Abstract

Background: The main challenge of the health financing mechanism in Indonesia is the allocation of health spending which is still dominated by the private sector, whereas the largest proportion comes from out of pocket payments. The system are a significant barrier in accessing health services. Many individuals with chronis diseases postpone the search for medical services because of high health care cost. The consequences of the delay is the loss of opportunities to overcome chronic illness. This research was aimed to analyze the relationship between health financing mechanism and mortality rate (GDR and NDR) in Santa Maria Pekanbaru Hospital. Methods: This cross-sectional study involves GDR and NDR of patients using out of pocket payment and those using health insurance in the year between 2014 - 2017. Data analysis was performed by independent samples t-Test (significance level p < 0,05).Conclusion: The health service outcome in patients with out of pocket payment is lower than health insurance patients. The out of pocket payment financing mechanism is one of the major problems in the transition to Universal Health Covered. It is recommended that our government must increase the health budget which is at least in accordance with the rule of law and improve the allocation of public sector health funds at least 2/3 of the total health budget to reduce the proportion of out of pocket to total health expenditure. Health care providers should do efficiency in all areas to reduce the cost of health services. Patient who do not have health insurance will have to rearrenge the allocation of their household expense to pay premium insurance and implement the healthy life habits.

Abstract

Background: Breastmilk provides nutrition that newborn and baby need during early stage of their lives. Breastmilk production can be increased by giving the mother oxytocin or prolactin enhancement that called galactogogue during breastfeeding. Giving galactogogue to mother bring side effect such as nausea, vomit, and insomnia. The example of galactogogue are domperidone and metoclopramide. Domperidone and metoclopramide not only useful to increase the breastmilk production as galactogogue but also can be used as antiemetic to reduce the effect in the galactogogue. This study aims to examine the effect of antiemetic for galactogogue during breastfeeding. Methods: This study was a systematic review that used PRISMA guidelines. Data obtained from Electronic databases ProQuest and Google Scholar that published between 2015 and 2018. By using keywords such as effect, antiemetic, galactogogue to find the relevant journal. Results: The search found 58 studies, of which 7 studies were relevant to analyze in this study. Majority of these studies used Randomized Control Trial (RCT) and 1 study used case study. Analysis of the studies found out that domperidone and metoclopramide can be used as antiemetic for galactogogue. Domperidone was not enter the blood circulation and did not interfere the breastmilk production

Abstract

Background: The progress of the hospital industry now requires effective management to improve the quality and quality of service appropriate to the standard goals. Preparation of this business strategic plan should involve decision-maker in hospitals and existing organizational resources efficiently and effectively by taking into account current conditions. This study aims to provide a comprehensive review of the process of preparing a hospital business strategic plan so that an alternative strategy can be obtained for the next 5 years. Methods: This study was used operational research, qualitative method with in-depth interviews and hospital performance reports analysis. External and internal environment factors included finance, customer, demographics, geography, policy, marketing, information system, human resources, physical facility and organization were used as evaluated variables. SWOT analysis and Balanced Scorecard framework were used to identify critical success management factors. The Consensus Decision Making Group (CDMG) matrix were used to determined factor evaluation matrix as a final score. Results: Based on an analysis of external and internal environmental factors, alternative strategies that are suitable for current hospital conditions are market penetration and service product development. In order to realize this strategy there are several things that must be of particular concern, namely the limited resources that are available which include the availability of human resources and financial support and there is no available integrated hospital information system. Conclusion: The business strategy plan recommended for developing RSD Kolonel Abundjani Bangko?s services for the next 5 years is market penetration and service product development. In order to realize this strategic plan requires a strong commitment from all human resources in the hospital and related stakeholders.

Abstract

Background: Examines properly implementation of Electronic Medical Records (EMR) which associated with workflows, policies and health services in enhancing patient safety. The electronic medical records have benefit to the patients due to the efficiency in the healthcare process. Therefore, the aim of this study was to examine the literatures on patients safety reporting based on electronic system review and assess the implementation. Methods: Systematic review with Prisma at the literature published between 2010 ? 2019 using three large database: Proquest, Oxford Academic Journals, and Scopus. The eligible articles in this review should show an effect of patient? safety, and product quality in hospital in correlation on using EMR. It must include some effect on medication error, prescribing error, error in the use of EMR that potentially endanger patient safety. Results:The most important function of EMR implementation is to improve patient safety in hospital and to reduce the cost. EMR reduce excess cost of Hospital Acquired Condition (HAC) by 16%, reduce death due to HAC by 34%. Doctor and nurses believe that the quality of patient data would be better by using EMR as their daily routines. Conclusion: EMR can improve patient safety due to some skills in technology. Thus, it would not harm to the patients safety. However, the implementation of EMR requires the human skill resources in using technologies, computer and programs.

Abstract

The implementation of National Health Insurance in 2014 is expected to increase access in health services while reducing the economic burden due to illness. This study aims to see how the behavior of people in choosing treatment when experiencing health problems, whether they tend to select self-medication or outpatient treatment. Quantitative analysis was carried out on variables previously formed based on the KOR Data questionnaire found in SUSENAS 2017. The results of the study showed that more respondents chose to do self-medication than those who accessed formal services with outpatient care. Multivariate tests show all variables except secondary education significantly influence individual decisions to self-medication.

Abstract

Abstrak Salah satu beban penyakit terbesar bagi sistem kesehatan negara-negara di dunia adalah kanker serviks. Penting untuk melakukan evaluasi ekonomi dari program vaksinasi dikarenakan terbatasnya sumber daya. Banyak faktor tantangan di Low-Middle Income Countries (LMIC) yang mempengaruhi kinerja dan interpretasi evaluasi ekonomi Program Vaksinasi HPV. Studi ini bertujuan untuk menggambarkan metode dan hasil dari studi Evaluasi Ekonomi Program Vaksinasi HPV yang diterbitkan serta mengidentifikasi tantangan dan hambatan untuk menerapkannya di LMICs. Kajian sistematik dari studi yang diterbitkan dari 2010-2020 dalam 9 database online mengenai Evaluasi Ekonomi Program Vaksinasi HPV di LMIC. Sembilan penelitian tersebut menunjukan Cost-Effectiveness Analysis vaksin HPV adalah yang paling umum digunakan, dengan merekomendasikan penggunaan rasio ICER per DALY dibandingkan dengan Produk Domestik Bruto (PDB), vaksin HPV bivalen, dengan target populasi anak berusia 9-13 tahun, dengan harga GAVI sekitar US$ 4,50. Penting untuk memahami bahwa efektivitas biaya berkaitan erat dengan anggaran yang tersedia untuk memastikan keberlanjutan program pencegahan kanker serviks. Program Vaksinasi HPV di LMIC hemat biaya; asalkan menggunakan penganggaran yang realistis dan parameter cakupan program yang layak digunakan. Abstract There are still many challenging factors in Low Middle Income Countries (LMIC) that influence the performance and interpretation of model-based economic appraisals of vaccines, especially HPV Vaccine. This study was to summarize the methods and results from publishes Economic Evaluation of HPV Vaccination Programs studies and identify their robustness and limitations for implementation in LMICs. Systematic review from studies published 2010-2020 in 9 online databases discussing the Economic Evaluation of HPV Vaccination in LMICs were searched. From nine studies in LMICs, assessment of the cost-effectiveness of HPV vaccines was the most common, recommended the use of ICER per DALY compared with the country’s Gross Domestic Product (GDP), bivalent HPV vaccine, 9 to 13-year-olds, at a GAVI negotiated price of US$4.50. The outcome is important for policy makers to understand the cost effectiveness should have some relation to the budget available for allocation to ensure sustainability of cervical cancer preventing program. The HPV Vaccination Program in LMICs is cost-effective; provided realistic budgeting is employed and feasible program coverage parameters are used. Abstract

Managing the hospital efficiency with still control the hospital quality and cost is a key to survive in NationalHealth Insurance Era. The objective of this study was to determine the impact of the implementation of theNational Health Insurance Policy (NHI) on the efficiency level of Hospital in South Sulawesi Province, Indonesia.The level of efficiency was analyzed using the Data Envelopment Analysis method. The study population was allBLUD hospitals in South Sulawesi Province. The Samples of research were 25 hospitals. Data from years 2014 to2017 are used to analyze. The results showed, in average the hospitals achive the efficiency level only in year 2016.In years 2014, 2015 and 2017 the hospitals were not efficient. The changing in the healthcare system, especially onpayment system to hospitals in JKN era, have had a negative impact on the level of hospital efficiency because theunadequate INA CBGs Tariff and hospital claims payment disrupted. Therefore, the government needs to overcomethose problems.

Abstract

Self-Medication practices are considered to be a quick solution in treatment when a person can not access the formal health services, but on the other hand, this behavior can cast up the economic burden and health losses if not properly monitored. Meanwhile, smoking is seen as a negative behavior, which can also cause a double burden, both in terms of increased expenditure also has a negative effect on health. This study aims to describe self-medication behavior among smokers in Indonesia and the determinants that influence it. This study is a quantitative study using secondary data from the Indonesia National Socio-Economic Survey (SUSENAS) in 2017. The observation unit is individuals, and the sample criteria are smokers with the probability of doing self-medication due to health problems. The number of analyzed samples was 53.317 respondents. Descriptive analysis is used to see the prevalence of self-medication among smokers. Logistic regression is used to see the determinants that influence a smoker's decision to make self-medication. The results of the follow-up analysis showed that most smokers did Swamedikasi (75.99%). The level of education (p=0,005) and ownership of health (p=0,000) insurance has a significant effect on self-medicated behavior. Through this research, it is expected to be material for learning and advocating about behavior in seeking treatment for smokers.

Abstract

The upfront payment policy in the hospital caused controversy and rejection from the society. To anticipate this, some hospitals apply no upfront payment policy. However the policy causes an increase in bad debts. Seeing this dilemma, the authors will conduct research to analyze the different of hospital financial performance before and after no upfront payment policy. The research method used is comparative descriptive with analyzed by paired sample t-test. The result was no upfront payment policy has a positive impact on hospital financial performance. The results of this study were expected to be an input for hospital management and government to revise and refine on the upfront payment policy.
