Abstract

Materials and methods: In the period from 2010 to 2017 quite a few people were examined, i.e. a ‘comparison’ group of 200 people and an observation group, i.e. 1667 people. The results were as follows. Optosonographic monitoring in 935 patients (56%) revealed surgical thyroid disease, in 732 patients (44%) revealed a surgical pathology of the mammary glands. Among the identified neoplasms, 741 (79%) were benign ones, and 194 (21%) were malignant ones. The number of patients with mammary neoplasms was 732 patients. Of these, 634 were benign ones (87%) and 95 were malignant ones (13%). Results: The sensitivity in the differential diagnosis by optosonographic method of benign thyroid neoplasms is increased from 63% to 95%, malignant neoplasms from 66% to 96%. Reliability increase from 60% to 98% for the diagnosis of benign breast neoplasms, for malignant ones from 37% to 98%. This allows the use of non-invasive optosonographic monitoring to verify the diagnosis.Keywords: Optosonographic method, Implantation, Mammary gland, Thyroid gland.

Abstract

A bacterial isolates of (80) from (17) soil samples has been collected from the rhizosphere of different plants of many regions Mosul city; the isolates were initially identified as a Streptomyces genus according to the chalky appearance and the odor of the moist earth. Antagonistic activity against fungi isolated from the soil rhizosphere of plantshas been tested. Only (49) isolates (i.e. 57.64%) proved their antagonized ability against one or more of the tested fungi. The best isolation  that gave antifungal efficacy against all fungi, and was the largest diameter of the inhibition area against A. flavus (48 mm) followed by M. Phaseolina (27 mm), the least inhibition zonewas (12 mm)  of Sclerotiumcepivorum (12 mm). The isolation elected to the level of the species was identified by the morphological, physiological and hematological tests and was found to belong to the species of Streptomyces lydicus. The identification of the antibiotic extracted by ethyl acetate was done using three solutions that are butanol: pyridine: water (6:3:4), methanol: dichloromethyne: water (1:1:1) and butanol: acetic acid: water (6:4:3). A single spot appeared on a thin-layer chromatography plate (TLC) indicating the production of one type of antibiotic. It was noticed that Rf for the spot in the 3 solvents was 0.82, 0.47, and 0.80 respectively. The solvent with Rf 0.86 was the best hence it gave the highest inhibition for the growth of at least 2 fungi. Ultra violet absorption for the antibiotic was (286) and appeared that this compound was a Macrolides. Also Infra red (IR) was measured suggesting that the expected structure of the antibiotic is Oleandomyein.Key words: Streptomyces antibiotics, Bacteria, Antifungal, TCL plate.

Abstract

Background: There are still many unresolved problems that reduce frequency of clinical and technical success of angioplasty of lower limb arteries. One of such problems is impact of the arterial pedal arch revascularization on the clinical result of angioplasty in patients with IV class critical lower limb ischemia according to Pokrovsky-Fontaine classification. Material and methods: The results of treatment of 98 patients with peripheral arterial disease (PAD), complicated by critical lower limb ischemia of IV class according to Pokrovsky-Fontaine classification were analyzed in the early postoperative period and 1 year later after the completed angioplasty. Patients were divided into two comparable groups: the first group-the patients with the arterial pedal arch revascularization, the second group – the patients whom it had been impossible to revascularize the arterial arch. Results: “Good result” in patients of the first group in the early postoperative period and one year later was 23.1% and 22.7% respectively. In the patients of the second group-9.2% and 5.6%. The both group patients avoided amputations in the early postoperative period, but one year later 53.3% of the second group patients with the “unsatisfactory result” underwent amputation. Conclusions: According to the results, the arterial pedal arch revascularization influences on the clinical results and decreases the quantity of amputations in patients with purulent-necrotic changes in the lower limb. Abstract

Turbidity is a major obstacle in preservation of fruit juice and concentrate. This phenomenon is also not acceptable in handling and distribution of commercial beverage. Fruit juice is a complex of soluble and insoluble multi components in suspension. Clarification is a crucial step to remove pectin, carbohydrate, metal ion as well as solid particles existing in fruit juice to achieve a clear and stable status. As a natural abundant polymer, chitosan has been proven as an alternative approach for juice clarification with diversified manipulations in appropriate cost. Our study focused on the feasibility of chitosan as clarifying agent to physico-chemical and organoleptic properties of pineapple juice during clarification. Different concentrations of chitosan (0.05, 0.075, 0.1, 0.125, 0.15%) and agitation speeds (40, 60, 80, 100, 120 rpm) have been investigated fruit juice to evaluate turbidity (NTU), viscosity (cP), total soluble solid (oBrix), total phenolic (mg GAE/g), flavonoid (mg GE/g) and sensory score after clarification. Our results revealed that 0.125% chitosan; agitation speed 80 rpm were adequate and efficient for juice clarification. As being nontoxic and biodegaradable, chitosan could be effectively utilized as an alternative agent for refining of fruit juice.Keywords: Pineapple juice, Clarification, Chitosan, Turbidity, Viscosity, Total soluble solid, Total phenolic, Flavonoid, Sensory score.

Abstract

Children are characterized as healthy when they grow and develop adequately. This is determined through age-appropriate ideal height and weight measurements. This study aimed to quantify the prevalence of stunting among 10-year old children. This observational study had a cross-sectional design. This descriptive research was conducted from October to November 2019 using a total sample of 392 children obtained through the multistage proportional sampling to select participants from the student in 19 first elementary school in the study area. The anthropometric measurements of height were taken using a microtoice. The measurement results were compared with the World Health Organization (WHO) AntroPlus 2007 standards for 10-year old children. Height was measured using standard methods and height for age and the z-score was computed to assess stunting. The hypothesis test was done using ANOVA. The prevalence of stunting among 10-year old children in central Sulawesi, Indonesia is 24.7%. Poso's district has the highest percentage at around 35.9%, Donggala district is around 35.7%, Sigi district is around 16.5% and Palu City is around 12.5%. The highest mean for height was found in Palu City at 135,7 cm and the lowest was in Donggala district at 129,9cm. ANOVA showed a significant value of <0.001 with z-scores of -0.72, -1.63, -1.45 and -1.09 in Palu City, Donggala District, Poso and Sigi respectively. The prevalence of stunting in Central Sulawesi for children aged 10 years old is 24.7%. Donggala and Poso district have the highest prevalence of stunting. Some strategies need to be executed in order to improve the level of nutrition of elementary school children. This can be done by providing safe and hygienic school canteens, promoting healthy food diversity, encouraging more fish consumption and other healthy protein ingredients.Keywords: Children, South Sulawesi, stunting.

Abstract

There are some particularities of managing patients who have been taking anticoagulants for a long time and who need dental treatment. People who regularly receive oral anticoagulant therapy are at increased risk of bleeding during and after surgery in the oral cavity. Clinicians must constantly weigh the risk of postoperative bleeding with continued therapy, as well as the risk of thromboembolic complications when the drug is discontinued. The purpose of this study is to study the effectiveness and safety of the indirect anticoagulant Warfarin in patients with diseases of the cardiovascular system undergoing dental treatment. In 2017-2018, based on the Moscow dental clinic No. 2 (Russian Federation), studies were conducted in which 350 patients participated, of which 5 were excluded from the sample. Moscow cardiological dispensary No. 1 patients who applied for dental care were involved. The first group consisted of 30 people with a diagnosis of severe diseases of the cardiovascular system (coronary heart disease, etc.), the second - 65 people with an average risk of thrombosis, the remaining 250 people of the 3rd group are patients with a low risk of thrombosis. Instead of Warfarin, the first two groups received low molecular weight heparin, the third - continued the course of Warfarin. In addition to general clinical methods (ECG, maintaining the INR level (international normalized ratio), etc.), examinations used a selection of local anesthetics and vasoconstrictors. Significant differences in the INR level before and after the operation between the three groups were not found - all of them were within normal limits. The maximum number of bleeding cases in patients from the 1st group is 2.1 times higher than in the 2nd group (p ≤ 0.01), and 3.5 times higher than in the first (p ≤ 0.001). Between the 2nd and 3rd group the difference was 1.6 times (p ≤ 0.05). Regardless of the use or rejection of Warfarin, the maximum number of bleeding cases during dental procedures occurs in patients with severe cardiovascular diseases. Refusal from Warfarin is justified in case of severe diseases of the cardiovascular system, but in the case of a low level of thrombosis, therapy with Warfarin is possible. The risk of bleeding is evenly distributed within each group, regardless of the combined use of a vasoconstrictor and anesthetic or only anesthetic, while maintaining the difference between the groups. We have shown that the severity of diseases of the cardiovascular system can also influence the number of cases of bleeding during dental interventions. The complexity of the dental intervention determines the likelihood of bleeding. In the first group, for patients who had one tooth removed, bleeding was observed 5 times less compared with removal of the cyst (p ≤ 0.001), and 4 times less - compared with operations on the pulp (p ≤ 0.001). Pearson's correlation between the type of dental intervention and the number of bleeding in this group is the highest (0.91), 0.82 in the 2nd group, 0.56 in the 3rd. The type of dental intervention, regardless of the use of Warfarin, has a direct relationship with the number of bleeding. Abstract

Background: The application of caring behavior based on knowledge of attitudes and professional behavior is important in nursing services. Nursing students who are carrying out professional education in hospitals must be able to understand and apply caring behavior in providing nursing care to patients. This study analyzes the effects of interpersonal communication, emotional intelligence, supervision, and achievement motivation on the caring behavior of nurses students in applying nursing care. Methods: Quantitative research with a path analysis approach. Sampling used a simple random sampling technique as many as 271 respondents from 871 populations. Data were collected using a developmental questionnaire. Results: interpersonal communication (p=0.385), emotional intelligence (p=0.134), clinical supervision  (p=0.286), achievement motivation (p=0.119) had a positive effect on caring behavior. Conclusion: Increasing interpersonal communication, emotional intelligence, clinical supervision, and achievement motivation can improve caring behavior.Keywords: Achievement motivation, Caring behavior, Emotional intelligence, Interpersonal communication, Supervision.

Abstract

Men’s attachment to masculinity is believed to contribute to the low access to health services. However, there is a paucity in the available literature to explain this further in Indonesia. This study aimed to narrow the gap by providing scientific evidence to explain whether a similar situation is occurring in Indonesia. We conducted a cross-sectional study involving 134 men aged 20-40 years old who lived in Surabaya as participants. The ideology of masculinity, masculinity norms and the gender role conflict were assessed for their correlations with men’s access to primary health care. Univariate analysis was employed to identify the studied variables; and bivariate analysis was conducted to the correlation with men’s use of the primary health care services, using Spearman’s correlation test. The men were moderately attached to the ideology of masculinity (52.2%), masculinity norms (56.7%) and have moderate gender role conflicts (53.7%), and used the primary health care center for services (54.5%). The bivariate analysis resulted that all the three studied variables did not correlate with men’s access to primary health care centers (p = 0.455, 0.654, 0.300 respectively). The results refute the widely accepted notion and suggest that the men’s attachment to masculinity did not contribute to their use of primary health care service. Instead, the men would access the primary health service as early as physical symptoms started to occur.Keywords: Health service utilization, Indonesia, Masculinity, Men, Primary health care.

Abstract

Mango (Mangifera indica L.) is one of the most important tropical fruits. It’s excellent source of nutrients, soluble fibres, bioactive compounds such as carotenoid, ascorbic acid and phenolic.  Mango pulp is very perishable with a short shelf life. It is very vulnerable to dehydration, discoloration, and decomposition. Objective of this study focused on the effectiveness of blanching (60oC in 4 minutes, 65oC in 3.5 minutes, 70oC in 3 minutes, 75oC in 2.5 minutes, 80oC in 2 minutes) to physico-chemical attributes of the minimally-processed mango during frozen storage for 6 months. Our results showed that minimally-processed mango should be blanched in hot water at 75oC for 2.5 minutes to maintain its shelf-life and physico-chemical characteristics during 6 months of frozen storage.Keywords: Mango; minimally-processed; blanching; physico-chemical; shelf-life; frozen storage

Abstract

Patient Development Integrated Records (CPPT) is the documentation for health personnel in conducting the assessment of hospitalized patients according to the Hospital Accreditation Committee Standard (KARS). The CPPT is a tool for all health professions to convert the data and treatment plan for the patient. This would lead to a collaboration and communication between healthcare professionals at the hospital. This study aims to analyze CPPT as the information system between healthcare professionals at the hospital. This design study used a mixed-method with quantitative research as a source of primary data and qualitative research as secondary data. This research used cross-sectional approach as the quantitative tool, involving 76 health workers, most of whom were nurses. The sample was done by systematic sampling. Data collecting was done using questionnaires chi-square bivariate analysis and multivariate logistic regression. Qualitative research was done through interviews, focus group discussions and documentation with the status of 140 documents. The results of this quantitative study showed that the quality of study documentation in hospitals is still 80,3%, good category, and based on documentation study shows 52.9%. Meanwhile, as an information system between professionals was influenced by education, length of work, understanding of the Standard Operating Procedure (SOP) and concern of health professionals who write Patient Development Integrated Records, with the most dominant factor is the knowledge of SOP with (p=0.003). These factors should be a concern for the hospital management as the evaluation in order to improve care to patients, patient safety and protect health care workers from lawsuits.Keywords: Influential factor, Inter-professional information system, Patient development integrated records.
