Abstract

Patient safety is the essence of health service quality. Hospitals are a combination of several elements that together produce a high-risk situation. Safety culture should be the core in hospital organization. From the data obtained there are still incidents of safety of patients in hospital X Semarang and there has been no trend decrease in the incidence of meaningful incidents over time. This research aims to determine the cultural overview of safety and how the management efforts influence in shaping the safety culture of hospital X Semarang. This type of research is quantitative analytic with survey method that analyzes management efforts in shaping safety culture in hospital X Semarang. Research conducted in January-March 2020. Data collection techniques using a questionnaire adapted from the American College of Healthcare Executives in 2017 and a description of safety culture were acquired by collecting data from a Patient safety culture questionnaire adopted from the Hospital Survey on Patient Safety Culture questionnaire published by The Agency for Healthcare Research and Quality (AHRQ) in 2004. Number of respondents as many as 220 people selected in proportionate stratified random sampling. The results showed that the safety culture at hospital X showed a positive response in the organizational learning dimension and continuous improvement (90.15%), the dimension of cooperation in units (81.82%) and dimension of management support to patient safety efforts (80.45%). The other dimension of safety culture has a negative response (< 75%). Management efforts that include the determination of vision, trust values, respect and inclusion, selection, development and management engagement, leadership development that prioritizes safety, just culture, expectations of organizational behaviour as well as assumptions, values and artifacts related to safety culture have an influence in the establishment of safety culture in hospital X Semarang.Keyword: Safety Culture, management efforts

Abstract

Baby given milk other than ASI has 17 times greater risk of experiencing diarrhea and 3 to 4 times more likely to be exposed to respiratory than babies who get breast milk. According to the data and the report of the Office of North Barito, coverage of exclusive breast feeding only 57.2% of the targets set in the amount of 80%. And according to data and reports KIA Lahei II public health scope of Exclusive breast feeding only amounted to 13.4%. The purpose of the study to find out the influence of health education on motivation against pregnant women in the exclusive breast feeding 0-6 month in the region of clinics Lahei II. Research methods using quasy experiment design with a non randomized control group pretest postest design. The sample is first pregnant women (G1) which are domiciled in the territory of origin and work of clinics Lahei II at the time the research was conducted. Sampling with purposive sampling, obtained 46 people are divided into groups of treatment and control. Data collection with interviews using questionnaire. The results of data analysis with Paired T Test with p = 0.000, there is a difference between the motivation of the respondent before with wellness counseling, and after p = 0,236 that means no difference between the control group motivation first measurement with measurement Second, as well as Unpaired T test Test with p = 0.001, there is a difference between the treatment groups with controls. It can be concluded that there is the influence of health education on motivation against pregnant women in the exclusive breast feeding.Keywords: Health Education, exclusive breast feeding, pregnant mothers motivation

Abstract

As a result of low health literacy levels, patients will face more challenges when they access and use the health care service system. This research purpose is to know about literacy levels based on a patient's characteristics and their activities in the Program of Chronic Disease Management (PROLANIS). The researcher did the descriptive research by using the HLS-EU-Q16 questionnaire which was adapted in the Indonesian language. Respondents in this study were 20 Diabetes Mellitus Patients in Prolanis Participants in 2020. The result of the research showed that the respondents who live in the Kedungmundu Primary Healthcare Center (PHC) area mostly were elderly (55%), female (55%), high school graduates (50%), have a job (50%). PROLANIS activities followed by the respondent are medical consultation (100%), education program (100%), and gymnastic (90%). Health literacy categories were Inadequate HL (35%), Problematic HL (50%), and Sufficient HL (15%). This result shows the extra need for handling people with Diabetes Mellitus, especially for the elderly. It should be important to monitor a patient's blood sugar and to educate the patients for better self-management.Keywords: diabetes mellitus, literasi kesehatan, prolanis

Abstract

Metabolic Syndrome is a set of deviation functions of the body in the form of central obesity, high blood pressure, dyslipidemia (increased levels of cholesterol especially LDL, triglyceride, and low HDL levels), and DM. Metabolic syndrome is basically a pain condition that is characterized by a set of abnormalities with a variety of consequences clinical if allowed to continue and are not addressed early will lead to various degenerative diseases such as diabetes mellitus. The purpose of this study is to determine the relationship between body mass index and metabolic syndrome in type 2 DM patients in Posbindu Sagita Kelurahan Nambangan Lor Manguharjo. This research design is Cross Sectional study. The population of this study are patients with Type 2 DM who is suffering from metabolic syndrome and do not suffer from the metabolic syndrome. The method used is Purposive Sampling technique with the analysis of the Chi-Square Test. The Variables that proved to the Body Mass Index is associated with Metabolic Syndrome in Type 2 DM patients in Posbindu Sagita Kelurahan Nambangan Lor Manguharjo is the value of the ?-value of 0.000 < ? 0.05 RP by 20,0 and (95% CI = 6,5 to 61,1). There is a relationship between Body Mass Index and the Metabolic Syndrome in Patients with Type 2 Diabetes Mellitus. This study shows that respondents who suffer from Type 2 diabetes with obesity are 20 times more likely to suffer from metabolic syndrome than respondents who are not obesity.Keywords: Body Mass Index, Metablic Syndrome, Diabetes Mellitus

Abstract

Medical records at non-hospital health care facilities are stored for at least 2 years after the last date of treatment. Halmahera Primary Health Care (PHC) has never done retention or destruction of medical record documents for pulmonary TB cases. The study aims to determine the factors that were obstacles in the retention of pulmonary TB cases at Halmahera PHC. This research is descriptive qualitative research with cross sectional approach. Data collection by observation and interviews. The subjects of this study were 5 filing officers. The object of this study were medical record documents of pulmonary TB patients, the policy of medical record document retention for pulmonary TB cases, the standard of medical record document retention for pulmonary TB cases. Based on the results of the study, 3 out of 5 filing staff understood retention of medical record documents in general, The filing storage systems was centralized but medical record documents for pulmonary TB cases were not included into the family folder, there were no policies and standards regarding retention of medical record documents for pulmonary TB cases, there is no facilities for medical record document retention for pulmonary TB cases. The obstacle in the implementation of medical record document retention for pulmonary TB cases were the unavailability of retention facilities, no schedule for archive retention, no place to store the inactive medical record documents, and excessive staff workload. Suggestions, The PHC need to make standards about document retention for pulmonary TB cases, provide information and technical guidance to filing staff about medical record document retention.Keywords: filing staff knowledge, storage system, retention standard Literature : 15 (2001 – 2020)
