Abstract

Background: Diabetes mellitus is associated with increased blood glucose and triglyceride levels, which can lead to an oxidative stress. Zinc (Zn) is a micronutrient that has antioxidant properties and involved in lipid and glucose metabolism. This study aimed to evaluate the effect of zinc on the levels of fasting blood glucose (FBG), triglycerides (TG), and malondialdehyde (MDA) in male diabetic Rattus norvegicus Wistar rats. Methods: Diabetes was induced by intraperitoneal (i.p.) injection of 40 mg/kg BW streptozotocin (STZ) and confirmed by FBG level higher than 200 mg/dL after 2 weeks. The rats were randomly divided into three groups: control group (STZ), treatment I (STZ + zinc 5 mg/kg BW), and treatment II (STZ + zinc 10 mg/kg BW). Zinc was administered by oral gavage for 30 days. At the end of the experiment, levels of FBG, TG, and MDA were measured. Data were analyzed using paired t-test or Wilcoxon test as appropriate. Results: Supplementation of 5 mg/kg zinc significantly decreased the levels of FBG (pre-intervention: 328.95±70.90 mg/dl, post-intervention: 144.35±34.27 mg/dl, p<0.05), TG (pre-intervention: 252.48±26.30 mg/dl, post-intervention: 147.90±12.18 mg/dl, p<0.05), and MDA (pre-intervention: 12.11±6.46 nm/ml, post-intervention: 4.75±2.34 nm/ml, p<0.05). Moreover, supplementation of 10 mg/kg zinc decreased the levels of TG (pre-intervention: 275.62±56.25nm/ml, post-intervention: 165.58±22.63 nm/ml, p<0.05) and MDA (pre-intervention: 13.08±6.60 nm/ml, post-intervention: 5.08±2.40 nm/ml, p<0.05). Conclusion: Supplementation of zinc significantly reduced the levels of FBG, TG, and MDA in diabetic rats.

Abstract

Background: The etiology of unexplained infertility has not been fully understood. This study aimed to determine the effect of antisperm antibody (ASA) from infertile women on viability, motility, plasma membrane integrity, and status of tyrosine phosphorylation in the human spermatozoa.Methods: An experimental in vitro study was conducted at the Department of Biology, Faculty of Medicine, Universitas Indonesia from February to November 2014. Spermatozoa from normal fertile donors was incubated with serum containing ASA from infertile women at several dilutions (1/1000, 1/100, 1/10, and without dilution) for 1 and 2 hours. The plasma membrane integrity was assessed with hypoosmotic swelling (HOS) test, whereas the status of tyrosine phosphorylation was analyzed using Western immunoblotting and immunocytochemistry.Results: After 1 hour incubation time, ASA caused a decrease in sperm viability, motility, plasma membrane integrity, and inhibit sperm tyrosine phosphorylation. ASA caused a decrease in viability, motility, sperm plasma membrane integrity, and tyrosine phosphorylation of sperm after 1 hour incubation time.Conclusion: ASA from infertile women reduced the sperm viability, motility, plasma membrane integrity, and capacitation in dose and time dependent manner.
The accuracy of Aziza’s scoring system in limited slice non-enhanced thoracic CT for the diagnosis of adult pulmonary tuberculosis

Abstract

Background: Tuberculosis (TB) is still a major cause of morbidity and mortality in Indonesia. Thus, a fast and accurate method in diagnosing pulmonary TB (PTB) is needed, especially in adult smear-negative-TB cases. The purpose of this study was to determine the accuracy of a novel scoring system (Aziza’s score) using clinical data and characteristic abnormalities that are found in the limited slice non-enhanced thoracic CT. Methods: This was a prospective cross-sectional study. A limited slice non-enhanced thoracic CT was performed to establish a diagnosis of PTB in patients with suspected PTB and to assess the diagnostic value of the scoring system. A reference standard used in this study was the clinical expertise of two pulmonologists. The scoring system was analyzed using bivariate and multivariate Cox regression analysis. Results: 84 of 130 suspected PTB patients were diagnosed as PTB. They were diagnosed based on pulmonary characteristic abnormalities that were found in the limited slice non-enhanced thoracic CT. The scoring system was analyzed using bivariate and multivariate Cox regression analysis. The total scores which were equal to or more than 29 could diagnose PTB with the accuracy of 96.1% (95% CIs=91.3–98.3), sensitivity of 96.5% (95% CIs=90.1–98.8), and specificity of 95.6% (95% CIs=90.1–98.8), This novel scoring method was name as an Aziza’s scoring system. Conclusion: Aziza’s scoring system performed in the limited slice non-enhanced thoracic CT has a higher accuracy compared to the reference standard for the diagnosis of adult PTB.

Abstract

Background: Helicobacter pylori is an important gastrointestinal pathogen associated with gastritis, peptic ulcers, and an increased risk of gastric carcinoma. The present study was carried out to determine the relationship between this organism with different gastrointestinal ailments. Methods: 150 outpatients referrals to Saudi Arabian Medical City, Riyadh, Kingdom of Saudi Arabia was recruited in January to June 2015. Each patient was subjected to endoscopic examination. Biopsy specimens were taken from the stomach for rapid urease test and culture. Suspected H. pylori colonies were subjected to colony morphology identification, microscopical examination and biochemical reactions. The samples were also subjected to PCR to detect ureA subunit of urease gene. Results: The endoscopic examination of patients revealed normal, gastric ulcer, duodenal ulcer, gastritis, and gastric cancer with a rate of 20.7%, 20%, 24%, 33.3%, and 2%, respectively. Direct smear exam revealed that 52% of patients were H. pylori positive while culture and rapid urease test showed a prevalence of 71.33%. Fifty four biopsies (36%) were urease positive after 1 hour at room temperature, 39 (62%) after 1 hour incubation at 37°C and 14 (71.33%) after 24 hours incubation. Isolated H. pylori showed that they were catalase, oxidase, and urease positive. PCR results showed 411-bp fragment, which is indicative for the ureA subunit of urease gene. Conclusion: The prevalence of H. pylori infection was high among tested population. Strong association between H. pylori and duodenal ulcer was noticed. A 411-bp fragment indicative of the ureA subunit of urease gene was detected in all the tested isolates.

Abstract

Reducing low density lipoprotein (LDL) cholesterol level is an established primary and secondary prevention strategy for coronary heart disease. However, not all patients are able to achieve their LDL targets as recommended by the guidelines. Over the last 10 years, high plasma LDL level is known to be associated with a higher level of pro-protein convertase subtilisin kexin-9 (PCSK-9). Loss-of-function mutations in the PCSK-9 gene is associated with lower plasma LDL level and cardiovascular risk. Since its discovery in 2003, PCSK-9 has triggered many researchers to design a PCSK-9 inhibitor to reduce LDL cholesterol through competitive inhibition of this molecule. Some phase III clinical trials have showed promising results of PCSK-9 inhibitor efficacy in lowering LDL level and improving clinical outcome. This article aims to discuss the role of PCSK-9 in LDL metabolism and the efficacy of PCSK-9 inhibitor in reducing plasma LDL level.

Abstract

Background: Candida species are important hospital-acquired pathogens in infants admitted to the neonatal intensive care unit (NICU). This study was performed in the NICU of Saudi Arabian Hospital, Riyadh region, KSA to analyze patterns of neonatal Candida colonization as well as to determine the potential risk factors. Methods: Weekly surveillance fungal cultures of anal area, oral cavity, umbilicus and ear canal of neonates were performed from birth until their discharge from the hospital. Colonization was analyzed for timing, site, species, birth weight and gestational age. Potential environmental reservoirs and hands of health care workers (HCWs) were also cultured monthly for fungi. Antifungal susceptibility of the identified isolates was also determined. Results: One hundred subjects have been recruited in this study. The overall colonization rate was 51%. Early colonization was found in 27 (27%) neonates whereas 24 (24%) neonates were lately colonized during their stay in NICU. Colonization was more in preterm neonates than in full and post term. Perianal area and oral cavity were the most frequent colonized sites. C. albicans was the main spp. (58.8%) isolated from the neonates followed by C. tropicalis (17.6%), C. glabrata (15.6%), and C. krusei (2%). Of the 51 isolated Candida spp., 68.6% were sensitive to fluconazole, 80% to itraconazole and 64.7% to ketoconazole, while only 33% were sensitive to amphotericin B. Conclusion: Candida has emerged as a common cause of infections in infants admitted to NICU, and C. albicans is the most commonly isolated candidal species. Neonatal infections caused by non- albicans species occur at a later age during their stay in NICU.
Antiproliferative activity and caspase enhancement properties of Annona muricata leaves extract against colorectal cancer cells

Abstract

Background: The prevalence of colorectal cancer is rising in Asia including Indonesia. Annona muricata tea leaves, that is traditionally used for maintaining health, and lately being used by cancer patients. The objectives of this study is to investigate its effects in human colorectal cancer cell in vitro and ex vivo. Methods: Thirty patients with colorectal cancer (CRC) were enrolled in a randomized double-blind placebo-controlled trial. They were equally divided into two groups: those treated with 300 mg A. muricata leaf extract and placebo daily for 8 weeks. Serum from supplemented CRC patients of both groups was compared for caspase 9 and caspase 8 enhancement activity. Antiproliferative effect of water extract of A. muricata leaves and its fractions were evaluated against colorectal cancer cell line (DLD-1 and COLO 205) compared with 5-fluorouracil and placebo, the dose range was 62.5-2,000 μg/mL. Method used was 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. Data were analyzed by Mann-Whitney U test. The p value was set at 0.05. Results: Ethanol-soluble fraction of A. muricata leaves extract water extract (ESFAM) leaves extract had cytotoxicity effects on DLD-1 as well as COLO 205 cell line, as shown by the lower IC 50 compared to 5-fluorouracil and placebo, 20.59 μg/mL and 654.9μg/mL, respectively. Serum of subjects supplemented with extract significantly induced caspase 9 (p=0.001) activity of DLD-1 colorectal cancer cell line, but not for caspase 8 activity (p=0.372). Conclusion: The study's results suggest the cytotoxicity potential of A. muricata leaves extract in in vitro and ex vivo studies.
Risk factors of sepsis after open congenital cardiac surgery in infants: a pilot study

Abstract

Background: Postsurgical sepsis is one of the main causes of the high mortality and morbidity after open congenital heart surgery in infants.This study aimed to evaluate the role of cardiopulmonary bypass duration, thymectomy, surgical complexity, and nutritional status on postsurgical sepsis after open congenital cardiac surgery in infants. Methods: A total of 40 patients <1 year of age with congenital heart disease, Aristotle Basic Score (ABS) ≥6 were followed for clinical and laboratory data before and after surgery until the occurrence of signs or symptoms of sepsis or until a maximum of 7 days after surgery. Bivariate analyses were performed. Variables with p≤0.200 were then included for logistic regression. Results: Duration of cardiopulmonary bypass ≥90 minutes was associated with 5.538 increased risk of postsurgical sepsis in comparison to those ≤90 minutes (80% vs 25%, RR=5.538, p=0.006). No association was observed between the incidence of postsurgical sepsis with poor nutritional status (86% vs 84%, RR=1.059, p=1.000), thymectomy (and 50% vs 76%, RR=0.481, p=0.157), and Aristotle Basic Score (p=0.870). Conclusion: Cardiopulmonary bypass time influences the incidence of sepsis infants undergoing open congenital cardiac surgery. Further studies are needed to elaborate a number of risk factors associated with the incidence of sepsis in this population.
High preterm birth at Cipto Mangunkusumo Hospital as a national referral hospital in Indonesia

Abstract

Background: Preterm birth is the leading direct that causes neonatal death. Indonesia was listed as one of the countries with the greatest number of preterm birth in 2010. This study aims to identify the prevalence and the potential risk factors of preterm birth among women underwent delivery in Cipto Mangunkusumo Hospital, an Indonesian national reference hospital. Methods: This retrospective cohort study involved 2,612 women who delivered between January and December 2013. Any clinical data which related to the potential risk factors and outcomes were recorded. The data were managed using chi-square for bivariate analysis and t-test or Mann-Whitney for numerical data followed by multiple logistic regression for multivariate analysis in SPSS version 20.0. Results: Preterm birth affected 1,020 of 2,616 pregnancies (38.5%). Non-booked patients increased nearly twice risk for preterm delivery (OR 1.89, 95% CI 1.37-2.61). While women with singleton pregnancy (OR 0.17, 95% CI 0.12-0.25), head presentation (OR 0.75, 95% CI 0.63-0.89), and regular ANC (OR 0.67, 95% CI 0.54-0.84) had lower risk for preterm birth. Apgar score, birthweight, and mode of delivery were significantly different between the pre-term group and the full-term group. Conclusion: Prevalence of preterm birth in Cipto Mangunkusumo Hospital was approximately 2.5 times higher compared to the national number. Several factors reducing preterm birth rate include singleton pregnancy, head presentation, and regular ANC.
Honey clinically stimulates granulation and epithelialization in chronic wounds: a report of two cases

Abstract

Various studies have been widely reintroduced honey as a wound dressing in chronic wound treatment. However, its utilization in Indonesia is still very limited. This study reported the effect of honey in stimulating the epithelialization and granulation on chronic wound treatment in our center. Two chronic wound cases, diabetic foot ulcer patient and gangrenous extravasation due to sodium bicarbonate, were daily treated with honey applications. After six-week evaluation, clinically the tissue granulation and epithelization were seen at the edge of the observed wounds and decreased the wound size significantly with no reported side effects. Honey clinically stimulates the epithelialization and granulation on chronic wound treatment.
