Abstract

Asperger syndrome is the term applied to the mildest and highest functioning end known as the spectrum of pervasive developmental disorders (or the autism spectrum). It is known to be related to a neurologically based disorder of development. Most of the causes of these disorders are unknown, which are deviations or abnormalities in three broad aspects of development: social relatedness and social skills, the use of language for communicative purposes and certain behavioral and stylistic characteristics involving repetitive or perseverative features and a limited but intense range of interests. It is the presence of these three categories of dysfunction, which can range from relatively mild to severe, which clinically defines all of the pervasive developmental disorders, from AS through to classic autism. Some researchers have suggested that the basic neuropsychological deficit is different for the two conditions, but others have been unconvinced that any meaningful distinction can be made between them. Abstract

Postpartum depression is an affective disorder that affects predominantly in women after childbirth. The typical characteristics are feeling miserable, fatigue, weariness, and there are changes in sleeping and eating patterns. History taking from patient and family is the essential part to diagnose the patient, which Edinburgh Postnatal Depression Scale (EPDS) is a screening tool frequently used. Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) criteria is utilized to establish if the patient is experiencing postnatal depression. Postnatal depression can become a major illness if not treated seriously. There are many treatments available to treat the postpartum depression such as drugs, psychotherapy, and/or alternative therapy. With suitable treatment, it can increase the relationship between mother-child and the quality life of the family. Keywords: Postpartum depression, affective disorder, psychotherapy

Abstract

Benign Prostatic Hyperplasia (BPH) is a nonmalignant enlargement of the prostate gland. BPH is commonly found in elderly man. The clinical manifestation include Lower Urinary Tract Syndrome (LUTS), Bladder Outlet Obstruction (BOO), acute and chronic urinary retention, urosepsis, urinary tract infection, bladder stone, and hematuria. BPH is diagnosed based on history taking, physical examintaion, rectal examination, laboratory examination, and urinary flow rate. Management of BPH based on patient symptom, include watchful waiting, medical therapy, minimally invasive therapy, and surgical therapy. Abstract

Reluctance to get treatment is one of the reasons why cancer patients come at the advance stage to health care provider. Numerous information of alternative medicine seems to influence the patients to try alternative medicine and delay their treatment in hospital. A descriptive study is conducted in 2 hospitals in Denpasar to obtain the description of alternative medicine history in cancer patients, with breast cancer and colorectal cancer patients as samples. Total samples were 38 patients, with 23 of them (60.5%) have tried the alternative medicine, whereas 13 patients (56.52%) after visiting the health care provider. Among those who tried the alternative medicine, 14 patients (60.87%) seek this medicine because of their fear to the conventional medicine. The most popular method of alternative medicines is herbal medicine. Most of the patients got information about alternative medicines from people whom close to them such as their family, relatives, and neighbors. However, most of these patients, who have tried the alternative medicine, in the end came back to the health care provider/ hospital after they realized that the result of the alternative treatment did not improve their health condition. Abstract

Alopecia or hair loss is a quite disturbing case. It can occur in both women and men. In women, the most familiar reason for hair loss is telogen effluvium (TE)succeed by female pattern hair loss (FPHL) and chronic telogen effluvium (CTE). Telogen effluvium generally occurs at any age, it is caused by prematurely transition of anagen hairs to telogen hairs. It can be induced by several events such as physiologic or emotional stress or certain drugs. Meanwhile, female pattern hair loss is caused by increasing of 5?-reductase enzymes and androgen receptors in effected scalp. Chronic telogen effluvium is the shedding of telogen hairs that occur over 6 month with idiopathic cause. There are also other causes of hair loss in women such as alopecia areata (AA), trichotillomania, and traction alopecia.TE and CTE are treated by removing the triggering factors. Patients need to be informed and educated that it is self limited and usually resolve with time. For FPHL, the treatment includes minoxidil 2% solution, antiandrogen therapies and hair transplantation. Meanwhile, AA is treated by intralesional corticosteroid injection, psoralen and ultraviolet A (PUVA). In the other hand, women with trichotillomania and traction alopecia need counselling and education to change their behaviours. Abstract

Post-traumatic stress disorder (PTSD) is a mental disorder that occurred after the exposure of traumatic event, which is characterized by re-experiencing, avoidance and hyperarousal. The Lifetime incidence of PTSD is estimated around 13%, and the lifetime prevalence rates ranges from 3%-78%. The exact pathophysiology of PTSD is still unclear. However, there are a number of factors that must be considered in interaction between environmental factor, personal response against stress, and psychopathology. The management of PTSD includes the use of antidepressant, CBT, and other psychotherapy. Keywords : PTSD, re-experiencing, avoidance, hyperarousal.

Abstract

Anorexia nervosa (AN) is a type of eating disorder, but it is more importantly a serious psychiatric illness associated with significant medical and psychiatric morbidity, psychosocial impairment and increased risk of death. The clinical features are characterized by deliberate weight loss, arising from food avoidance often in combination with self-induced vomiting, excessive exercise and using diuretics and appetite suppressants. Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) criteria is used to determine if the patient is suffering from anorexia nervosa. Anorexia can have dangerous psychological and behavioral effects if left untreated. The treatment of anorexia has to focus on more than weight gain and the physical damage caused by malnutrition.
