Abstract

Introduction: Antithyroid drugs are one of the treatment options for hyperthyroidism, alongside surgery and radioactive iodine treatment. The antithyroid medications used are thiocarbamide/thiocyanate derivatives, such as propylthiouracil (PTU) and methimazole. Information on the characteristics of hyperthyroid patients, especially regarding possible hematological adverse effects such as agranulocytosis, thrombocytopenia, and pancytopenia, can assist physicians in considering treatment options. Methods: This descriptive, retrospective study used medical records to describe the hematological profile of hyperthyroid patients receiving antithyroid medication therapy at the Endocrine, Metabolic, and Diabetes Polyclinic of RSUP Prof. dr. I.G.N.G. Ngoerah, Denpasar, Bali in 2023. Results: A total of 261 patients were included. The most common age range was 51–60 years (64 patients, 24.5%), and the majority were female (78.5%). The duration since hyperthyroidism diagnosis was most often <12 months (129 patients, 49.4%). The most frequent diagnosis was suspected Graves’ disease (187 patients, 71.6%), and most patients (210, 80.5%) had no comorbidity. In the methimazole group (218 patients, 83.5%), no patient (0%) developed agranulocytosis, 70 (32.1%) had hemoglobin <12 g/dl, and 14 (6.4%) had platelets <150 ×10³/μl. In the propylthiouracil group (43 patients, 16.5%), 1 patient (2.3%) developed agranulocytosis, 18 (41.9%) had hemoglobin <12 g/dl, and 3 (7%) had platelets <150 ×10³/μl. Discussion: The low occurrence of agranulocytosis (0% with methimazole and 2.3% with propylthiouracil) indicates that antithyroid drug-induced hematologic abnormalities are very rare, consistent with the reported figure of 0.2–0.5% in patients with Graves’ disease. Differences between groups may be influenced by study population characteristics, genetic factors, duration of drug use, and the unequal proportion of the two treatment groups. Conclusions: Hematologic abnormalities among hyperthyroid patients receiving antithyroid drugs were low and tended to be more frequent in the propylthiouracil group. Normal neutrophil counts were found in 191 patients (87.6%) of the methimazole group and 39 patients (90.7%) of the propylthiouracil group.

Abstract

Osteoarthritis is a degenerative joint disease that affects the synovial membrane, cartilage, ligaments, and bones. Several risk factors for osteoarthritis include increasing age, family history of generalized osteoarthritis, heavy physical activity, obesity, and previous joint trauma or deformity. This study aimed to identify the knee osteoarthritis risk factors associated with patients’ quality of life. A cross-sectional study was conducted among 91 patients with knee osteoarthritis in Ubung Village, Denpasar City. Data were collected using the Short Form-36 (SF-36) questionnaire to assess the quality of life of the patients. Body weight was measured using a weighing scale, and height was measured using a stadiometer to determine body mass index (BMI). The results showed that several factors were significantly associated with poor quality of life among patients with knee osteoarthritis in Ubung Village, including age >60 years (odds ratio [OR]=4.331), female sex (OR=5.571), excess body weight (OR=7.353), and vigorous physical activity (OR=3.368). Overall, 73.6% of patients demonstrated a good quality of life. However, domain-specific analysis revealed moderate to low median scores in several components, including work limitations due to physical health disorders, pain, and general health. Bivariate analysis furtherdemonstrated that age >60 years, female sex, overweight or obesity status (BMI >23 kg/m²), occupations involving heavy physical activity, and a history of knee trauma were significantly associated with poor quality of life.

Abstract

Background: Tuberculosis (TB) remains the leading opportunistic infection and cause of mortality among people living with human immunodeficiency virus (HIV). Micronutrient deficiencies, particularly zinc deficiency, have been implicated in immune dysfunction and increased susceptibili.ty to TB. However, data regarding the role of serum zinc levels in HIVTB coinfection in Indonesia remain limited. Objective: To evaluate low serum zinc levels as a risk factor for tuberculosis infection among HIV-infected patients. Methods: This was a nested case-control study conducted at RSUP Prof. dr. I.G.N.G. Ngoerah, Denpasar, Bali, Indonesia, between May and October 2024. A total of 60 adult HIV patients wereenrolled, consisting of 30 HIV-TB coinfected patients (cases) and 30 HIV patients without TB (controls), matched by age and sex. Serum zinc levels were measured using inductively coupled plasma-mass spectrometry (ICP-MS). Tuberculosisdiagnosis was established using the GeneXpert MTB/RIF molecular assay. Bivariate and multivariate logistic regression analyses were performed to identify independent risk factors for TB coinfection. Results: Low serum zinc levels(<70 mcg/dl) were significantly more frequent among HIV-TB patients compared to controls (76.7% vs. 30.0%). Multivariate logistic regression showed that zinc deficiency was independently associated with TB coinfection (adjusted odds ratio [aOR] 5.56; 95% confidence interval [CI] 1.431–21.590; p=0.013). Other variables, including lack of isoniazid (INH) preventive therapy and CD4 count <200 cells/μl (aOR 2.64; 95% CI 0.54–12.98; p=0.223), were not independently associated with TB coinfection. Conclusion: Low serum zinc levels are an independent clinical risk factor for tuberculosisamong patients with HIV. Assessment of zinc status may help identify HIV-infected individuals at higher risk for TB and support targeted preventive strategies.

Abstract

Knee osteoarthritis is a whole-joint disease leading not only to structural damage but also to pain, functional limitations, impaired daily activities, and reduced quality of life. This article discusses the association between various major risk factors for knee osteoarthritis, including advanced age, obesity, female sex, history of knee injury, repetitive mechanical loading, muscle weakness, joint malalignment, and metabolic comorbidities, with patients’ quality of life. The severity of radiographic findings in knee osteoarthritis does not always correlate with the intensity of pain or functional limitations experienced by patients. Therefore, assessment and management should be oriented toward improving patients’ function and quality of life through education, physical exercise, weight management, rational pharmacological treatment, and selective intraarticular interventions. Treatment success should ultimately be evaluated based on its ability to improve patients’ mobility, independence, and social participation.
