Abstract

Heart failure with reduced ejection fraction (HFrEF) is a complex clinical syndrome characterized by a left ventricular ejection fraction ≤ of 40%. Primarily the etiology of heart failure is due to coronary artery disease (CAD). Patients with multivessel disease are considered for revascularization procedures with coronary artery bypass grafting (CABG). We report patient with HFrEF due to three-vessel disease (3VD) is not indicated for revascularization and only gets a conservative therapy to improve the patient's quality of life. A 52-year-old man visited the cardiologist to get control his condition. The patient experienced shortness of breath, heartburn, cold sweat, and fatigue. The physical and blood examination was normal, electrocardiography showed fragmented QRS complex, V1-V3 poor R waves progression, and V5-V6 T waves inverted. The echocardiography showed a result of ejection fraction is only 30%. He was treated with acetylsalicylic acid, bisoprolol, valsartan, furosemide, nitroglycerin, spironolactone, and isosorbide dinitrate. Treatment with CABG was not carried out. The ejection fraction was raised to 40% and he was clinically improved. HFrEF due to CAD3VD could be treated with fully conservative therapy and not indicated for revascularization treatment because of coronary anatomy and myocardial viability. A comprehensive approach should be evaluated by Heart Team.

Abstract

Osteoarthritis is a degenerative disease characterized by progressive damage to joint cartilage. The prevalence of osteoarthritis will continue to increase and continue as the population and age increase. Cases of knee osteoarthritis are more common than other types of osteoarthritis. The high prevalence of osteoarthritis will have implications for the high treatment regimen given to patients. Irrational use of drugs can lead to drug-related problems (DRPs). This study used a descriptive design. The sample in this study were knee osteoarthritis patients at the Bina Sehat Jember Hospital. The sampling technique used is purposive sampling. The data in this study used secondary data from patient medical records for the period January 2020 - April 2021. A total of 3 knee osteoarthritis patients have the potential to experience drug-related problems in the drug interaction category.

Abstract

The coronavirus disease 2019 (COVID-19) was declared an International Public Health Emergency (PHEIC) on 30 January 2020 by WHO. Many studies have analysis the clinical, radiological, laboratory profile, and treatment strategies of COVID-19, but there are still no studies on the factors that influence the negative conversion (NCT) of RT-PCR for COVID-19 in Indonesia. This type of study was cross-sectional and included 25 confirmed COVID-19 inpatients at Aminah Hospital Sidoarjo with a median NCT 8 days from initial hospital admission. Men need longer conversion time than women (p-value 0.044). The severity of COVID-19 and patient comorbidities were significantly associated with NCT of COVID-19 RT-PCR (p-value 0.035). Patients with severe severity at baseline had a longer conversion time (9 days) than those with moderate (8 days) and mild (7 days). Patients with comorbidities (diabetes, hypertension, or both) required longer NCT of RT-PCR (9 days) than those without comorbidities (7 days). The results of the log-rank test of age, leukocyte count, platelets, hemoglobin, lymphocyte and granulocyte percentages did not have a significant relationship with NCT of RT-PCR. Gender, case severity, and comorbid history of COVID-19 patients are factors that are associated with COVID-19 RT-PCR conversion time. Keywords: Factor, Conversion, RT-PCR COVID-19

Abstract

Delays returning inpatient medical record files at the Purwoharjo Health Center reached 54.01%, far from the standard 100% return target within 2x24 hours after the patient returned home. The purpose of the study was analyzing the factors causing the delay in returning medical record files at the Purwoharjo Health Center. The research subjects were 8 informants. Collecting data using interviews, observation, documentation, priority problems with USG, and improvement planning to fix problems with FGD. The results of the analysis of the delay in returning inpatient medical record files due to lack of knowledge of officers on the standard of returning medical record files, indiscipline of officers in filling medical record files, SIMPUS often an error, non-optimal use of expedition books, incomplete filling of medical record files, patient motivation in the form of encouragement from medical record officers have been done but officers still often return medical record files delayed. Priority problems include incomplete filling of patient medical files, indiscipline in filling patient medical record files, and lack of motivation in the form of reward. Improvement planning is making SOP for filling the medical record, improving SOP for returning the medical record, posting SOP in every officer room, giving the reward.

Abstract

When the skin is scratched by rubbing it on a rough surface, ginger contains anti-inflammatory compounds that help accelerate the healing of scratches. The therapeutic effect of elephant ginger extract (Ginger Officinale Var Rosc) on white male rats (Rattus norvegicus) Wistar strain? The only real test was an experiment with a control group design. This study was conducted for seven days with samples divided into four groups, namely P1 (NaCl 0.9%), P2 (Gel 0%), P3 (Ginger Agar 8%), and P4 (Ginger Agar 12%). The gel extract application was observed on the 1st, third and fifth days. On the seventh day, it was analyzed by the One Way ANOVA test. The results of the average exfoliated wound area on day 7 P1 = 1.47 cm2, P2 = 0.58 cm2, P3 = 0.31 cm2, P4 = 0 cm2, the average healing rate P1 = 4.9%/day, P2 = 10.6%/day, P3 = 12.31 performances. %/day, P4 = 14.2%/day. The results of the one-way ANOVA test showed a significant effect (p < 0.05). Elephant ginger extract gel (Zingiber officinale var rosc) can accelerate the healing of abrasions in white rats (Rattus norvegicus).

Abstract

Background: In Baruharjo Public Health Center in Trenggalek, hypertension is ranked 1 as the most common cases in 2021. Based on the results of Riskesdas in 2018, the increase in cases of hypertension in Indonesia is in line with the prevalence of obesity. Method: This study is a Systematic review. 34 journals published at least 5 years ago were acquired. Journals contain the topic of health problems about obesity as a risk factor for hypertension. Result: Through the process of reviewing and selecting articles, researchers obtained 34 journals that matched the inclusion and exclusion criteria, and were relevant to this research. From all the 34 journals that we have reviewed, most of them found a significant relationship between obesity and hypertension. Only 1 journal that showed if there was no relationship between obesity and hypertension, but there is still no further explanation about the reason why there was no relationship between both of them. Discussion: Someone with an overweight/obese BMI is four until eight times more likely to have hypertension than a person with normal BMI. Obesity can cause hypertension through various mechanisms, either directly or indirectly. The pathophysiology of obesity induced hypertension involve activation of the sympathetic nervous system and the RAAS or renin-angiotensin-aldosterone system. In addition, the occurrence of endothelial dysfunction and the abnormalities in kidney function are also important factors in the occurrence of hypertension in obese people. Every 10 percent increase in body weight can increase systolic levels by about 5.6 mmHg. Conclusion: Obesity is strongly associated with hypertension. Losing even a small amount of weight if you're overweight or obese and or controlling your weight can help reduce your blood pressure.

Abstract

Background: Dengue hemorrhagic fever (DHF) is a viral infection transmitted by mosquitoes and is one of the health problems in community. DHF is currently still a health problem in Indonesia because the incidence of DHF cannot be reduced. The DHF morbidity in Indonesia in 2015 reached 50.75 per 100,000 populations, and the Incidence Rate (IR) in 2016 reached 78.85 per 100,000 populations, exceeding the national IR target (49 per 100,000 populations). The presence of mosquito larvae is an indicator of mosquitoes in an area. As a matter of fact, there are many factors that affect the presence of mosquito larvae. Objective: To determine the house environment factors related with the presence of mosquito larvae Method: Observational analytic with cross-sectional design. The sampling used cases and controls of 285 houses. The sample filled out the informed consent agreement, then the researchers gave the questionnaire, and made observations, next the researchers filled out the checklist according to the observation. The data were analyzed in bivariate technique with chi-square test, followed by multivariate test using logistic regression test. Results: The results of the multivariate test showed 7 factors that influenced the presence of larvae, consisting of the variables of PSN (Mosquito Breeding Ground Eradication) action (p = 0.012; OR = 0.022; CI = 0.001-0.435), PSN attitude (p = 0.005; OR = 1658247.9; OR: CI = 0.000-0.014), Number of people in the house (p = 0.013; OR = 0.071; CI = 1.731-113.550), Frequency of cleaning containers (p = 0.006; OR = 1139.1; CI = 0.000-0.080), Mosquito breeding sites (p = 0.006; OR = 0.006; CI = 0.424-6148.76), Fish in containers (p = 0.003; OR = 434.272; CI = 0.000-0.134), container location (p = 0.007; OR = 0.006 ; CI = 4.047- 4653.77). Conclusion: Many factors affect the presence of mosquito larvae, both in the home environment and containers owned by residents of the house. The presence of fish in containers is the most influential factor. Breeding fish as mosquito larva predators is included as parts of 3M Plus which is quite easy to do by the community. Thus, it requires maximum effort to increase knowledge of the community about these steps.

Abstract

Background: Body composition consisting of body fat and non-fat body mass is an indicator of health profile. One method for assessing body fat mass is to measure body fat percentage. Body fat can be distributed in the trunk and extremities with many influencing factors, including gender. Still, it is not yet known whether body fat distribution will be different in different categories of body fat percentage. Objective: To understand the differences in the dominance of body fat distribution based on body fat percentage in late adolescent male and female. Method: This observational analytic study used a cross-sectional approach involving 40 male and female respondents (UMM Medical Faculty students) selected using purposive sampling. The data taken was in the form of height, mass, BMI and SFT which were then entered into the Jackson, Pallock, and Ward (1980) formula. The data obtained were analyzed using the fisher's test. Result: The Fisher's test results showed a sig value of 0.409 > 0.005. meaning not significant, so there is no relationship between the distribution of body fat with body fat percentage Conclusion: There is no difference in the dominance of body fat distribution based on body fat percentage in late adolescent male and female

Abstract

Administration officer is a profession that sits for a long duration, working on a computer device. Sitting for a long time can cause low back pain. Low back pain is one of the diseases that can cause disability and disorders. This disease can reduce effectiveness in work. This pain arises from a long sitting position causing continuous spasms on muscle. Research Objective: Knowing the relationship between sitting duration while working with lower back pain in administration officers. Research Method: This study used Cross-Sectional research methods with a total sample in this study of 202 people. Sample were given Rolland Morris questionnaires and responded directly. The data was analyzed using Chi-Square Test. Results: 16.7% of people with a long sitting duration <4 hours had complaints of low back pain, and as many as 48% of respondents with a working duration of > 4 hours had complaints of back pain. Statistical test results using the Chi-Square Test obtained a p = 0.026 < 0.05. Thus it can be concluded that there is a long-standing relationship of sitting with complaints of low back pain in Administration officer of Saiful Anwar Malang Hospital. Conclusion: Long sitting duration done for several years can cause disorders in the body. The working period causes a continuous static load, and workers who ignore ergonomic factors can cause low back pain.

Abstract

The Covid-19 pandemic that is currently hitting Indonesia requires the active role of all parties, including from institution Education. University Muhammadiyah Poor is the only private campus that has carried out general check-up activities with a covid-19 screening examination or rapid test for all lecturers and staff. The purpose of this activity is as a form of campus concern for the health of its lecturers and employees, especially during the current Covid-19 pandemic. In addition, the University of Muhammadiyah Malang held activities like this to support Government among them through activity devotion as part from implementation tridharma task of higher education. This research method uses an analytical survey method. Sampling using total sampling technique. The stages of implementing this activity began with registration carried out by UMM campus lecturers and employees, complete blood tests, and rapid tests and PCR swabs for those whose rapid results were reactive, and self-isolation if there was a lecturer or employee which results swab the PCR positive. Results from study this is could knowing the health status of lecturers and staff at the University of Muhammadiyah Malang. Of all employees who have registered as participant GCU this obtained 22 employee which results the swab positive or around 1.7% of the total number of employees who have undergone health screening (rapid test). This figure proves that the number of confirmed cases of Covid is still below 10%. As a follow-up, independent isolation is carried out for lecturers and employees who have tested positive for Covid-19 while remaining under supervision by campus staff and the Muhammadiyah University covid task force. Poor.
