This study focusing on Design of Quality Management System ISO9001:2008 in PT. XYZ. ISO 9001:2008 is a quality for Quality Management System that have been been proven effective for various type of organization. The purpose of this study is to design the required documents by Quality Management System ISO 9001:2008 in PT. XYZ as paper manufacturing company. This study is qualitative data collection using interview and fields observation in PT.XYZ. The analysis assessed based on GAP Analysis by comparing the early situations to each clause found in ISO 9001:2008. After the gaps in company is known, the results can be used as the data to correct and improve then company standard and Quality Management System. After the Design of Quality Management System, GAP Analysis is performed again to see the gaps of the company final preparation for implementing ISO 9001:2008. The results of company GAP Analysis show that the company have sufficient preparation preparation for implementing and perform the Internal Audit.

This research was conducted to the determination of the location of depot and disaster relief delivery line by minimizing delivery distance from one location to another. Method used in this research is one of VRP which is Evolutionary Algorithm by using each village as chromosome. The object of this research is distance between villages within Pohjentrek and Gondang Wetan district. The result of this research shows that the optimum delivery distance of Pohjentrek District is 92.8 kilometers and the best location for the depot is in Legowok Village. The optimum delivery distances for Gondang Wetan District is 81 kilometers and the best location for the depot is in Gondangrejo Village.

IMM department is the business core of PT. X, it was noticed that there were many reject products passed to the next department and to the customer. Internal Block Belt data is data flowout to internal customer in 2015 which showed that reject flowout was still higher than the target (3%). Therefore, the company wants to identify the cause of flowout reject and what improvements can be applied to reduce it. The root cause of flowout reject is found out using fishbone diagram and priorities determination is using FMEA. The root cause of reject flowout is because of too much workload to be completed by the QC inspector. Suggestions given are the changes of form Inspection Report and additions of 1 QC inspector.

PT.Y is a company that produce cigarette. Currently PT.Y wants to revise the spare part receiving system that was applied. Spare part receiving process first start at main store, then will be delivered to sub store. The improvement that wants to be applied is to eliminate the main store, so that spare part can be delivered directly to warehouse sub store. By portraying the current process using value stream mapping, two waste could be eliminated if warehouse main store role is erased. Unnecessary transportation and over processing is the occurring waste in these process. The proposed counter measure is to change all store location for all material in the data base so that when PR was created the store location will automatically determine the real receiving warehouse. The proposed counter measure can shorten the processing time by 16.918,91 second per day. Counter measure can also decrease transportation fees by Rp.6.140,00 per day.

Existing problems at PT. FSCM Manufacturing Indonesia’s finished goods warehouse are the goods flow is not FIFO and insufficient capacity. Improvement will be conducted in both Plant II and Plant III PT. FSCM Manufacturing Indonesia. The current layout in both warehouse used block stacking method. The suggested design included change the storing method into pallet flow, change the layout, design a control board and work instruction. Pallet flow method will be applied by using roller conveyor. The design will make the goods flow FIFO and the PC Store can store all the goods.

This study was conducted to implement the KPI and design e-KPI system at PT. XYZ. KPIs are implemented using the basic four perspectives based on the Balanced Scorecard. The initial condition, the company has KPI for two divisions from six divisions and also the absence of a system of e-KPI. The result shows that six divisions have implemented KPI and e-KPI system has been designed at PT. XYZ. KPI will be assessed for performance appraisal on PT. XYZ for a period of time to come. e-KPI system provides the facility to the management to access KPI of division/department anytime and anywhere.

PT. X is a job-order plastic packaging industry using line flow in their floor. Currently, the company faces the imbalance line flow and a long transportation time. The goal of this final project is to balance a production line and reduce the transportation time. A proposed facitily Layout and balanced production line are implemented. The result shows Line Efficiency for powder compact products increase from 61.83% to 80.77%, for pot products decrease 80.14% to 72.85% and for eyeshadow compact products increase 74.22% to 81.86%. A proposed layout gives the better solution compare to existing layout in term of moment.

Production Efficiency (PEFF) is a KPI evaluation productivity that compares of inputs and outputs in total working hours. PT TMMIN can be more competitive and profitable by strengthen productivity management through implementation PEFF. Data for calculating PEFF consists of standard time, good product quantity and actual working hour. This thesis aims to ensure the accuracy of the data of actual working hour to complete 100% step 3 on productivity management. Data of actualworking hour has been through the process of checking by HRD and will be integrated by the system for calculating PEFF. Integrated system will use PEFF Dashboard that design is made with concept of a single sourve to ensure the data used is the same for each division

Abstract: This study was done to control the delivery system components of Plant 1 to Plant 2 for the machine assembly is not off. The main problem faced by PT. FSCM Manufacturing Indonesia is not their delivery schedules so that components are submitted can not be controlled. The next problem is the production Planning is administered orally and the misscomunication between foremen and operators that cause components to ASF (After Surface Finishing) empty. Improvement award is to create the delivery schedule number and weight of components with components that have been determined. Make a design planning card to make the order type of components to be processed so that the planning of production to be sustainable and ASF components will not be empty. To draft a new component card for improving the quality of the product components

Abstract: Performance of a company depends a lot on its employee’s performance. That causes many companies try so many ways to improve their employee’s quality and performance. XYZ company has a program that supports performance improvement named Continuous Improvement. In this program, there is a rating system which is applied to the machines the company have. These ratings will be reported to the machine operator. Machine that achieves its target will be declared as the winner and every employee that operates with that machine will be rewarded with bonus. However this system has a lot of problems especially in processing data. The occurring problem is, it takes a long time to process data and there are errors when employees are inputting the data. These problems become the background of this research. In this research, a proposal of a project that could decrease the time required to process the data and errors that occurs so that data’s consistency can improve will be given. The result of this project shows that the time required to process the data is reduced by 70% and human error can be avoided by making warnings and barricades.
