Abstract

Cervical cancer is one of the main chronic diseases in women with a variety of symptoms throughout the patient's life. The adaptation process of the symptoms is an important part which is the responsibility of patients with chronic diseases such as cervical cancer. Conservation and self-efficacy models can assist nurses through a framework based on four principles of conservation that are integrated with all four components of self-efficacy. The integration of these two models can represent the views of individual holistically, active, and independent, and involves the active participation of the patient in the nursing plan. This can help patients to manage a normal life in order to survive and improve their health status.Keywords: cervical cancer, the integration of conservation and self-efficacy theory.

Abstract

Background: Pain that has been experienced by patients in intensive care can lead them into worse condition. The efforts relieving the pain have been done a lot. One of them is to use music therapy. Nevertheless, the researcher has not found the the research of keroncong music therapy as original Indonesian classical traditional music for pain. Purpose: The aims of this study was to know the effect of keroncong music for pain level of the postoperative patients in intensive care. Method: This research used quasy experimental with pre-post nonequivalent control group design on 32 postoperative patients in critical care rooms. Keroncong music therapy has been implemented to the second day postoperative patients, it has been given twice a day and each implementation the patients listened keroncong music for 20 minutes. The pain level was measured using Visual Analogue Scale (VAS). Result: Postoperative pain level in intervention group (22,94 ± 14,63) significantly decreased than control group (2,06 ± 21,90) with p = 0,003, with a difference of mean 20.88 mm decrease in pain.Conclusion: Keroncong music therapy had effect on the decreasing of pain level of postoperative patients. Keroncong music therapy can be recommended to be independent complementary nursing intervention to decrease the pain level of postoperative patients in intensive care unit.

Abstract

Background: Instead of reduce pain, TENS is also capable of stimulating the peripheral nerves to work optimally and improving blood circulation where the electrodes are applied. Having good blood circulation enhances good wound healing as well. The decrease in pain level will boost the immune system thus the wound healing can also be improved. It needs further studies on the effect of TENS on wound healing. Objective: The research aimed to explore specific effect of TENS on wound healing. Methodology: The research was conducted by using Science Direct article, Medline, Google Search and Pro Quest to find articles which are appropriate with inclusion and exclusion criteria to be reviewed. Results: The necrosis cases of the post-operative wound were higher in the control compared to the intervention group, there were no complications with significant p-value of (P <0.0001). There were differences in the provision of TEENS with additional of heat protocol before, during and after the intervention towards the blood circulation with the significance p value of (P, 0.05). Good granulation occurs, the hair follicle grew well and Pro-inflammatory declined (TNF-α) with the significance p-value of (P <0.05). The comparison of necrosis percentage in five groups were 43.88%, 39.20%, 38.57%, 32.14% and  44.13% in G1 to G5 respectively. The statistical tests proved the TEENS intervention in group G4 was more effective with the significance p value of  0.032. The TENS with frequency of 10 Hz is more effective compared to 100 Hz TENS and control groups who received placebo towards the adrenergic receptor. TENS intervention at a dose of 100 Hz can improve the reactivity of venous blood circulation well. The amount of oedema in the wound is reduced and capillary refills 2 seconds with the significance p-value of (P <0.001) significantly. Discussion: TENS at a frequency of 10 Hz to 100 Hz is the bioelectrical body frequency. At low frequencies, it will be able to stimulate the secretion of endorphin hormone so the patients who received TENS intervention can be more relaxed and feel better as the pain is relieved. The immune system works well and can help the wound heals properly by inhibition of inflammatory factor. Conclusions: TENS therapy is proven to help wound healing. TENS equipment is available everywhere, easy to use, economical, does not cause addiction, and can be given at any time with fewer side effects to the patients. The results of the study cannot be generalised yet. Further research is needed. Keywords: Transcutaneous Electrical Nerve Stimulation, wound healing, literature review.

Abstract

Critically ill adult patients with a ventilator in intensive care often receive treatment that causes pain. Pain is a symptom that often occurs in critically ill adult patients with ventilator and it is very individual. Pain assessment in critically ill patients with the ventilator is needed because of the patient unable to self-report of pain. Pain assessments critically ill adult patients have been developed. However, there is no valid and reliable instrument to assess pain. The objectives of the literature review are to identify the instruments of pain assessment in patients with ventilator. The method of literature was performed through seeking publication of articles in MEDLINE, Google Search, PubMed, and Proquest with keywords pain assessment, tool, critical care, adult, critically ill, unconscious and ventilator. Literatures were undertaken from 2000-2015 with a cross-sectional study design, before and after studies and observational study. The results of the study according to the characteristics of the research was found five pain assessment instruments, they were NVPS, P.A.I.N, Comfort scale, BPS and CPOT. The validity and reliability CPOT is highest among others. All instruments had been measured its validity and reliability, but it had never tested its sensitivity and specificity. So, more researches should be conducted related to the sensitivity and specificity of all the instruments of pain in critically ill adult patients with a ventilator. Keywords : Pain assessment, critically ill, adult, ventilator.

Abstract

Diabetes is one of the chronic health problems and the psychological impact on the patient and is often associated with with several psychiatric disorders. Patients with diabetes were twice as likely to have status anxiety, depression and serious psychological problems. The psychological pressure of patients with diabetes mellitus often associated with the spiritual welfare and compliance religion. The limited information relating to the spirituality of the patient as well as their different views in interpreting spirituality and religion make less than optimal in increasing the spiritual wellbeing of patients that can ultimately help patients increase their quality of life. spirituality and religion how it can be distinguished patients with diabetes who have problems of spirituality and religion of patients who experienced an issue, because both have different concepts and dimensions.

Abstract

Nature sound music is a combination between classical music with sounds of nature. The use of natural sound music like the sounds of birds, waves, wind, and  water flow as a medical therapy has achieved satisfactory results that can improve relaxation, physical condition, and psychological state for individuals of various ages. This study aimed to determine the effect of natural sound music to decreased anxiety on critically ill patients in the ICU. This research is helpful in healing the critical patient, and as the basis for realizing evidence based practice in the management of critically ill patients.This research is using quasi-experimental non-equivalent to the pre-posttest control group design, with consecutive sampling by randomized allocation. Respondents were 20 people the intervention group and 18 people in the control group. Nature sounds music that was used is the sound of birds with a combination of 2 x 30 minutes in the morning (08.00-noon) and in the evening (20:00 to 22:00) for 3 days. Anxiety levels were measured using visual analog scale-anxiety (VAS-A). Data then were analyzed using Wilcoxon and Mann Whitney.The results of this study showed a decrease in anxiety in the intervention group with a value of 62.25 ± 7.304 pretest, posttest 41.65 ± 5.976, whereas the control group pretest 46.55 ± 12.76, 43.00 ± 12.35 posttest. The result of the comparison test on the anxiety before and after intervention in the intervention group p = 0.000 (p <0.05) and in the control group p = 0.007 (p <0.05). While the results of the difference test on the anxiety between the intervention group and the control group were p = 0.000 (p <0.05), which means there is a significant difference in anxiety levels between the intervention group and control group. Conclusion. Music sounds of nature is effective to reduce anxiety in critically ill patients.Keywords: music therapy, natural sounds, anxiety.

Abstract

Background: Sudden deterioration defined as a condition in which a progression over a complex illness. Progression of pain to the patient causing displacement status characterized by the vital signs of patients who experienced abnormality. Nurses in medical and surgical ward almost everytime facing with patients with acute deterioration conditions. Nurses should have confidence when giving proper treatmen for acute patents deterioration. Methods: This study utilize quantitative descriptive research conducted in the hospital ward illness Islam Sultan Agung Semarang. The number of respondents that used in this study were 98 nurses, utilized consecutive sampling technique. Results: The results showed that the average level of confidence gained nurses treating patients in the ward conditions of acute deterioration in the disease is expressed 3.03. It is mean that respondents have moderate level of confidence. Conclusions: The level of confidence of nurses in treating patients with acute exacerbation of conditions needs to be improved to provide excellent service qualityKeywords: nurse, self-confidence, sudden deterioration
