Abstract

Leukemia granulositik kronik (LGK) merupakan suatu keganasan myeloproliferatif dengan karakteristik ditemukannya kromosom Philadelphia (Ph). Kromosom Ph ini disebabkan oleh translokasi resiprokal antara kromosom 9 dan 22 [t(9;22)] dan terbentuknya fusi gen BCR-ABL1. Fusi gen tersebut akan menghasilkan suatu protein yang menderegulasi aktivitas dari tirosin kinase (p210) yang berperan pada patogenesis LGK.

Abstract

Introduction. The Socal and Hasford scores developed before the era of tirosine kinase inhibitor (TKI) therapy to provide predictions and prognosis for the results of therapy for chronic myeloid leukemia patients (CML). Until now the superiority of Socal and Hasford scores is still debated in predicting the results of therapy in the era of TKI. Imatinib mesylat is a first line TKI in the treatment of CML. This study aims to determine the superior predictive score in predicting hematological and molecular response of patients treated with imatinib mesylate. Methods. A survey study was conducted with data taken retrospectively from medical records. Subjects were chronic phase LGK patients who had at least been given imatinib mesylate therapy for 12 months. Calculation of Socal and Hasford scores was carried out based on patient data when first receiving Imatinib. To see the difference in distribution between the two scores, a marginal homogeneity statistic was used. To see the process of changing the response of patients treated with Imatinib, a suitability test using chi-square and Fischer was conducted with a significant p value, p 0,05. Conclusion. There was no significant difference between Hasford and Socal scores in predicting hematological responses and molecular responses of chronic CML patients. Socal scores and Hasford scores can be used to predict hematological and molecular responses in chronic CML patients treated with imatinib mesylate.

Abstract

Introduction. Hypoalbuminemia is a strong predictor of mortality in nonoperative and operative patients. Albumin plays a role in maintaining oncotic pressure in critical conditions such as sepsis. The administration of exogenous Human Serum Albumin (HSA) in sepsis with hypoalbuminemia conditions is still controversial with varying outcomes.This study aimed to know the mortality ratio between intravenous albumin administration and no intravenous albumin administration in sepsis patients with hypoalbuminemia conditions and also to know the factors that influence the mortality of sepsis patients. Methods. An observational study with a prospective approach involving 75 research subjects aged >18 years with sepsis accompanied by hypoalbuminemia (<2.5 g/dL) treated in the high care unit of RSUD dr. Saiful Anwar Malang during the period 1 September 2018-31 August 2019. Subjects were divided into two groups namely albumin and nonalbumin groups. Both groups were followed during hospitalization until they discharged or died. Differences in mortality between the two groups were analyzed by chi square bivariate test. The most influential factors on mortality were analyzed by multivariate binary logistic regression tests. Results. There were 39 people (52%) from the albumin group and 36 people (48%) from the non-albumin group. Difference in mortality between albumin and nonalbumin groups {25 (64.1%) vs 16 (44.4%) with OR 2, p=0.138} means there was no statistically significant difference. Factors that influence mortality included: SOFA score (OR 34.27, p <0.001), MAP value (OR 8, p<0.001), septic shock (OR 4.31, p=0.03), diabetes mellitus (OR 0.28, p=0.009), respiratory failure (OR 8.02, p <0.001), decreased of consciousness (OR 64.75, p <0.001), cardiovascular failure (OR 6, p <0.001), hematological failure (OR 3.05, p=0.027). The most dominant factor affecting mortality in sepsis patients is decreased of consciousness (OR 2.67, p=0.001). Conclusion. The administration of albumin transfusion did not make a significant difference in the incidence of mortality in sepsis patients with hypoalbuminemia. The most influential factor on mortality of sepsis patients is decreased of consciousness.

Abstract

Introduction. Breast cancer is the most prominent cancer and causing death in women. Treatment of breast cancer requires immunohistochemistry (IHC) to determine prognostic and predictive and imaging tests to determine staging. However, currently in Indonesia only 17 hospital can conduct IHC test. RSUD Kota Bogor like other Type B Hospital have no IHC test facilities. To fullfil that test, its important to refer patient to referal hospital. Because of limitation there are patients who did not do IHC test and continued get anthracycline chemotherapy. The objective of this research is to find out immunohistochemistry profile of breast cancer patients who received anthracycline-based chemotherapy in RSUD Kota Bogor. Methods. A descriptive retrospective study by collecting the breast cancer patients medical records was done since January 2014 to December 2017 in RSUD Kota Bogor. Results. Total of 174 subjects were include in this study which consist of 84 subjects (48%) with IHC test and 90 subjects (52%) without IHC test. The immunohistochemistry profile of breast cancer patients who received anthracycline-based chemotherapy since January 2014 to December 2017 in RSUD Kota Bogor were luminal type (64%), HER2-enriched (18%), and TNBC (18%). Conclusion. Luminal type is the most molecular subtypes in breast cancer patients who received anthracycline-based chemotherapy in RSUD Kota Bogor. However, because of limitation, since January 2014 to December 2017 only 48% of breast cancer patient that get anthracyclin chemotherapy who got IHC test in RSUD Kota Bogor.

Abstract

Introduction. Cardiovascular disease is one of the health problems in Indonesian pilgrims and has become the main cause of death for pilgrims in the past 3 years. Most of them are considered high risk for cardiovascular disease. Some of those risk factors are predicted to have strong association with the pilgrims mortality rate. This study is meant to seek for risk factor associated with the pilgrims mortality rate. Methods. Observational study method of the Indonesian pilgrims in 2017 was conducted to assess the characteristics of Indonesian pilgrims who follow the hajj in 2017 as well as the characteristics of pilgrims who died from cardiovascular disease. Result. The proportion of mortality from cardiovascular disease was 49.2% of all deaths in the Indonesian hajj pilgrims in 2017. Generally, pilgrims are in the category of high risk. Pilgrims who died from cardiovascular disease are mostly aged over 60 years (76.7%) with cardiovascular risk factors including hypertension (53.4%) and obesity (37.3%), and pilgrims with the final wave departure time (53%). Conclusios. The proportion of deaths from cardiovascular disease in the Indonesian pilgrims in 2017 is 49.2%. The characteristics of pilgrims who died with the cause of cardiovascular disease are generally age more than 60 years, pilgrims with hypertension, obesity, and final departure time.

Abstract

Introduction. Sarcopenia is defined as a declined in skeletal muscle mass and strength along with its function may causes an increase in hospitalization, mortality, and health burden. Multi-factorial conditions of the aging process may cause sarcopenia. To assess those multi-factorial conditions in the elderly, a comprehensive geriatric assessment (CGA) method should be used, in which comprises of socio-demographic, medical, psychological, and functional domains. This research aims to analyze the components of CGA, including age, gender, nutritional status, mental status, and functional status as factors associated to sarcopenia in the elderly community in Surabaya. Methods. This community research was a cross sectional designed analytic observational study. The subjects were all elderly people visiting five chosen Posyandu, an integrated health service post, that meet the inclusion and exclusion criteria. A total of 308 data were collected and then bivariate and multivariate analyzes were performed to determine the components related to sarcopenia. Bivariate analysis was performed on components of age, sex, nutritional status, comorbidities, cognitive status, mental status, and functional status. Variables included in multivariate analysis were age, sex, nutritional status, comorbidities, cognitive status, and functional status. Results. The subjects were dominated by elderly females (74.7%). The median of age were 63 years (range 60-100 years). After performing multivariate analysis, three variables had the association to sarcopenia which were nutritional status assessed by MNA score ≤23.5 (OR 3.61, 95% CI 2.11–6.19), age ≥70 years old (OR 2.82, 95% CI 1.58–5.04), and male (OR 1,83, 95% CI 1,04–3,24). An area under curve (AUC) of 66.2% was obtained from the prediction model. Conclusion. The method of CGA has the power to predict sarcopenia of the elderly in the community as much as 66.2%

Abstract

Urinary tract infection (UTI), especially recurrent UTI, is a common problem, occurring in >75% of kidney transplant recipients. Urinary tract infection (UTI) can reduce the quality of life, graft potential, and patient survival. Urosepsis with impairment of graft function is potential long-term sequelae of recurrent UTI in kidney transplantation recipients. We present a case of UTI that developed within a month after renal transplant. The patient got a urinary tract infection repeatedly. We suspect lack of hygiene as the cause of urinary tract infection, the patient was rarely cleaning-up, took a bath, changing clothes, and did not wash after urinating. But, after we did some interventions, the patient still had urinary tract infection. Laboratory findings showed decreasing of renal function since one month before. Urine culture result showed that the cause of infection was Klebsiella pneumonia. Uroflowmetry result showed post void residual about 50-60 cc. We suspected anatomy anomaly at the urinary tract of this patient. To evaluate the renal graft without biopsy, we conducted an MRI and found a pyelonephritis renal graft. We got increasing CMV antibody for opportunistic infection. The last, renal biopsy result was acute cellular rejection. After giving appropriate treatment with ganciclovir 1 x 250 mg up to 6 months and cotrimoxazole 1 x 960 mg for 14 days, the renal function still decreased and so did the urine output, then the patient fell down into graft rejection.
