Jurnal ARSI (Administrasi Rumah Sakit Indonesia) was initiated by the Center for Health Administration and Policy Studies (CHAMPS) Faculty of Public Health, University of Indonesia (FKM UI) and is currently managed by the Department of Health Administration and Policy, Faculty of Public Health, University Indonesia . Jurnal ARSI (Administrasi Rumah Sakit Indonesia) was published with the Indonesian Hospital Association (PERSI) and the FKM UI Hospital Management Association (IKAMARS). Jurnal ARSI (Administrasi Rumah Sakit Indonesia) is a peer-reviewed journal that focuses on service administration and management in hospitals in Indonesia. The articles or scientific manuscripts published in the Jurnal ARSI (Administrasi Rumah Sakit Indonesia) include original research, case studies, and reviews supporting corporate governance, clinical governance, or both (bridging). Abstract

The main challenge of the clinical laboratory unit in hospitals nowdays is making expenditures efficiency, especially human resources expenditures. On the other hand, patients and doctors normally request faster and more accurate laboratory checking results. The purpose of this study is to get proper number of medical laboratory technology experts needed based on workload in Santa Maria Pekanbaru Hospital Clinical Laboratory Unit. Research method applied is operational research with quantitative and qualitative analysis. Workload analysis was done toward the whole medical laboratory technology experts, which were 15 person, with the combination method of work sampling and daily log. The calculation of expert need was done with 3 methods: Ilyas, WISN, and the Full Time Equivalent Method. The study result is there are 18 medical laboratory technology experts needed according to Ilyas Method, 21 experts according to WISN Method, and 17 experts according to Full Time Equivalent Method. It is suggested to Santa Maria Pekanbaru Hospital to add 3 more medical laboratory technology experts.

Abstract

Febrile neutropenia may have a high morbidity and mortality impact for the patient. The death rate of febrile neutropenia in 2002 was 38.8%, while in 2009 was 27.3%. The difference in mortality rates could caused by several factors such as availability of the Neutropenic Fever Management Guidelines in 2006, in addition to infrastructure, human resources and equipment. This study aims to determine the effect of the availability of the guideline and other factors to cancer mortality rate in the compromised immune isolation room of Dharmais Hospital. This study is a cross-sectional retrospective study which investigate mortality rates and compare with adherence to febrile neutropenia guidelines for the period 2008-2012. Data were taken from the patient's medical record file, then analyzed using univariate and bivariate analysis. The mortality rate in the period 2008-2012 was 20,7%. The effect of age, sex, and degree of risk factor on mortality was not significantly different (p=0,409, p=0,404, and p=0,324). The proportion of deaths was higher in patients borne by third parties (26.8%) than in the case of personal (10%) although not statistically significant (p=0,065). Of the three types of adherence, only one had a significant effect of adherence to treatment flow (p=0,033).

Abstract

A quick and precise diagnosis is essential in hospital services, especially in the emergency department. In condition with high volume patients, perfomance of laboratory usually decreases. Studies which identify directly the delay of laboratory examinations in emergency department in relation to length of stay in wards have not been conducted. We did correlative study using secondary data obtained from medical records. Inclusion criteria was child, age range from 29 days to 12 years old who needed supporting laboratory examinations, directly admitted to wards or PICU after getting treatments in emergency department. Exclusion criteria was child with growth and developmental problem, malnutrition, immuncompromised condition, incomplete medical records data, and child died in the emergency department. Average patient in 2016 was 3,6 yeras old and in 2017 was 2 yeras old, mostly boys. Mostly with chief complaints of infections Analysis in 2017 showed correlation coefficient of 0.466 which means there was strong association between laboratory turnaround time and length of stay and statistically significant (p 0.03). multivariate analysis shown there was strong correlation between laboratory turnaround time and length of stay and if the predictive value increased by one unit, the length of stay would increase 6,5%.

Abstract

This study aimed to evaluate the role of Jaminan Kesehatan Nasional (JKN) in improving the quality of cochlear implant services using Strength, Weakness, Opportunity, and Threat (SWOT) analysis. The study identified the internal factors of the cochlear implant center in Cipto Mangunkusumo Hospital as a consideration for developing hospital policies and the external factors to determine strategies for future development. The role of JKN was highlighted on the SWOT analysis, showing that the cochlear implant center was strategically located, thus increased the quality and the competitive factor compared to other hospitals. Strength-Opportunity strategy was recommended in improving service quality and identifying the possibility of JKN and other grants to cover the cost and improve the services, research, and education. The SWOT of the Cochlear Implant Center was identified as: 1. Good internal condition (average score of the strengths is superior to the weaknesses), 2. Good external condition (average score of the opportunities is higher than the threats), 3. Using strengths to maximize the opportunities and alternative strategy to be an aggressive strategy. 4. The role of JKN presented in the SWOT quadrants would give JKN a potency and important role in improving the treatment of severe hearing impairment in Indonesia.

Abstract

Hospitals as plenary health service providers have an obligation to maintain the quality of services provided. The nurse is one of the spearheads of service and is the largest part of the hospital's resources. The quality of nursing services with the main goal is to maintain patient safety by controlling the turnover rate. During 2015 to 2017 there were 15.4%, 14.3%, and 18.9% turnover rates for nurses at awal bros hospital batam. The method of research are quantitative and qualitative approach to determine factors related to nurse turnover. Quantitative research for all nurses who had left was 79 people. Qualitative research on nurses in the inpatient and critical care section with informants 7 people.Quantitative results with univariate statistical show nurses who had been turnover were 77.2% women and 22.8% men; 82.3% aged up to 30 years and 17.7% aged over 30 years; diploma level III 89.9 % and strata I 10.1%; married 55.7% and not married 44.3%; 62% of recruits came from outside Batam and 38% came from Batam; and working period up to 5 years 81% and working period above 5 years 19%. The bivariate statistical test showed that the factors associated with turnover were marital status (Asymp. Sig 0.003) and the area of recruitment (Asymp. Sig 0.000). Qualitative analysis with in depth interviews and focus group discussions found non-physical environmental factors, especially leadership problems lead to turnover. The turnover rate in Awal Bros Hospital Batam is still high with factors of marital status and area of recruitment, non-physical working environment that are associated with turnover. This can be taken into consideration as the Awal Bros Hospital Batam as a factor to be able to control nurse turnover.

Abstract

The writing of a diagnosis on medical resume is the responsibility of the physician in charge of the patient (DPJP), for the benefit of BPJS claims. This relates to doctors' compliance in the diagnosis of medical resumes. Knowledge, attitudes, and motivations affect the physician's neck in writing the diagnosis on medical resume. This study aims to determine the influence of knowledge, attitudes, and motivation either partially or simultaneously to the doctor's compliance in the writing of the diagnosis on medical resume at Zahirah Hospital in 2018. This research is quantitative analytic, with cross sectional and prospective approach. The results showed that knowledge had no significant effect on the adherence of the diagnosis on the patient's medical resume (p value 0.091; R2 0.113). However, attitudes and motivations positively and positively affect the adherence of diagnosis to the patient's medical records (p value 0,000 and 0,000; R2 0,562 and 0.574). Knowledge, attitudes and motivations jointly affect the compliance (p value 0,000; R2 0.679). Zahirah Hospital should continue to support the work environment in the hospital so that doctors' compliance in writing disease diagnosis on the patient's medical resume increases. Doctors at Zahirah Hospital should be able to write medical diagnoses on medical resumes on time.

Abstract

Medication error is the incident of a drug provision that can inflicting injury to the patient with should be able to prevent as long as in control of a trained health professional, and of patients itself. From the 10 fact from World Health Organization on patient safety for the developing country in 2017 mentioned that 1 of 10 patients treated at hospital runs the risk against an onset of medical error and drug mistakes that may cause adverse effect. From the data of patient safety incident in hospital X performed from 2015 until 2016 there were 30 cases of patient safety incidents there were 12 cases of them was the case of error in administering medicine in inpatient unit. It is not conforming to standard of which are regulated by the Ministry of Health of Indonesia in 2008 which states that the incident at the hospital supposed to be zero accident. A design of this research is the qualitative study that aimed to analyze factors affecting patient safety in the process of administering medication by nurses in the X hospital for inpatient unit. The result shows that factors affecting the application of patient safety in administering medication are the lack of supervision of leaders, the lack of human resources, nursing turnover, absence of standard procedure of 7 right principle in administering medication, absence of training program for nurses, and also the lack of socialization
