan resistansi insulin dan khususnya perubahan terkait produksi sitokin oleh jaringan adiposa yang merupakan kontributor utama terhadap sifat agresif dari kanker payudara yang berkembang melalui pengaruhnya terhadap angiogenesis dan stimulasi kemampuan invasif dari sel kanker. Studi kohort retrospektif yang dilakukan di Rumah Sakit Kanker Dharmais ini bertujuan untuk mengetahui pengaruh IMT terhadap disease-free survival (DFS) lima tahun pasien kanker payudara. Penelitian ini dilakukan dari Agustus sampai November 2014. Sampel yangdigunakan pada studi ini diambil secara konsekutif sebanyak 127 pasien. Dari studi ini, diketahui bahwa DFS lima tahun pasien kanker payudara adalah 70,0%. Berdasarkan kategori IMT, pasien kanker payudara dengan IMT tinggi (>22,9 kg/m2) memiliki DFS lima tahun yang paling besar, yaitu 75,5% ; diikuti pasien dengan IMT rendah (<18,5 kg/m2) sebesar 68,6%; dan 60,4% untuk pasien dengan IMT normal (18,522,9 kg/m2). Hasil analisis multivariat menunjukkan bahwa IMT tidak memiliki asosiasi dengan kejadian kekambuhan atau metastase (HR=1,052, 95% CI 0,413-2,678) setelah dikontrol oleh variabel pendidikan, sosioekonomi, stadium, keterlibatan kelenjar getah bening, histopatologi, pekerjaan, dan subtipe biologis.

Abstract

ABSTRACTThe aim of this study to evaluate clinical and management of penile cancer patients at Sardjito Hospital. Data were collected from medical record of Sardjito Hospital from 2006 - 2013. The clinical factors and therapy were studied from each patient. Correlation between age and stadium of disease were analyzed using Chi-Square Test while the correlation among pathological grade and T, N and M stage were analyzed by using Fisher Test. There are 35 cases of penile cancer with mostly of 40-60 years old man (45.7%) , circumcised (42.9%), squamos cell type (91.4%) and perform partial penectomy (45.7%) followed by chemotherapy (6 patients) and radiotherapy (4patients). There are no correlation between age and stadium of cancer (P>0.05), also among pathological grade with T, N and M Stage (P>0.05). Penile cancers at Sardjito Hospital mostly found on 40-60 years old, and already developed into advanced disease. Thetherapy is consist of partial penectomy followed by chemo- radiotherapy.ABSTRAKPenelitian ini bertujuan untuk mengevaluasi klinis dan manajemen kanker penis di Rumah Sakit Sardjito. Data dikumpulkan dari rekam medis RS Sardjito tahun 2006-2013. Faktor klinis dan terapi dipelajari dari tiap pasien. Hubungan antara usia dan stadium penyakit dianalisis menggunakan tes Chi-Square, sedangkan hubungan antara tingkatan histopatologi dengan stadium T, N, M dianalisis dengan tes Fisher. Ada 35 kasus kanker penis dengandominan pada usia 40-60 tahun (45,7%); telah disunat (42,9%); tipe karsinoma sel skuamosa (91,4%); dan dilakukan penektomi parsial (45,7%) diikuti dengan kemoterapi (6 pasien) serta radioterapi (4 pasien). Tidak ada hubungan antara usia dengan stadium penyakit (>0,05) dan juga antara tingkatan histopatologi dengan stadium T, N, M (p>0,05). Kanker penis di RS Sardjito kebanyakan ditemukan pada usia 40-60 tahun dan telah berkembang menjadi tingkat lanjut. Terapi terdiri dari penektomi parsial diikuti kemo-radioterapi.
