Abstract

Background: One of the main therapies for cancer is chemotherapy, however the administration is often accompanied by a wide range of Adverse Drug Reactions. Gemcitabine-carboplatin is one of the chemotherapy regimens frequently used in cancer treatment. Purpose: This study aims to determine the incidence, severity, and risk factors associated with the occurrence of hepatotoxicity after gemcitabine-carboplatin use. Methods: This study uses prospective observational research methods, including medical records and in-person interviews, with laboratory data (SGPT and SGOT) as the primary outcome. The sampling technique was purposive—the severity of hepatotoxicity was classified using the CTCAE instrument. The data obtained were then analyzed using bivariate and multivariate methods. Results: The incidence of ADRs was 16.66% in SGPT, with higher rates in grades 1 (14.44%) and 3 (2.22%). While the incidence of ADRs on SGOT increased by 22.22% across severity grades: 1 (15.56%), 2 (2.22%), and 3 (4.44%). Comorbidity is a significant risk factor for ADR incidence (p-value < 0.05). Conclusion: Patients with comorbidities had 8.076 times the tendency to experience ADRs as increased SGPT and 6.729 times the tendency to experience ADRs as increased SGOT. Results: The incidence of ADRs was 16.66% in SGPT increased with severity grade 1 (14.44%) and grade 3 (2.22%). While the incidence of ADRs on SGOT increased by 22.22% with severity grade 1 (15.56%), grade 2 (2.22%), and grade 3 (4.44%). Abstract

Background: Menstrual problems affect an estimated 500 million people worldwide, with reported prevalence among schoolgirls and female university students ranging from 23% to 76.60%. In Indonesia, the prevalence has been reported as 11.70%. Academic pressure may contribute to stress and insomnia, which in turn may increase the risk of menstrual irregularities. Purpose: Analyze the direction and strength of the relationships between stress, insomnia, and menstrual irregularities among midwifery students at the Faculty of Medicine, Airlangga University. Methods: This observational analytic study used a cross-sectional design. Data were collected from 109 respondents selected through simple random sampling using validated and reliable questionnaires. Data normality was assessed using the Kolmogorov-Smirnov test, and associations between variables were examined using Spearman’s rank correlation test. Results: Among the respondents, 34% reported regular menstrual cycles, while 66% experienced menstrual irregularities. The normality test indicated that all variables were not normally distributed (menstrual irregularity p<0.01; stress level p=0.01; insomnia level p=0.01). Spearman’s rank correlation analysis showed significant positive correlations between stress and menstrual irregularities (p=0.00; r=0.48) and between insomnia and menstrual irregularities (p=0.00; r=0.32). These findings indicate that higher levels of stress and insomnia were associated with greater menstrual irregularities. The association was moderate for stress and weak for insomnia. Conclusion: Stress and insomnia were significantly associated with menstrual irregularities among midwifery students. These findings highlight the importance of stress reduction, sleep management, and regular reproductive health screening in this population.

Abstract

Background: Uncontrolled hypertension remains a major public health problem and increases the risk of cardiovascular complications. Limited evidence is available regarding behavioral and psychological determinants of uncontrolled hypertension in secondary healthcare settings in Indonesia. Purpose: This study aimed to identify factors associated with uncontrolled hypertension among hypertensive patients. Methods: A cross-sectional study was conducted among hypertensive patients treated during January–December 2023. A total of 202 patients were selected from medical records using purposive sampling. Variables examined included age, smoking behavior, psychosocial stress, and anxiety status. Anxiety was identified based on standardized psychological assessments recorded in medical records. Data were analyzed using chi-square and logistic regression tests. Results: Age, smoking behavior, and anxiety were significantly associated with uncontrolled hypertension. Smoking habits (p=0.03; OR=1.55) and anxiety (p=0.001; OR=3.62) increased the likelihood of uncontrolled blood pressure. Age was also significantly associated with uncontrolled hypertension (p=0.04; OR=0.47). Conclusion: Psychological factors, particularly anxiety and smoking behavior, play an important role in hypertension control. Older patients may require closer blood pressure monitoring and regular follow-up to improve hypertension management. Integrating mental health support alongside smoking-related interventions may help improve blood pressure control in clinical settings.
