Abstract

Background: Health cadres are strategically positioned to deliver cervical cancer prevention messages, but limited training and educational resources may reduce their motivation to provide counseling. Objective: To determine changes in cadres' motivation to provide cervical cancer prevention counseling after receiving combined video education and a guidebook. Methods: A pre-experimental one-group pretest-posttest study was conducted in Wijimulyo Village, Nanggulan, Kulon Progo, Indonesia, in March 2026. Twenty-five female health cadres were selected by simple random sampling from a population of 110. Motivation was measured using a 19-item Likert questionnaire with a raw score range of 0-57. The instrument had item-total correlations above 0.361 and a Cronbach's alpha of 0.725. Participants completed a pretest, watched a 2-minute 47-second educational video, participated in a researcher-guided review of the three guidebook modules, and joined a facilitated discussion in which key messages were clarified and linked to their counseling roles before completing an immediate posttest. The educational intervention, including the video presentation, structured guidebook review, and facilitated discussion, lasted approximately 60 minutes. Paired scores were analyzed using the Wilcoxon signed-rank test. Results: The median motivation score increased from 36 (interquartile range [IQR] 34-37) before the intervention to 37 (IQR 35-41) afterward. Thirteen cadres had higher scores, six were unchanged, and six had lower scores. The increase was statistically significant (Z = -2.28, p = 0.023) with an effect size of r = 0.52. Conclusion: Combined video education, structured guidebook review, and facilitated discussion were associated with an immediate increase in cadres' self-reported motivation to provide cervical cancer prevention counseling. Actual counseling performance and community screening participation were not measured; therefore, no conclusions can be drawn regarding these outcomes. Follow-up assessment is needed to determine whether the motivational change is sustained over time.
