Abstract

Introduction : Stevens-Johnson Syndrome (SJS) can cause keratinization of the tarsal conjunctiva, conjunctival scarring and inverted eyelid margin, resulting to cicatricial entropion, causing skin and lashes to brush against the eye. This condition can occur in 27-59% of cases in ocular SJS. The surgical treatment for cicatricial entropion is to repair the position of eyelid margin. One of the procedures that can be used is Sie Boen Lian (SBL) technique. Case Illustration : A 70-year-old woman complained of inversion of the left upper eyelid, which caused foreign body sensation, redness, and lacrimation since 4 months ago. The patient had a history of SJS caused by the use of quinolone antibiotic. Anterior segment examination revealed entropion in the left upper eyelid with palpebral margin keratinizing. The patient was diagnosed with cicatricial entropion post- SJS. SBL surgical technique was chose to manage this condition. Post-operative ophthalmological examination showed a relieved symptoms and an improvement in the position of the eyelid margin. Discussion : The primary goal of therapy in cicatricial entropion is to reduce the occurrence of chronic ocular irritation by reverting the structure of the eyelids to a physiological position. The anterior lamellar shortening was performed with a modified tarsotomy, known as the SBL surgical technique. The post-operative evaluation was implemented to assess the reposition of the eyelid margin. Conclusion : The SBL surgical procedure is effective to manage cicatricial entropion post-Steven-Johnson Syndrome.

Abstract

Introduction : Globe rupture is a vision-threatening emergency. The management of globe rupture must arrange immediate. Case Illustration : A 59-year-old woman presented with painful and sudden vision loss on her left eye after hit by wood an hour prior. From the examination, the palpebra was oedema, and spasm, the conjunctiva was chemosis and bleeding, the sclera was laceration. Visual acuity of the left eye was 1/~. We diagnosed as globe rupture of the left eye. We gave levofloxacin injection, methylprednisolone injection, tranexamic acid injection, prednisolone eyedrop every 3 hours, levofloxacin topical every hour, and oxytetracycline eye ointment once a day in night. After 10 days, clinical manifestation was improved, but the visual acuity. From the examination, we found retinal detachment on her left eye. Patient was referred to the primary health care. Discussion : In globe rupture, infection risk is high and requires immediate systemic empiric antibiotics and surgical management. The conservative treatment was administered since the patient rejected for the globe exploration and scleral suture surgery. The aim treatment for this case is to prevent the infection. We used combination systemic and topical levofloxacin antibiotics and corticosteroid. Systemic tranexamic acid was to control the bleeding. No improvement of visual acuity after 10 days, we suspected because of retinal detachment and need surgery management. Conclusion : Surgical exploration and infection prevention are needed for globe injury. The sooner management, the better prognosis it could be.

Abstract

Introduction : Kissing nevus, also known as kissing mole, is a rare dermatological condition in the world and characterized by the presence of congenital nevi on both the upper and lower eyelids in symmetrical fashion. Additionally, this report highlights the treatment options available for patients with kissing nevus and discusses the importance of regular monitoring. Case Illustration : An 11-year-old female patient came with a complaint of a mole on the upper and lower eyelids since birth. The mole was felt to be getting bigger and starting to bother her. The patient feels insecure with the presence of this mole. The patient underwent nevus excision followed by retroauricular graft placement on the superior and inferior eyelidss. Then a biopsy showed benign nevus. After follow-up, it was found that the graft had grown and there was no necrotic tissue. Discussion : Kissing nevus is a rare skin condition that affects babies and young children. It is characterized by two identical birthmarks that appear on either side of the body, such as on the eyelids. Surgery involves removing the birthmarks surgically. Our patient underwent wide excision and palpebral reconstruction. Conclusion : Kissing nevuse is a unique skin condition marked by the presence of congenital nevi on both the upper and lower eyelids in a symmetrical pattern. It is worth noting that while treatment may be effective, there is a risk of scarring, infection, and other side effects associated with these procedures.

Abstract

Introduction : Penetrating ocular trauma includes open-eye trauma, which leads to the time and a precision of eye- penetrating trauma’s management determine the advanced vision prognosis. In this case a male experience penetration trauma of the right eye due to exposure to the bamboo, delaying and inadequate management resulted in evisceration of the right eye. A 42-year-old male came to ER RSUD Prof. Soekandar mojokerto with bloody right eye and pain from getting a piece of bamboo from two days before entering the hospital. The patient also complained that the right eye is swollen so that it is unable to open the eyes. vision examination revealed LP (-) in the right eye, visual acuity in the left eye was 6/6, soft palpation in the right eye, normal palpation in the left eye, examination of the right anterior segment of the eye revealed that part of the eyeball content had prolapsed and for the left eye within the normal range. During laboratory tests, 15.280 white blood cells were found, which confirmed the infection of the right eye. Case Illustration : Descriptive Study Discussion : The observations on these patients include covered the right eye, antibiotic, steroids, and analgesic by intravenous injection. Based on the condition of the patient's right eye, it is advisable to do evisceration of the right eyeball under general anesthesia. Conclusion : Penetrating ocular trauma resulting in extensive intra ocular tissue damage and poor vision prognosis, considered for evisceration, in order to prevent the development of the sympatetic ophthalmia.

Abstract

Introduction : Congenital nasolacrimal duct obstruction (CNLDO) is a common cause of epiphora in infants; its prevalence rate of 5% to 20%, although only 2–6% will need intervention. The pathogenesis involves an imperforate membrane of the nasolacrimal duct but it may involve bony abnormalities and narrowing of the lacrimal drainage system. Case Illustration : An 11-year-old boy came to the hospital with complaints of a watery and excreted eye since he was born. On the dye disappearance test the result is +4. The patient was decided to have probing procedures. There was positive dye flow in the nasolacrimal duct after probing and irrigation with saline and dye. One week after the probing procedure, dye disappearance test was performed and the result is negative. Patients also no longer complain of watery and sticky eyes. Discussion : The management of CNLDO remains controversial. Probing can be used as a primary treatment for children <36 months of age, where success rate of 78–93% can be achieved. For older children success rate is even less. Studies on older children between 25 and 60 months have shown failure rates as high as 28%. The prevalence of complex obstruction in children between 49 and 60 months was 43% with a success rate of only 33% on probing. Conclusion : Appropriate time for probing the CNLDO is still under debate. The high success rate of probing in patients older than 2 years old confirm that the first intervention could be probing before considering complex intervention.

Abstract

Introduction : Orbital exenterations are performed to treat malignancies, orbital injury or other chronic conditions unresponsive to conservative therapy. Exenteration can result in depression, difficulties in daily activities, and aesthetic problems. Orbital prosthetic is an alternative therapy for aesthetic rehabilitation and protection of the orbital cavity. Case Illustration : A 46-year-old woman came with a chief complain of a black mass on the left lower eyelid ten years ago. Patient was diagnosed with basal cell carcinoma and underwent extirpation of the mass with skin graft reconstruction. On histopathological examination, it was discovered that the incision base was not free of tumor. Patient came two years later with recurrent black mass invaded cornea and sclera, and then underwent exenteration of the left eye (Figure 1). Patient completed 40 cycles of radiotherapy and then used spectacle retained orbital prosthetic (Figure 2 & 3). Discussion : Orbital prosthetics are synthetic replacement after exenteration that can restores orbit, eyelid, eye and periorbita anatomy. Orbital prosthetic retention can be aided by attachment to spectacle, use of magnets, adhesives or osseointegrated implants. Spectacle retained orbital prosthetic is the easiest and the most affordable prosthetic to construct. Conclusion : Orbital defects rehabilitation improves physical and psychological well-being. Due to the complexity of prosthetic fabrication and production cost, retention of the orbital prosthetic with spectacles is the most suitable alternative treatment option in some cases, it is also the easiest prosthetic to construct and inexpensive.

Abstract

Introduction : cicatricial ectropion is a malposition lower eyelid by shortening and/ or scarring of the anterior lamella of eyelid but it can occur in the upper eyelid. Ocular manifestations in cicatricial ectropion occurred epiphora, redness eyes, foreign body sensation, dry eyes due to direct exposure to the outside. In severe case may occur ulceration and loss of vision Case Illustration : A 33 year old male patient was being came to ophthalmologist department at Bali Mandara General Hospital with complaints that the right eye could not close completely, the felt dry eyes, foreign body sensation, and sometimes redness eyes after maxillofacial surgery one month ago before admitted to hospital. The physical examination the patient was normal. The ophthalmological examination visual acuity 6/6 for both eyes, intraocular pressure was normal for both eyes. The movement of the eyeballs was normal. The segment anterior examination was a scar on the lower right eyelid, inferior palpebral retraction, lagophthalmos 4 mm. The segment anterior examination of the left eye was normal. Discussion : The patient was treated to repair the cicatricial ectropion with surgery under general anesthesia with postauricular full thickness skin graft, antibiotic eye ointment, antibiotic oral, and lubricant eye drop. Conclusion : Cicatricial ectropion is a complication that occurs due to trauma or surgery. Reconstructive surgery to repair eyelid function, improve cosmetic appearance and minimize complications of further eye damage such as keratitis to blindness. Knowing the etiology along with the progression of the disease is necessary for early diagnosis and prompt treatment.

Abstract

Introduction : A retained foreign body (FB) can lead to severe complications, the most devastating of which is eye loss. We present a discrepancy of imaging that resulted in a dilemma in the approach management of a mixed global-adnexal FB case. Case Illustration : A 31-year-old man came to the emergency room with blurry vision for 1.5 hours after a nail hit his right eye (RE). The visual acuity of RE is light perception. Ophthalmological examination revealed a central Y-shaped full-thickness corneal laceration with iris and vitreous prolapse. The lens appeared cloudy, and the posterior segment was difficult to assess. Imaging showed a foreign metal object asa nail in the oculi to intraconal without penetration to the orbital wall. He was planned to undergo FB extraction with corneal laceration repair and aspiration of lens mass irrigation. However, the nail was hard to find intraoperatively because the vitreous and choroid kept coming out. Hence evisceration with DFG was performed. The nail was found more posteriorly and attached to the bone. Discussion : While primary repair is the standard practice for open globe injury, managing retained intraocular metallic foreign bodies is controversial. Imaging such as CT scans can help determine the location of FB and approach management. In this case, imaging showed the FB was in the iris plane and not penetrating the orbital wall, so primary repair and FB extraction were planned. Conclusion : The imaging may not always be accurate in determining the exact location of FB, and the surgical approach may need to be adjusted accordingly.

Abstract

Introduction : A corneal ulcer is an ophthalmological emergency that can threaten vision. It is characterized by inflammation or infection involving the corneal epithelium and stroma. Prolonged use of contact lenses is more likely to cause corneal ulcers. The purpose of this paper is to report on the management of corneal ulcers. Case Illustration : A 30-year-old female complained of blurred vision, redness, pain, swelling, and sensitivity to light in both eyes for the last two days. She reported falling asleep in her contact lenses two nights before the visit. The patient had compound myopic astigmatism before and her visual acuity was decreased to 3/60 in the right eye and 5/60 in the left eye, examinations showed palpebral edema and conjunctival hyperemia in both eyes. The use of fluorescein in both eyes highlighted the area of the defect. The patient was diagnosed with corneal ulcers. Treatments were Moxifloxacin Eye Drop every hour and Ibuprofen 2x400 mg by intravenous infusion. On the last visit, the corneas were clear with visual acuity 6/15 in both eyes. Discussion : According to the study by Kishor et al., ophthalmic eye drop is the most effective treatment and fourth-generation fluoroquinolones represent the most frequently used strategies in Corneal Ulcer.Conclusion : Antibiotics and analgesics are effective for the management of corneal ulcers in this patient. Education regarding contact lenses is important to prevent the occurrence of corneal ulcers.

Abstract

Introduction & Objectives : The prevalence of myopia has approximately doubled in the past three decades, with rates of 70–87% among populations of school children and young adults in Asia. Approaches to treating myopia include spectacles and Rigid Gas Permeable (RGP) contact lenses. RGP has become a new "preference" because it is more comfortable and provides a more aesthetic look. This study aims to determine which method is better for correcting visual acuity in myopia patients. Methods : This cross-sectional study analyzed the outcomes and exposures measured at the same time. The sample included myopia patients (based on spherical equivalent) who underwent RGP contact lens fitting in the refraction outpatient clinic of M Djamil Hospital from January 2021 to December 2022. The samples were selected based on inclusion and exclusion criteria. The collected data included gender, age, and best-corrected visual acuity using spectacles and RGP. Results : Forty-three patients were eligible as the sample in this study. Most of the subjects were women (62.8%), and the mean age was 23.58 years old. The median LogMAR for the RGP group was lower than the spectacles group (0.00:0.10 on both eyes). Statistical analyses found a significant visual acuity improvement in the RGP group compared to the spectacles group in both the right and left eyes (p = 0.014 and p = 0.005, respectively (p < 0.05)). Conclusion : The use of RGP provides significantly better visual acuity in correcting myopia than spectacles. Myopia is more frequently found in young women.
