Abstract

Tujuan: menunjukkan adanya perubahan pada best corrected visual acuity (BCVA) dan ketebalan retina sentral (central retinal thickness/CRT) setelah 1 tahun pemberian intravitreal ranibizumab pada kasus Age Related Macular Degeneration (ARMD) neovascular berdasarkan jumlah injeksi dan metode pemberian. Metode: Penelitian retrospektif ini terdiri dari 38 mata yang mendapatkan injeksi ranibizumab pada tahun 2011-2012 di Jakarta Eye Center. Berdasarkan jumlah injeksi, 24 mata mendapatkan 1-3 kali injeksi/tahun dan 14 mata mendapat lebih dari 3 kali injeksi/tahun. Berdasarkan metode pemberian, 10 mata diberikan loading dose dan 28 mata diberikan injeksi sesuai dengan kebutuhan. Hasil: usia rata-rata subyek adalah 72.5±8.83 tahun. Pengamatan dalam 1 tahun dimulai sejak injeksi intravitreal ranibizumab pertama diberikan. Berdasarkan jumlah injeksi, keompok dengan lebih dari 3 kali injeksi/tahun menunjukkan peningkatan BCVA yang lebih tinggi dibandingkan dengan kelompok yang mendapatkan 1-3 kali injeksi/tahun (-0.1 LogMAR vs -0.05 LogMAR). Kelompok yang mendapat lebih dari 3 kali injeksi/tahun juga menunjukkan pengurangan CRT yang lebih besar dibandingan dengan kelompok yang mendapat 1-3 kali injeksi/tahun (118.29 ?m vs 107.6 ?m). Berdasarkan metode pemberian, kelompok dengan loading dose menunjukan BCVA -0.1 LogMAR sedangkan pada kelompok yang mendapat dosis sesuai kebutuhan tidak menunjukkan perubahan. Kelompok metode loading dose menunukkan pengurangan CRT yang lebih besar dibandingan kelompok yang mendapat dosis sesuai kebutuhan. Simpulan: injeksi intravitreal ranibizumab dengan lebih dari 3 kali injeksi dan dengan metode loading dose memberikan hasil yang lebih baik dalam tatalaksana ARMD neovaskular.

Abstract

Introduction and Objective : With the improvement in surgical techniques, accurate preoperative intraocular lens (IOL) power calculations in cataract surgery are fundamental to achieving the desired refractive outcomes. To determine IOL power, biometry data are necessary in estimating of the postoperative effective lens position. Currently there are 3 techniques in biometry examination; optical, applanation, and immersion. In Cipto Mangunkusumo Hospital immersion technique was most often used. The aim of this study is to determine the accuracy of IOL calculations in eyes undergoing phacoemulsification cataract surgery with IOL implantation using immersion A-scan ultrasound biometry and evaluating the most influencing factors in biometry. Methods : Review retrospectively medical records of patients having phacoemulsification with in-the- bag IOL implantation measured in ultrasound immersion biometry, determining AL, average K, ACD, lens thickness IOL power calculations with the SRK/T formulas, and postoperative predictability of the device. Demographic characteristic, refractive outcomes, and correlation between variable were evaluated. Results : Five hundred and six patients were evaluated. The accuracy of immersion ultrasound biometry in this study was 82% in range of <0.25D. The estimation refractive outcome had positive correlation with sphrerical equivalent post operatively. ACD was shown to be the most significant variable in predicting the accuracy of biometry. Conclusion : The realibility of physician for doing and interpreting immersion ultrasound biometry has reached the ideal result in this study. A comprehensive learning with continuing education and training is essential for biometry’s operator, to ensure the accuracy of post-operative result in cataract surgery.

Abstract

Objective: To evaluate and compare the efficacy and safety between endoresection and proton beam radiotherapy, as primary treatment in choroidal melanoma.Methods: Articles that were published from 2008 to 2018 were collected from multiple sources including Pubmed, Clinical Key, and Ophthalmology Advance. All study that comply with the inclusion and exclusion criteria were categorized based on level of evidence Oxford Center for Evidence-based Medicine Levels of Evidence. Primary outcome is secondary enucleation. Secondary outcomes are metastasis, recurrence, death, visual outcomes, and complication. Result: Twelve articles were eligible to be reviewed. Mean secondary enucleation, metastasis, and death rate is lower in endoresection group (6.29% vs 12.94%; 8.00% vs 20.85%; 6.86% vs 20.43) while recurrence rate is lower in proton beam group (4.78% vs 6.86%). The most common complication that observed is retinal detachment. Other complications that were reported includes cataract formation, radiation retinopathy, neovascular glaucoma, vitreous hemorrhage, elevated intraocular pressure (IOP), iris neovascularization, and optic neuropathy. Conclusion: Endoresection as primary treatment for choroidal melanoma shows better efficacy compared to proton beam therapy, regarding the ability to preserve the eyeball. The safety between endoresection and proton beam therapy, both therapy shows similar result.

Abstract

Background: Acquiring an accurate intraocular (IOL) power in children undergoing cataract surgery is challenging. Different IOL calculation formulas in children have been previously studied to achieve a precise prediction of the IOL power. Larger errors in IOL formula predictions have shown in several studies on children as future growth of the eye affects the keratometry readings and axial length. Prediction error (PE) and absolute prediction error (APE) can be effective indicators in assessing the accuracy of IOL power calculation formulas. Therefore, this review aims to investigate the accuracy of IOL power calculation formulas in pediatric eyes by measuring PE and/or APE value. Methods: A comprehensive search was conducted from various electronic databases (Pubmed, Clinical Key, and Ophthalmology Advance) using relevant search terms. Included studies were screened using predefined inclusion and exclusion criteria to identify comparative studies comparing the accuracy of IOL calculation formulas in pediatric patients. Result: Ten studies including 964 eyes were identified to compare different IOL formulas: Hoffer Q, SRK/T, SRK II, Holladay 1, and Holladay 2. Smaller prediction error is related to better postoperative refraction. Among included studies, Holladay 2 had the smallest mean prediction errors (PE), while SRK/T formula was the best formula of all included studies in pediatric patients as it had the smallest mean absolute prediction errors (APE). Hoffer Q also had the smallest mean APEs in shorter eyes (axial length <22 mm). Conclusion: This study demonstrates that none of the established formulas found to be more superior than any other formulas in predicting IOL power in children. Biometry examination in patients with less than one year of age tends to show a lower accuracy.

Abstract

Tujuan : Untuk melaporkan aspek klinis, terapi, hasil terapi dan insiden dari endoftalmitis akut pasca operatif katarak di RSUD Dr. Saiful Anwar, Malang.Metode : Penelitian deskriptif analitik retrospektif yang menggunakan rekam medik dengan diagnosis endoftalmitis akut pasca operatif dari Januari 2015 - Desember 2018 di RSUD Dr. Saiful Anwar. Kami mencatat kasus endoftalmitis pasca operasi katarak yang terjadi dalam 6 minggu setelah operasi. Data yang dikumpulkan meliputi demografi, temuan klinis, hasil mikrobiologi, terapi dan komplikasi. Hasil : Didapatkan 31 kasus endoftalmitis akut pasca operatif, dimana 29 kasus merupakan rujukan. Dua puluh dua kasus pasca fakoemulsifikasi, 5 kasus pasca extra capsular cataract extraction dan 4 kasus pasca small incision cataract surgery. Lebih dari 50 % pasien adalah laki-laki (58,06 % ), berusia 61-70 tahun (38,7%) dan terjadi pada minggu pertama pasca operasi (45,2%). Penyakit penyerta meliputi diabetes mellitus (41,9%) dan hipertensi (32,3%).Terapi diberikan sesuai protokol Endophthalmitis Vitrectomy Study, diantaranya injeksi antibiotik intravitreal, vitrektomi, antibiotik sistemik dan topikal, kortikosteroid sistemik dan topikal. Didapatkan perbedaan signifikan antara visus sebelum dan setelah terapi (p=0.038), namun tidak didapatkan perbedaan antar kelompok terapi. Pada 9 kasus terjadi komplikasi, 2 diantaranya dilakukan eviserasi oleh karena perforasi kornea. Kesimpulan : Sebagian besar kasus endoftalmitis akut pasca operatif terjadi setelah fakoemulsifikasi, pada laki-laki, usia 61-70 tahun dan terjadi pada minggu pertama pasca operasi. Organisme terbanyak yang teridentifikasi adalah bakteri gram positif. Didapatkan perbaikan visus yang signifikan dengan pemberian terapi.

Abstract

Pendahuluan : Neuroretinitis merupakan suatu sindrom klinis yang ditandai dengan penurunan visus unilateral akut dengan makulopati eksudatif berupa hard exudates yang tersusun dalam pola bintang di sekitar fovea. Pada artikel ini, penulis mempresentasikan suatu kasus neuroretinitis yang memperlihatkan outcome yang baik setelah pemberian terapi ciprofloxacin. Presentasi kasus : Seorang pasien perempuan, umur 18 tahun, datang dengan keluhan utama berupa penglihatan mata kiri kabur. Visus mata kiri saat presentasi awal yaitu 1/300 dan pada funduskopi hanya didapatkan edema papil nervus optik disertai perdarahan peripapil. Pasien diberikan metilprednisolon intravena secara intravena selama 3 hari namun tidak didapatkan perbaikan gejala klinis. Pada follow up hari keempat, muncul gambaran macular star pada funduskopi yang khas untuk neuroretinitis dan diberikan terapi ciprofloxacin. Setelah pemberian ciprofloxacin selama 2 minggu didapatkan perbaikan visus menjadi 5/60 dengan visus akhir pada follow up bulan ketiga menjadi 6/12. Kesimpulan : Meskipun neuroretinitis bersifat self-limited tapi pada kasus ini pemberian ciprofloxacin mempercepat perbaikan visus dan resolusi dari edema papil nervus optik dan makula. Tiga bulan pasca presentasi awal juga tidak menunjukkan adanya sekuele berupa gangguan penglihatan warna atau penurunan sensitivitas kontras.

Abstract

Introduction: Usher Syndrome (USH) is an autosomal recessive disorder characterized by congenital sensorineural deafness and progressive loss of vision due to retinitis pigmentosa. The aim of this report is to report rare cases of USH by perform diagnostic steps based on clinical examination and family history with expectations that if a similar case is found, an earlier diagnosis and rehabilitation can be done. Methods: Diagnosis is based on history, ophthalmology examination and ear, nose, and ear (ENT) examination. Results: A 23-year-old man came to the Ophthalmology Outpatient Department with complaints of bilateral blurred vision especially at night and visual field constriction starting 12 years ago. The patient experienced gradual vision deterioration followed by appearance of white spot in his left eye. Patient is deaf and mute since birth. Three family members of the paternal line exhibit the same symptoms. The best corrected visual acuity was right eye 6/15 and left eye hand movement. Bilateral cataract was found during eye examination. Posterior segment examination revealed peripheral bone spiculae appearance and waxy pallor optic disc in both eyes. Loss of neurosensory layer, worse in the left eye, was discovered through Optical Coherence Tomography (OCT). Humphrey perimetry of the right eye indicated tunnel vision. Examination by otorhinolaryngologist revealed a sensorineural hearing loss. In this patient, there is no management for the disorders due to the poor prognosis of visual acuity. What can be done to improve the patient's quality of life is supportive therapy. It is important to screen and educate families suffering from the same disorder for early diagnosis & rehabilitation. Conclusion: Clinical examination and family history confirm the diagnosis of USH. Early diagnosis and early rehabilitation are essential to improve patient's quality of life. In this case, no management was carried out for the cataracts because the visual acuity of both eyes did not match the cloudiness of the lens so there are possibility the visual acuity could not improve. Rehabilitations that can be done for the patients are visual and social rehabilitation to improve quality of life.

Abstract

Objective: To present a case of goniotomy procedure and angle imaging on primary congenital glaucoma (PCG) in Indonesian patient.Case presentation: a 6 month old baby presenting with enlarged left eye since birth. The ophthalmological examination found good light and object fixation on both eyes. The eyes were orthophoric. There was no nystagmus and movement restriction. The corneal diameter of the right eye was 10 mm and the left eye was 13 mm. Haab striae (+) on left eye. The intraocular pressure (IOP) was 11 mmHg on right eye and 35 mmHg on left eye. There were no other abnormalities on both eyes. A goniotomy procedure was done on the left eye. The IOP of the left eye on 1 day, 1 week and 3 month post surgery were 6, 18 and 17 mmHg respectively. The cornea of the left eye was clear despite the Haab striae dan bruckner test was good. Optic nerve head examination of the left eye was good and the cup to disk ratio was 0.5.Conclusions: Surgical therapy is usually warranted in the management of PCG. Goniotomy is typically chosen as the initial surgery when the refractive media is clear and direct visualization of the angle is visible. The advantage of goniotomy over other angle surgeries for PCG is spare of the conjunctiva for filtration surgery if required in the future. Our case of goniotomy on PCG in Indonesian patient showed good results and promising prognosis post procedure.

Abstract

Pendahuluan: Refractive surprise merupakan kejadian yang tidak diharapkan pasca operasi katarak. Refractive surprise dapat ditatalaksana baik secara konservatif maupun dengan tindakan operasi berbasis lensa maupun berbasis kornea. Tujuan: Untuk melaporkan hasil akhir sebuah kasus refractive surprise yang ditatalaksana dengan implantasi lensa intraokular piggyback sekunder. Ilustrasi Kasus: Pria berusia 54 tahun datang ke unit katarak dan bedah refraktif Rumah Sakit Mata Cicendo dengan keluhan utama mata kanan buram pasca operasi katarak 4 bulan sebelumnya. Tajam penglihatan dasar mata kanan 2/60 dan mata kiri 0.5. Hasil refraktometer mata kanan didapatkan S -6.00 C-1.00 x 96 dan tajam penglihatan terbaik dengan S-6.00 adalah 0.63. Pasien kemudian didiagnosis dengan myopic surprise mata kanan, pseudofakia dengan kekeruhan kapsul posterior grade I-II mata kanan dan katarak subkapsularis posterior mata kiri. Pasien kemudian ditatalaksana dengan implantasi lensa intraokular piggyback menggunakan Sensar AR40 foldabel three-pieces hidrofobik akrilik. Tiga bulan pasca operasi didapatkan tajam penglihatan terbaik pasien adalah 0.8 tidak dapat dikoreksi. Kesimpulan: Lensa intraokular piggyback sekunder merupakan tatalaksana refractive surprise berbasis lensa yang aman, mudah diprediksi, dan bersifat reversibel.

Abstract

We report an uncommon presentation of Wyburn-Mason syndrome in a pediatric patient. An 11-year- old girl presenting with no light perception in her right eye. Examination revealed an exotropia and a large and tortuous retina vessel in the posterior part of her right eye. However, other examinations, including dermatological and neurological examination, are within normal limits. Magnetic Resonance Angiography (MRA) showed vascular malformation at the basal cistern that extends to the retrobulbar area. Therefore, a thorough examination of the eye and brain, including MRA, is mandatory in every case, even in pediatric with normal neurological examinations.
