THE IMPLEMENTATION OF LEAN STRATEGY TO ALLEVIATE OVERCROWDING IN EMERGENCY ROOM: NARRATIVE REVIEW

Background: Emergency Room (ER) of hospital worldwide undergoes significantly challenging problems; overcrowding patient leads to block the access, causes discomfort and increases demands to be served by medical personnel immediately. To overcome these problems, the ER needs to implement lean strategies. This study presents a narrative review aimed to identify whether there is a positive impact of the lean implementations in overcoming the overcrowded ER or not. Methods: This narrative review explores the current literature from an online database and highlights the lean strategies adopted by several ER to reduce overcrowding and delaying and to streamline patient flow. Literatures without data on waiting time, length of stay (LOS), and untreated patients leaving the ER, will be excluded from this review. The types of intervention were grouped into team triage, streaming, fast track, point-of-care test (POCT), and nurse-requested x-ray. Results: The evidence of lean intervention is beneficial in reducing patientsâ waiting time and length of stay (LOS). Fast track reduces the length of waiting time, LOS, and the number of untreated patients by physician. Team triage consisting of appointed doctors and nurses also has a positive impact on shortening the waiting time. There is not enough evidence to suggest that streaming, POCT early in arrival, and x-rays by nurses can reduce waiting times and LOS.Conclusions: The implementations of lean strategy have a positive impact in shortening the waiting time and LOS, and help speed up the patientsâ flow and access to other supporting units.Â

Background: A turnover occurrence in the hospital was a constant problem. The cause of turnover was largely inclined to the level of job satisfaction, hostile work environment, excessive workload, resulting in a lack of motivation to stay. Methods: This study was conducted in a narrative review by looking at the factors causing turnover at various hospitals. The review was done by searching for articles and journals related to factors that lead to nurse turnover in the hospital. The referral journals are published in the accredited journal publications. The literature investigation was conducted in October-December 2017 through Universitas Indonesia online library and Google Scholar. Journals were selected based on predefined criteria and carried out a critical appraisal process. Results: The results of the journal investigation showed that some factors that support the nurse turnover in the hospital were the poor work environment, job dissatisfaction, excessive workload, the poor relationship between nurse and manager, lack of self-appreciation, burnout and low social support. Conclusions: The suggestion that can be given is to improve the work environment that is emphasized through policies to maintain the nurse in the workplace.Â

Background: Among others, healthcare professional is one of the main drivers in the hospital. Medical doctor plays an important role in patient-centered care system in the hospital, leading health services to patients, cooperating with other healthcare professionals. Medical doctor shortage and high turnover in the hospital has become a problem in many countries. High turnover of medical doctor can lead to an inadequate supply of health services, thereby compromising people?s access to health care and endangering the quality of patient care and safety. The objective of this study is to get an overview of the factors that affect medical doctor turnover in the hospital.Methods: A systematic review of articles published between 2014 and 2019 that contain information from all relevant published articles were extracted and analyzed. Results: The literature search and screening process resulted in 9 (nine) studies. From the selected studies, overtime, job satisfaction, and work stress are among the things that affect the turnover of medical doctor in the hospital. Conclusions: Consequences of high medical doctor turnover in the hospital are including financial loss, low patient satisfaction, and poor organizational practices. Factors affecting medical doctor turnover in the hospital are long work hours, job satisfaction, work stress, workplace condition, and hospital management.

In the era of globalization, the hospital view is not only to carry out its social mission to improve public health but also to be profit-oriented. Therefore, marketing has an important role in the hospital industry. A good marketing strategy will be able to create welfare for patients, but also for the hospital's financial performance. The branding strategy in hospital marketing is a popular strategy at this time and is one of the strategies that are closely related to creating brand equity of the hospital. This study aims to further examine how branding strategy is in hospital marketing and its assessment of consumer loyalty and engagement. This study is a literature review of articles obtained from the ScienceDirect, ProQuest, and Scopus databases. a total of 7 articles were screened from 723 articles and reviewed. It was found that the strategy can create brand equity for the hospital organization as well as for a good relationship with health and patient loyalty.
