Abstract

This study analyzes the formulation and implementation process of Depok Mayor Regulation No. 61 of 2023 on Tuberculosis (TB) Control using Friedman's legal system theory (1975) and the eight General Principles of Good Governance (AUPB). The regulation provides a comprehensive framework, encompassing promotive, preventive, curative, and rehabilitative aspects to support TB control. The legal structure includes the establishment of a cross-sectoral Tuberculosis Control Acceleration Team, involving government agencies and community organizations. Findings reveal that while the regulation has fulfilled several AUPB principles, such as legal certainty, utility, and public interest, there are areas requiring improvement. These include strengthening collaboration culture, clearly defining responsibilities among stakeholders, and enhancing transparency. Furthermore, the analysis highlights the need for ongoing evaluation and adjustments to address challenges in implementing the policy effectively. This study concludes that while Depok Mayor Regulation No. 61 of 2023 provides a robust foundation for TB control, its implementation demands further refinements to ensure alignment with good governance principles and the sustainable achievement of tuberculosis elimination goals.

Abstract

Integrated primary healthcare (ILP) is part of the transformation of primary healthcare aimed at improving access to quality healthcare. This study aims to analyze the readiness of puskesmas and their networks to implement ILP. This research used a qualitative approach, focusing on a case study of a puskesmas in an urban setting, specifically in the operational area of Puskesmas Pamulang in South Tangerang. Data collection methods include in-depth interviews, observations, and document reviews. This research adopts the conceptual framework of PHC monitoring based on WHO and Weiner's theory of organizational change readiness. The research findings indicate that ILP readiness at puskesmas is not fully prepared for cluster-based services, networking approaches, and digitalization strengthening. Resource availability, including human resources, infrastructure, and facilities, is inadequate, particularly at the puskesmas network level. Readiness for digital technology is constrained and the lack of funding hampers ILP preparation. Individual commitment, demonstrated through understanding information and positive assessments of ILP, and organizational commitment evidenced by governance support and allocated funding, are factors facilitating ILP implementation. The study recommends prompt funding allocation for ILP preparation, resource fulfilment at puskesmas and posyandu, and the need for cooperation and commitment from all stakeholders in ILP implementation.

Abstract

Gestational Diabetes Mellitus (GDM) is a health condition that increases during pregnancy and can lead to serious complications for both the mother and the fetus. Self-management, encompassing understanding, implementing, and maintaining health behaviors, is crucial in managing GDM. The Health Belief Model (HBM) is a theoretical framework that can serve as the basis for interventions to enhance self-efficacy in pregnant women with GDM. This study analyzes the relationship between self-efficacy and self-management in controlling gestational diabetes in pregnant women. It is a literature review using a literature search method. Literature searches were conducted through three databases: PubMed, Scopus, and Google Scholar, covering articles published between 2014 and 2024. The search utilized MeSH keywords such as "Gestational Diabetes Mellitus," "Self-Efficacy," and "Intervention Self-Management." The search yielded seven English articles that met the inclusion criteria. Analysis was conducted narratively, referring to the identified literature findings. Literature review findings indicate a positive correlation between the implementation of Health Belief Model-based self-management and the improvement of self-efficacy in pregnant women with gestational diabetes. Factors such as risk understanding, perception of benefits, and understanding of barriers are crucial in influencing self-efficacy levels. Applying the HBM contributes to enhancing the ability of pregnant women to engage in self-management. This intervention effectively boosts the confidence of pregnant women in managing dietary patterns, physical activity, and blood glucose monitoring, assisting them in coping with gestational diabetes.

Abstract

This paper delves into the widespread implementation of Electronic Medical Records (EMR) within healthcare facilities across Indonesia. It addresses the challenges encountered by clinic owners and healthcare providers in transitioning to these digital systems. The study is supported by empirical data collected through a mixed-method approach, combining internal data from Klinik Pintar with qualitative validation from key stakeholders responsible for clinic operations. The research focuses on two case studies: LAPAS II Purwokerto Clinic and PMI Purbalingga Clinic, where the adoption of EMR is analyzed in terms of efficiency, compliance with regulatory standards set by the Ministry of Communication and Informatics (Kominfo) and the Ministry of Health (Kemenkes), and its impact on daily healthcare workflows. Through in-depth interviews and observational studies, the perspectives of clinic administrators, healthcare professionals, and IT personnel are explored to understand the benefits and constraints of EMR implementation in real-world settings. The findings highlight key factors influencing successful EMR adoption, including user training, infrastructure readiness, and regulatory compliance. Additionally, barriers such as resistance to change, technical limitations, and integration challenges with existing healthcare systems are discussed. This study provides valuable insights for policymakers, healthcare providers, and technology developers in enhancing the effectiveness of EMR implementation in Indonesian healthcare facilities.
