Abstract

Background: Pain, stiffness, and decreased mobility are the main symptoms of osteoarthritis (OA), a degenerative joint disease that mostly affects the knee. The increasing prevalence of OA worldwide highlights the necessity of efficient non-pharmacological management techniques. Purpose: To evaluate the effectiveness of isometric knee workouts in enhancing functional results and reducing pain in individuals with osteoarthritis. Method: A comprehensive search was conducted using Google Scholar, PubMed, ScienceDirect, and ProQuest to find publications published between 2014 and 2024. The inclusion criteria focused on studies that used isometric exercises as an intervention for knee OA. The Critical Appraisal Skills Program of the Joanna Briggs Institute (JBI) was used to assess the quality of the selected publications. Data were analyzed thematically to compile conclusions on pain alleviation and functional enhancement. Results: Eleven studies totaling 876 participants were included in the review. The treatments, which lasted anywhere from four to twelve weeks, continuously showed notable improvements in joint function and pain reduction. Results were improved by using isometric exercises both on their own and in conjunction with treatments like ultrasound and biofeedback. The Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), the Visual Analog Scale (VAS), and the Numerical Rating Scale (NRS) were frequently used to measure pain. Despite certain constraints, such as small sample sizes and brief follow-up periods, the quality assessment showed that the majority of studies had excellent methodological rigor. Conclusion: For individuals with knee OA, isometric knee exercises are useful in lowering discomfort and enhancing functional results. The effectiveness of these exercises can be further increased by combining them with biofeedback or other therapeutic methods. These results provide a useful, low-risk strategy for enhancing patient quality of life and support the incorporation of isometric exercises into non-pharmacological therapy options for knee OA.

Abstract

Background: Type 2 diabetes mellitus (DM) is a chronic metabolic disease whose prevalence is increasing globally, including in Indonesia. Risk factors such as unhealthy diet, low physical activity, smoking, low fiber consumption, and pesticide exposure contribute significantly to the incidence of type 2 DM. Purpose: To analyze the association between pesticide exposure, smoking habit, physical activity, consumption of sweet foods, and fiber consumption with the incidence of type 2 DM in the early adult population. Method: A cross-sectional design was conducted to 110 early adult respondents aged 24-40 years. Data were collected through a structured questionnaire and analyzed using the chi-square test with a significance level of 5%. Odds Ratio (OR) was used to assess the odds of risk. Results: Pesticide exposure (p=0.009; OR=3.4), low physical activity (p=0.004; OR=3.5), excessive consumption of high-sugar foods (p=0.009; OR=3.1), and low fiber consumption (p=0.004; OR=3.7) were significantly associated with the incidence of type 2 DM. However, smoking habit showed no significant association (p=0.453). Conclusion: Factors such as pesticide exposure, low physical activity, excessive consumption of sugary foods, and low fiber consumption are significant predictors of type 2 DM. Healthy lifestyle education and control of environmental risk factors are recommended to reduce the prevalence of type 2 DM.

Abstract

Background: Several iterations of Early Warning Systems used in the birthing domain for the purpose of identifying and addressing early clinical hazards aim to prevent morbidity and mortality. The potential of early warning systems lies in their ability to improve the quality of health care and reduce the likelihood of maternal death. Purpose: To validate the effectiveness, accuracy and sensitivity of implementing different obstetric early warning systems and physiological triggers to predict morbidity in postnatal wards in high-risk women. Method: A systematic review was conducted in accordance with PRISMA guidelines to identify peer-reviewed studies on obstetric Early Warning Systems published between 2016 and 2023. The search spanned multiple databases, including Scopus, ScienceDirect, Research Rabbit, Google Scholar, and PubMed, and was supplemented by a review of relevant reference lists and professional organization websites. The review focused on studies evaluating the effectiveness, accuracy, and sensitivity of MEOWS, MEWC, OEWS, SMO, and SMS in predicting clinical deterioration and improving outcomes in hospitalized maternity patients. Studies involving pediatric or non-obstetric populations and qualitative designs were excluded. Due to heterogeneous findings, a narrative synthesis was applied. Results: Out of 1,152 identified articles, 20 met the inclusion criteria. These studies examined the predictive accuracy and sensitivity of midwifery Early Warning Systems (EWS) in relation to maternal morbidity and mortality. Eight studies focused on the systems’ effectiveness in improving clinical outcomes, while one addressed both aspects. Eight versions of the midwifery EWS were analyzed, most of which incorporated six core clinical parameters: pulse, respiratory rate, temperature, blood pressure, and level of consciousness. Eleven studies specifically evaluated the accuracy of these systems in enhancing clinical outcomes. Additionally, researchers contributed to the development of midwifery-specific EWS tools. Conclusion: The maternal early warning system has shown effectiveness in predicting severe morbidity among obstetric patients and mortality in critically ill cases. These systems hold promise for enhancing service quality, preventing the progression of maternal complications, and improving overall health outcomes. However, current evidence on their impact in reducing maternal mortality across all populations remains limited. Additionally, most versions rely on clinical parameters routinely collected from constrained resources, which may contribute to less-than-optimal outcomes. Suggestion: A validated and standardized national obstetric early warning tool will enable the consistency and uniform approach needed in the assessment and management of potentially deteriorating conditions of pregnant women and compliance of health workers in implementing obstetric early warning tools according to standards.

Abstract

Background: Controlling blood glucose needs to be done in patients with Diabetes mellitus so that the level is close to the normal threshold. Complications such as retinopathy, neuropathy, and nephropathy can occur as a result of uncontrolled blood glucose levels. To keep their blood glucose levels stable, people with diabetes mellitus can control their diet according to a predetermined diet. Adherence to a diabetes mellitus diet can be influenced by family support Positive family support will generate motivation both when it comes to controlling diet patterns and blood sugar levels Purpose: To determine the relationship between family support and dietary adherence in patients with type 2 diabetes mellitus in the Diabetes Mellitus Association of Surakarta. Method: Observational research with a cross sectional design. This research was carried out at the Diabetes Mellitus Association of Surakarta, with a sample of 41 people. The instruments used in this study were the Hensarling Diabetes Family Support Scale (HDFSS) questionnaire and the Perceived Dietary Adherence Questionnaire (PDAQ) questionnaire Results: The relationship between family support and dietary adherence was found that as many as 28 respondents (93.4%) had a high family support category with high dietary adherence compared to respondents who had a low family support category with low dietary adherence as many as 6 respondents (54.5%). The results of the statistical test on the variable of family support with dietary adherence obtained a value of p = 0.008 (p <0.05) which means that there is a relationship between family support and dietary adherence in patients with type 2 diabetes mellitus in the Diabetes Mellitus Association of Surakarta Conclusion: There is a relationship between family support and dietary compliance in type 2 diabetes mellitus patients at the Surakarta Diabetes Mellitus Association.

Abstract

Background: Knowledge of BSE among adolescents is very high. Through breast self-examination, early changes can be detected, health awareness can be increased, and breast cancer prevention can be contributed. Breast cancer is one of the most common types of cancer in women and is a malignant tumor that grows in breast tissue. Early detection is needed to prevent breast cancer in adolescents. State Senior High School 1 Batukliang is one of the Senior High Schools in Batukliang District, Central Lombok Regency that has not received information about breast cancer. Purpose: To determine the relationship between the level of knowledge of female adolescents in grade XII of State Senior High School 1 Batukliang and efforts to detect breast cancer early. Method: This is an analytical observational study using a cross-sectional research design that only takes data at one time, which was conducted from July to December 2023. The population in this study were all female adolescents in grade XII of State Senior High School 1 Batukliang totaling 68 people. Data analysis was carried out using univariate analysis and bivariate analysis using the Chi square test and Spearman test. Results: The majority of respondents were at a high level of knowledge of 27 respondents (46.6%), sufficient knowledge of 25 respondents (43.2%), and the lowest knowledge of less than 6 respondents (10.3%). In addition, most female students made early detection efforts, namely 36 respondents (62.1%) and those who did not make early detection efforts were 22 respondents (37.9%). Based on the Spearman test, there was a statistically significant relationship between BSE knowledge and the attitudes of young women (ρ value = ​​0.00 <0.05). Conclusion: There is a relationship between the level of respondent knowledge and early detection efforts of breast cancer in Central Lombok Regency in 2023, with the results of the chi square test obtained a value of ρ = ​​0.000 <0.05.

Abstract

Background: Blacksmithing work poses high risks for occupational diseases, including musculoskeletal disorders (MSDs), such as hearing loss, injuries from sharp tools, and muscle strain from hammering activities. In Sawan Village, there are 25 workplaces with at least two workers per location, most of whom report fatigue and discomfort. Purpose: To describe the prevalence of musculoskeletal disorders among blacksmith workers in the village. Method: A descriptive study design with a cross-sectional method and total sampling technique, involving 50 workers from 25 blacksmithing centers. Data were collected using risk factor questionnaires, Nordic Body Map (NBM) questionnaires, and measurements of body weight and height. Data analysis was conducted through cross-tabulation using SPSS software, based on risk factors and the severity of the disorders. Results: Showed that most workers were under 45 years old (75%), had a BMI classified as thin-normal (72.72%), had 11-20 years of experience (59.09%), worked ≤8 hours per day (54.55%), and took breaks lasting less than 60 minutes (43.18%). Musculoskeletal disorders were predominantly found in the wrists and hands, with all respondents (100%) categorized as having a low-risk level. Conclusion: 44 workers reported musculoskeletal complaints, all within the low-risk category. Suggestion: Interventions in the form of improving work ergonomics, regulating rest periods, and occupational health education are essential to reduce the risk of MSDs and improve the quality of work of blacksmiths.

Abstract

Background: Quality of care affects patient outcomes and health system efficiency. In the emergency department, length of stay is an important indicator influenced by several factors such as clinical skills, communication, and resource management. Purpose: To explore the relationship between quality of care and length of stay in the emergency department. Method: A Systematic Literature Review (SLR) with PRISMA guidelines, reviewing articles between 2014 and 2024 regarding the impact of quality of care on length of stay in the emergency department. Results: Decreased quality of care, such as delays in service and lack of resources, can increase length of stay, while better care can reduce length of stay. Conclusion: Improving the quality of care in the emergency department can reduce length of stay and result in better and more efficient care. Further research is needed to identify specific strategies to improve quality and reduce length of stay.

Abstract

Background: Having a child admitted to the intensive care unit (ICU) is an extremely distressing experience for parents. This heightened stress can negatively affect their mental health and their capacity to support their child's recovery process. To address this, a range of interventions aimed at empowering parents has been developed. Nonetheless, a comprehensive evaluation of the effectiveness of these interventions in alleviating parental stress is still lacking. Purpose: To evaluate different parent empowerment strategies used in pediatric critical care settings and examine their effects on reducing stress among parents. Method: A systematic review was carried out, following the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines for selecting relevant studies. The review began with formulating clinical questions and defining PICOS criteria—P (Population): parents of children in the pediatric intensive care unit (PICU); I (Intervention): empowerment strategies for parents; C (Comparison): routine care without empowerment or parent groups not receiving targeted interventions; O (Outcome): parental stress levels during ICU hospitalization; S (Study design): randomized controlled trials. The literature search used specific keywords, including: “Parent Empowerment,” “Family Centered Care,” “Pediatric Intensive Care,” “PICU,” “NICU,” and “Parental Stress.” Results: Interventions involving educational support, parental empowerment, and family engagement contribute to better mental health outcomes for parents, particularly enhancing their ability to care for critically ill or premature infants in the NICU. Conclusion: Empowerment strategies in pediatric intensive care settings are effective in lowering stress among parents. This review underscores the importance of educational initiatives, family involvement, supportive empowerment techniques, and digital tools such as telenursing in promoting parents' psychological resilience and participation in infant care.

Abstract

Background: One of the main efforts in promoting sustainability is the implementation of ISO 14001, an international standard for Environmental Management Systems (EMS). In addition to helping companies comply with environmental regulations and reduce the risk of legal sanctions, ISO 14001 also contributes to improving operational efficiency, reducing costs, and achieving business sustainability. Effective implementation not only supports environmental preservation but also creates healthier and more productive working conditions. Purpose: To analyze the implementation of the ISO 14001 environmental management system in a concrete production plant. Method: Descriptive observational research with a qualitative approach using a cross-sectional design. This research was conducted at a concrete production company in Demak City, namely PT Varia Usaha Beton Plant Sayung, which has been operating since 1991 and employs 59 workers. The researcher determined the main informant as the subject of this research, namely the manager who is directly responsible for the implementation of ISO 14001 and workers from the quality control section, while for the triangulation informant the researcher chose the Factory Head. ISO 14001 contains 10 clauses, but the researcher only used six clauses with a checklist to assess the implementation of ISO 14001, including organizational context, leadership, planning, support, operations, performance evaluation, and continuous improvement. Results: The implementation of ISO 14001 is in accordance with the standard of 87.2%. Around 71.1% of respondents have a smoking habit which is a major concern in the context of occupational health. This habit can increase the prevalence of health problems such as shortness of breath and flu/prolonged cough observed in some respondents. As many as 20.3% of respondents experienced shortness of breath while working, and 76.2% experienced flu or cough for more than a week. These figures are quite high and may indicate exposure to dust, air pollution, or poor ventilation in the workplace. Most respondents had no history of illness (83.1%), but the high prevalence of respiratory symptoms indicates the need for an environmental evaluation in the workplace. The results of dust measurements also showed that there were areas where the threshold value was exceeded. Conclusion: Although the company has taken positive steps in implementing an environmental management system, controlling dust and enhancing protection for workers remains a critical focus to prevent occupational diseases that are still occurring. Suggestion: To improve the effectiveness of the Environmental Management System (EMS), organizations need to strengthen operational control and communication with external suppliers to ensure that all processes and business partners meet the established environmental standards. In addition, emergency preparedness should be improved through regular evaluation of emergency response procedures, so that responses to critical situations can be faster and more effective.

Abstract

Background: Standard precautions practice is the main action in preventing and controlling infections in hospitals. Nurse compliance with standard precautions is influenced by various factors, including knowledge, perception, and motivation. Purpose: To analyze the relationship between the level of knowledge, perception, and motivation of nurses with the practice of standard precautions. Method: A quantitative approach, specially focusing on analytical survey and cross-sectional method. The study took place from December 2024-Febuary 2025 in Islamic Hospital of Muhammadiyah Medical Care Center, Pekajangan. The independent variable was knowledge, risk perception, and motivation of nurses, dependent variable is standard precautionary practices in hospitals. Using systematic random sampling and slovin formula, 173 respondents were selected, who met specific inclusion and exclusion criteria. The analysis proceeded with a univariate test to determine the frequency distribution of respondent’s characteristics, followed by bivariate analysis using spearman rank test Results: The study revealed significant factors influencing the practice of standard precautions. It showed a gender-based difference in adherence (p = 0.008), a positive relationship between the level of perception and practice (p = 0.033), and a significant association with motivation level (p = 0.01). Conclusion: Nurses generally had good to fair levels of knowledge, perception and motivation regarding standard precautions, indicating effective implementation of patient safety. Perception and motivation significantly influenced compliance, while knowledge without significant influence.
