Abstract

Urinary tract infection (UTI) is an infection caused by the growth of microorganisms in the urinary tract, which in normal conditions urine does not contain bacteria, viruses or other microorganisms. Urinary tract infections can occur in men and women of all ages. The purpose of this special study is that the author is able to apply the provision of drinking water therapy and warm compresses to overcome pain problems and changes in urine elimination patterns in patients with Mrs. SM with a medical diagnosis of UTI through a nursing process approach. The method used in the case study on Mrs. SM with UTI is a case study with a nursing process approach which was carried out on March 29-April 1, 2025 in the Internal Medicine Room of Ende Hospital. The results of the case study showed that the assessment data found in Mrs. S.M with UTI were weakness, pain in the waist, lower abdominal pain, pain that comes and goes and feels like being stabbed, painful urination, abdominal distension, bloating, grimacing face, BP: 110/80 mmHg, S: 38 0C, N: 94x/min, RR: 20x/min, urination comes out a little at a time. The nursing diagnosis raised in Mrs. S.M's case was pain and impaired urinary elimination. After implementation, the results obtained were that the problems of pain and impaired urinary elimination were resolved thanks to the cooperation of the patient, family and nurse. Nursing care carried out on Mrs. S.M's patient can run well thanks to the cooperation and support of the family. Therefore, it is expected that the patient will behave healthily by not holding back urine and consuming lots of water 8-10 glasses a day.

Abstract

Hypertension is a major health problem in developed and developing countries. Hypertension is a non-communicable disease and is the number one cause of death globally (Mahfud et.al, 2019). World Health Organization (WHO) data for 2022 shows that around 1.13 billion people in the world suffer from hypertension. This case study aims to provide nursing care to Mrs. E. E with Hypertension and analyze the gap between theory and real cases. The method used in this scientific paper is the case study method with a nursing care approach which includes assessment, nursing diagnosis, nursing intervention, implementation and nursing evaluation. The results of the case study on Mrs. E. E found BP data 160/90 mmHg, headache, nape felt tense, weakness, CRT>3 seconds Hb 5, 2 g/dl. The nursing problems that arise are: ineffective peripheral perfusion related to increased blood pressure, acute pain related to physical injury agents, activity intolerance related to weakness and risk of decreased cardiac output related to increased afterload. Nursing interventions are carried out based on nursing problems. Implementation was carried out for 3 days and the results of the evaluation were: ineffective peripheral perfusion was resolved, acute pain was resolved, activity intolerance was resolved and the risk of decreased cardiac output was resolved. Every person suffering from hypertension has different signs and symptoms depending on how long the person has suffered and the length of treatment the patient has undergone. Therefore, it is hoped that you will maintain your health and always comply with taking medication

Abstract

According to Basic Health Research (RISKESDAS, 2018), the prevalence rate of asthma at all ages in Indonesia is 2.5%. The incidence of asthma is most common in the age group of 75 years and above and begins to decrease in the age group of 15-24 years. The role of nurses in providing nursing care to patients is by providing pharmacological therapy, non-pharmacology and providing education about bronchial asthma. In the implementation of nursing care in asthma cases, nurses are challenged or have problems with the lack of understanding of nurses about the use of literature in setting service standards such as having to use the Indonesia Nursing Diagnostic Standards (SDKI), Indonesia Nursing Output Standards (SLKI), Indonesia Nursing Intervention Standards (SIKI) and patient non-compliance in preventing asthma attacks. This case study aims to describe the nursing care of Mrs. M. E. with a medical diagnosis of bronchial asthma and analyze the gap between theory and real cases. The method used is a case study with a nursing care approach which includes review, diagnosis, planning, implementation and evaluation of nursing. The results of the case study found three nursing problems in the case, namely ineffective airway clearance related to airway hypersecretion, ineffective breathing patterns related to inhibition of breathing effort, and sleep pattern disorders related to shortness of breath. Nursing interventions and implementations are designed and implemented with dependent actions such as observation, education and interdependent such as collaborative actions. After 3 days of implementation, the problem of ineffective airway cleaning was partially resolved, and the problem of ineffective breathing patterns and sleep pattern disorders was resolved. After 3 days of implementation, the nursing problem of ineffective breathing patterns and sleep pattern disorders was resolved, while the problem of ineffective airway clearance was partially resolved and it was recommended for patients and families to follow all recommendations from health workers with the aim of preventing the onset of more severe complications

Abstract

Tuberculosis (TB) is a contagious infectious disease that is one of the leading causes of death worldwide after HIV. Based on WHO reports, it is estimated that there are 10,600,000 cases of tuberculosis globally with a mortality rate of 1,400,000 cases. The purpose of this study is to improve the quality of life of Mr. MKE with a medical diagnosis of pulmonary tuberculosis at the RPK RSUD Ende. The method used is a case study with a nursing care approach. The results of Mr. MKE's assessment found that the patient said he was a little short of breath, coughing up thick yellow phlegm, sometimes greenish, which sometimes can be expelled, sometimes difficult to expel since ± 4 months. Pain in the right side of the chest when coughing, fever that goes up and down for + one month, dizziness when getting out of bed, weak, limp. Decreased appetite, nausea, vomiting, eating 5 times a day, the portion consumed is only + 2-3 spoons, sweating at night without doing any activity, previous weight: 60 kg. Physical activities such as getting out of bed, moving, etc. are assisted by the family. Hb 6 gr/dl. Nursing problems found were ineffective airway clearance; hyperthermia; nutritional deficits; ineffective peripheral perfusion; Activity intolerance; ineffective health management. After 3x24 hours of care and treatment, the results showed that ineffective airway clearance was partially resolved, hyperthermia was resolved, nutritional deficits were partially resolved, ineffective peripheral perfusion was resolved, activity intolerance was partially resolved, and ineffective health management was resolved. This is expected to improve the patient's quality of life. It is recommended that patients and their families follow all recommendations from doctors and nurses with the aim of accelerating the patient's healing process.

Abstract

Pulmonary Tuberculosis (TB) is an infectious disease that causes the highest number of deaths in the world. Pulmonary tuberculosis is caused by the bacteria Mycobacterium Tuberculosis. Tuberculosis is a threat to the Indonesian population, because this disease is the third leading cause after heart disease and acute respiratory disease (Puspitasari et al, 2023). This case study aims to provide nursing care to Mrs. M. T with Pulmonary Tuberculosis and analyzes the gap between theory and real cases. The method used in this scientific paper is a case study method with a nursing care approach which includes assessment, nursing diagnosis, nursing intervention, implementation and nursing evaluation. The results of the case study on Mrs. M. T found the patient complaining of coughing, shortness of breath, dizziness, weakness, chest pain, cough mixed with blood, decreased appetite approximately 3 months ago, CRT>2 seconds, HGB: 2.6 mg/dL, WBC: 13.78. Nursing interventions are carried out based on nursing problems. Implementation was carried out for 3 days and the results of the evaluation were: the problem of ineffective airway clearance was partially resolved, the nutritional deficit problem was partially resolved, the peripheral perfusion problem was partially resolved and the problem of activity intolerance was resolved. Every person suffering from Pulmonary Tuberculosis has different signs and symptoms depending on how long the person has suffered and the length of treatment the patient has undergone. Therefore, it is hoped that you will maintain your health and always comply with taking medication.

Abstract

Hypertension is a cardiovascular disease with a major risk factor for global mortality. This case study aims to describe nursing care for patient Mr. I. R diagnosed with hypertension in the Internal Medicine Ward I, II of RSUD Ende. The method used is a case study approach, including assessment, nursing diagnosis, planning, implementation, and evaluation. The results showed nursing problems including acute pain, risk of ineffective cerebral perfusion, sleep pattern disturbance, and knowledge deficit. Nursing interventions were carried out over three days, with most problems partially or fully resolved. A gap between theory and practice was found, where the theory includes activity intolerance, which was not present in the real case. It is recommended that patients modify their lifestyle to be healthier and comply with pharmacological therapy

Abstract

Tuberkulosis disebabkan oleh bakteri Mycobakterium tuberculoisis yang penyebarannya melalui udara. Tuberkulosis adalah salah satu penyebab utama kematian sehingga perlu dilakukan program penanggulangan tuberkulosis secara berkesinambungan. Estimasi jumlah orang terdiagnosis tuberkulosis TBC pada tahun 2023 secara global sebanyak 8,2 juta kasus. Tujuan studi kasus penulis mampu menggambarkan pelaksanaan asuhan keperawatan pada pasien dengan diagnosis tuberkulosis paru di RSUD Ende. Metode yang digunakan dalam karya tulis ilmiah ini adalah metode studi kasus dengan pendekatan asuhan keperawatan yang meliputi pengkajian, diagnosa keperawatan, intervensi keperawatan, implementasi keperawatan dan evaluasi keperawatan. Hasil studi kasus pada Tn. A.P ditemukan data: sesak napas, batuk >2 minggu disertai dahak berwarna kuning kental dan sulit dikeluarkan, mual, muntah, keringat tanpa aktivitas, demam sesekali, nafsu makan menurun, berat badan menurun, terdengar suara napas wheezing dan ronchi, pada pemeriksaan sputum ditemukan bakteri Mycobakterium tuberculosis. Masalah keperawatan yang muncul adalah bersihan jalan napas tidak efektif berhubungan dengan hipersekresi jalan napas, pola napas tidaka efektif berhubungan dengan hamabatan upaya napas, defisit nutrisi berhubungan dengan faktor psikologis (keengganan untuk makan), gangguan pola tidur berhubungan dengan kurang kontrol, ketidakpatuhan berhubungan dengan program terapi lama, resiko penyebaran infeksi berhubungan dengan peningkatan paparan organisme patogen lingkungan. Intervensi keperawatan dilakukan berdasarkan masalah keperawatan. Implementasi dilakukan selama 3 hari dan hasil evaluasi ditemukan masalah sebagian teratasi. Disimpulkan bahwa kasus Tn. A.P terdapat kesenjangan antara teori dan kasus yaitu batuk berdarah dan nyeri dada tidak ditemukan. Oleh karena itu tuberkulosis paru sebaiknya patuh terhadap pengobatan serta berbagai upaya pencegahan yang telah diajarkan kepada pasien maupun keluarga.

Abstract

Pulmonary Tuberculosis is a disease that has the nature of airborne infection caused by the bacteria Mycobacterium Tuberculosis, this disease attacks the respiratory tract so that it can cause problems in the respiratory tract. One of the actions that can be given is airway management or airway management aimed at reducing complications related to the airway such as clearing the airway. The purpose of the case study is to be able to apply airway management to improve airway clearance in TB patients treated at Ende Regional Hospital. This study uses a case study method on patient Mr. L.N.H with Pulmonary Tuberculosis who was treated at Ende Regional Hospital on May 8-10, 2025. The results of the study were carried out based on the nursing process approach in the assessment found patient data Mr. L.N.H complained of coughing up yellow mucus for ± 1 month, decreased appetite, nausea, chest pain when coughing, body feels weak, both legs feel weak and heavy, weight loss before illness was 60 kg and when sick was 46 kg. The nursing problem that arose in Mr. L.N.H.'s case was ineffective airway clearance related to airway hypersecretion. After providing nursing care, it was found that implementing airway management could help the patient improve ineffective airway clearance.
