Abstract

The treatment effectiveness of hypertension could be influenced by patients’ characteristics and patients’ adherence with medication. Besides reaching the goal of blood pressure decrease after the treatment, their quality of life has become the main concern regarding effectiveness of hypertension treatment. This study aimed to explore the hypertension patients’ adherence and quality of life. In addition, it was studied which factors associated with adherence and quality of life in hypertension patients treated withantihypertensive at Gunung Jati Hospital, Cirebon. We recruited 85 adult hypertension patients who were treated with antihypertensive agents for at least 6 months. The patients’ adherence was measured by Medication Adherence Report Scale and the patients’ quality of life was measured by Indonesian version of Short Form-36 questionnaire. The patients’ adherence was found as 24.03 (SD: 1.98) and there were no significant differences of patients’ adherence using monotherapy and combination therapy. Thepatients’ characteristics such as, age, gender and education level could not predict patients’ adherence (p>0.05). The average of Physical Component Summary (PCS) and Mental Component Summary (MCS) were 43.35 (SD: 9.4) and 52.13 (SD:5.59). Age and gender may predict PCS, however, education and comorbidity may predict MCS (p<0.05). Hypertension patients’ adherence in Gunung Jati hospital is good. The PCS and MCS scores in this study are comparable to the other previous studies. The patients’characteristic could not be the predictor of patients’ adherence.Keywords: Adherence, hypertension, quality of life, MARS, SF-36Kepatuhan dan Kualitas Hidup Pasien Hipertensi Di Rumah Sakit Gunung Jati CirebonEfektivitas terapi pasien hipertensi dapat dipengaruhi oleh beberapa faktor karakteristik pasien dan kepatuhan pasien. Selain menurunkan tekanan darah, luaran lain dari terapi hipertensi adalah meningkatkan kualitas hidup pasien. Tujuan penelitian ini adalah untuk mengetahui kepatuhan dan kualitas hidup pasien hipertensi serta memahami faktor prediksi kepatuhan dan kualitas hidup pasien. Sejumlah 85 pasien hipertensi yang telah mengonsumsi obat hipertensi minimal 6 bulan berpartisipasi dalam penelitian ini. Kepatuhan pasien diukur dengan kuesioner Medication Adherence Report Scale versi Indonesia dan kualitas hidup pasien diukur dengan kuesioner Short Formulary-36 versi Indonesia. Kepatuhan pasien hipertensi di RS Gunung Jati Cirebon adalah 24,03 (SD:1,98) dan tidak terdapat perbedaan signifikan antara kepatuhan pasien hipertensi yang menggunakan monoterapi dan kombinasi terapi. Karakteristik pasien seperti usia, jenis kelamin, dan tingkat pendidikan bukan merupakan prediktor kepatuhan (p>0,05). Rata-rata Physical Component Summary (PCS) dan Mental Component Summary (MCS) adalah 43,35 (SD: 9,4) dan 52,13 (SD:5,59). Usia dan jenis kelamin merupakan prediktor PCS dan pendidikan serta komorbiditas merupakan prediktor MCS (p<0,05). Kepatuhan pasien hipertensi di RS Gunung Jati Cirebon cukup baik. Komponen PCS dan MCS cukup baik dibandingkan dengan penelitian lain. Abstract

Peningkatan kepatuhan minum obat warfarin pada pasien sindrom koroner akut (SKA) dan fibrilasi atrium (FA) salah satunya dapat diberikan melalui edukasi dengan pemberian leaflet. Penelitian ini bertujuan untuk mengetahui pengaruh pemberian edukasi dengan leaflet oleh apoteker terhadap kepatuhan minum obat warfarin pada pasien SKA dan FA. Penelitian ini menggunakan metode uji sebelum dan sesudah menggunakan pembanding. Pengambilan data dilakukan secara prospektif selama 8 minggu periode Juni–Juli 2014 pada pasien rawat jalan SKA dan FA di poliklinik RS PKU Muhammadiyah Yogyakarta. Data diambil melalui rekam medik pasien dan kuesioner Morisky Medication Adherence Scale (MMAS) yang diberikan pada pasien. Analisis statistik menggunakan uji Wilcoxon. Hasil penelitian pada kelompok pembanding “ uji sebelum dan sesudah” sebesar p=0,194 dan kelompok perlakuansebesar p=0,058. Berdasarkan hasil tersebut menunjukkan bahwa tidak ada perbedaan signifikan (p>0,05) setelah diberikan edukasi dengan leaflet. Pemberian edukasi dengan leaflet tidak memengaruhi kepatuhan minum obat warfarin pada pasien SKA dan FA di RS PKU Muhammadiyah Yogyakarta.Kata kunci: Fibrilasi atrium (FA), kepatuhan, sindrom koroner akut (SKA), warfarinThe Effect of Medicinal Education on Adherence Taking Warfarin in Acute Coronary Syndrome (ACS) and Atrial Fibrilation (AF) Patients at PKU Muhammadiyah Yogyakarta HospitalIn order to improve warfarin medication adherence in patient with Acute Coronary Syndrome (ACS) and Atrial Fibrillation (AF), giving education with leaflet administration is one of the solutions. This study was aim to know the impact of pharmacist education with using prepared leaflet on the adherence to warfarin in ACS and AF patients. This study used pre test and post test with control group design. Data were collected prospectively during 8 weeks in June–July 2014 at the ambulatory ACS and AF patients at PKU Muhammadiyah Yogyakarta hospital, Indonesia. Data were collected by medical record and the questionnaire using Morisky Medication Adherence Scale (MMAS). Wilcoxon test was used for statistical analysis. The results shows pre test and post test value in the control group was p=0.194 and pre and post test value in the test group was p=0.058. There was no significant difference (p>0.05) after giving education with leaflet. The education with leaflet had no effect to adherence in warfarin in ACS and AF patients at PKU Muhammadiyah Yogyakarta hospital.Keywords: Acute coronary syndrome, adherence, atrial fibrilation, warfarin

Abstract

Terapi target (targeted therapy) pada karsinoma kolorektal merupakan terapi alternatif yang diharapkan memberikan hasil yang lebih baik diantaranya dengan pemberian metabolit aktif vitamin D, yaitu calcitriol dan inhibitor Phosphatidyl Inositol 3 Kinase (PI3K). Penelitian ini bertujuan untuk mengetahui korelasi imunoekspresi VDR dan PI3K terhadap derajat diferensiasi adenokarsinoma kolorektal. Penelitian ini menggunakan desain potong lintang dengan analisis kategorik dari 30 blok parafin adenokarsinoma kolorektal (kelompok low grade dan high grade masing-masing 15 kasus) dari Departemen Patologi Anatomi Fakultas Kedokteran Universitas Padjadjaran/RSUP Hasan Sadikin Bandung. Analisis Lambda menunjukkan bahwa imunoekspresi VDR dan PI3K masing-masing memengaruhi diferensiasi adenokarsinoma kolorektal (p=0,029, p=0,0225, R= 0,533). Berdasarkan nilai p, PI3K lebih kuat daripada VDR dalam memengaruhi derajat diferensiasi. Analisis Spearman menunjukkan imunoekspresi VDR dan PI3K tidak terbukti berkorelasi dengan diferensiasi (p=0,186, p>0,05, R: 0,248) secara bersamaan/simultan. Imunoekspresi VDR dan PI3K berkorelasi positif dengan kelompok low grade. Disimpulkan bahwa VDR tidak menggunakan jalur MAPK bersama-sama dengan PI3K dalam memengaruhi diferensiasi adenokarsinoma kolorektal.Kata kunci: Adenokarsinoma kolorektal, diferensiasi, imunoekspresi PI3K, imunoekspresi VDRVitamin D Receptor (VDR) and Phosphatidylinositol 3-Kinase (PI3K) Independently Affected Colorectal Adenocarcinoma Differentiation Targeted therapy in carcinoma colorectal such as is as calcitriol and Phosphatidyl Inositol 3 Kinase (PI3K) is an alternative therapy for better treatment outcome. The aim of this study was to determine the correlation between VDR and PI3K immunoexpression with the degree of differentiation (grading) in colorectal adenocarcinoma. Cross-sectional, categorical analysis of 30 paraffin blocks of colorectal adenocarcinoma (low and high grade group, 15 cases each) obtained from Department of Pathology Faculty of Medicine, Universitas Padjadjaran/Dr Hasan Sadikin General Hospital Bandung. Analysis of Lambda showed that immunoexpression of VDR and PI3K affected differentiation of colorectal adenocarcinoma (p=0.029, p=0.0225, R=0.533). Based on p value, PI3k is stronger than VDR in influencing the degree of differentiation. Spearman analysis showed that simultaneous immunoexpression of VDR and PI3K was not shown to correlate with differentiation (p= 0186, p<0.05, R: 0.248). Positive immunoexpression of VDR and PI3K correlates with low-grade group. It can be concludes that VDR does not use the MAPK pathway together with PI3K to affect differentiation of colorectal adenocarcinoma.Keywords: Colorectal adenocarcinoma, differentiation, PI3K immunoexpression, VDRImmunoexpression

Abstract

Penyakit metabolik merupakan penyakit yang berkaitan dengan peningkatan usia seperti hipertensi, diabetes melitus, dislipidemia, dan obesitas. Penyakit-penyakit tersebut ditangani dengan terapi obat yang sifatnya polifarmasi sehingga dapat menyebabkan risiko terjadi interaksi obat-obat. Penelitian ini dilakukan secara restropektif dengan menggunakan rekam medis pasien usia lanjut dengan penyakit metabolik yang di rawat jalan di Rumah Sakit Umum Pusat Haji Adam Malik Medan. Sebanyak 328 sampel yang termasuk ke dalam kriteria inklusi diperoleh data bahwa jumlah interaksi obat-obat yang terjadi cukup tinggi sebesar 78,96%. Pola mekanisme yang terbanyak adalah farmakokinetik (63,6%) dengan tingkat keparahan yang terbanyak adalah moderat (69,8%). Penelitian ini menunjukan adanya korelasi antara jumlah interaksi dengan jumlah obat (r= 0,728; p= 0,0001), dan jumlah interaksi dengan jumlah diagnosis (r= 0,264; p= 0,0001).Kata kunci: Interaksi obat, penyakit metabolik, polifarmasiPolipharmacy and Drug Interactions in Elderly Patients with Metabolic DiseasesMetabolic disease is a disease that is associated with increasing age such as hypertension, diabetes mellitus, dyslipidemia and obesity. Such diseases are treated with polypharmacy therapy that can cause increased risk of drug interactions. This study was conducted using a restropective method using the medical records of elderly patients with metabolic diseases in outpatient unit of H. Adam Malik General Hospital Medan. A total of 328 samples were included in the inclusion criteria data showed that the numberof drug-drug interactions that occur quite high at 78.96%. The pattern of drug interaction mechanism that most frequently occur is pharmacokinetic (63.6%) and the highest severity was moderate (69.8%). This study shown a correlation between the number of interactions with a number of drugs (r= 0.728; p=0.0001), and the number of interactions with a number of diagnoses (r= 0.264; p= 0.0001).Keywords: Drug-interactions, metabolic-disease, polypharmacy

Abstract

Penggunaan antibiotik yang tidak rasional dapat menyebabkan peningkatan morbiditas dan mortalitas. Monitoring penggunaan antibiotik diperlukan dalam mendukung program pemerintah khususnya Dinas Kesehatan yang menyatakan penggunaan antibiotik untuk penyakit ISPA non-pneumonia adalah kurang dari 20%. Evaluasi penggunaan antibiotik ini menggunakan metode ATC/DDD dan DU 90%. Antibiotik yang digunakan untuk ISPA non-pneumonia adalah sebanyak 9 jenis dan antibiotik yang masuk dalam DU90% sebanyak 3 jenis yaitu amoksisilin 500 mg (2,723 DDD/1000 pasien-hari), siprofloksasin (0,378 DDD/1000 pasien-hari) dan sefadroksil (0,202 DDD/1000 pasien-hari). Analisis data secara kuantitatif menggunakan ATC/DDD menunjukkan bahwa antibiotik yang banyak digunakan adalah amoksisilin (500 mg) 2723 DDD/1000 pasien-hari dan yang paling sedikit yaitu amoksisilin (125 mg/5 ml) 1,5 DDD/1000 pasien-hari. Efek peresepan penggunaan antibiotik jangka pendek pada pelayanan pengobatan dasar dapat meningkatkan kejadian resistensi. Diperlukan studi kualitatif untuk mengetahui pola ketidakrasionalan penggunaan antibiotik di pusat pelayanan kesehatan masyarakat tersebut dan mengembangkan model intervensinya.Kata kunci: ATC/DDD, DU 90%, ISPA non-pneumonia antibiotikMonitoring the Use of Antibiotics by the ATC/DDD Method and DU 90%: Observational Studies in Community Health Service Centers in North Gorontalo DistrictIrrational use of antibiotics may lead to increase morbidity and mortality. The effects of short-term use of antibiotic prescribing in primary medical care could increase the resistance. Penelitian ini dilakukan untuk mengetahui pengaruh konseling oleh farmasis terhadap kepatuhan dan hubungan antara kepatuhan dan hasil terapi pasien hipertensi anggota program pengelolaan penyakit kronis (PROLANIS) di Klinik Mitra Husada Kabupaten Kendal. Penelitian dilakukan pada November 2013–Januari 2014 melalui desain eksperimen semu (control group design with pretest posttest). Sebanyak 55 pasien dikelompokkan secara random menjadi kelompok yang memperoleh intervensi konseling (28 pasien) dan kelompok tanpa intervensi atau kontrol (27 pasien). Subjek penelitian diikuti selama dua bulan untuk mengamati tingkat kepatuhan minum obat dengan kuesioner MMAS dan penurunan tekanan darah sebelum dan sesudah konseling. Konseling oleh farmasis menyebabkan tingkat kepatuhan minum obat berubah signifikan pada pasien hipertensi, pasien hipertensi dengan DM, maupun pasien hipertensi dengan penyakit lain (p=0,015; 0,025; 0,009). Tingkat kepatuhan pasien kelompok kontrol sebelum dan setelah penelitian diketahui tidak signifikan (p≥0,05). Pemberian konseling mampu menurunkan tekanan darah sistolik/diastolik sebesar 10,7/8,2 mmHg. Penurunan tekanan darah ini belum menunjukkan perbedaan yang bermakna jika dibandingkan dengan kelompok kontrol.Kata kunci: Hipertensi, kepatuhan minum obat, konseling farmasis, penurunan tekanan darahThe Influence of the Pharmacists Counseling on Patient Adherence and Hypertension Control on Patient of Prolanis at Mitra Husada ClinicsPatient adherence and intensive treatment in controlling blood pressure are important factors for achieving success therapy in hypertension. This study was conducted to determine the effect of pharmacist counseling on patient adherence and to determine the relationship of adherence and clinical outcomes of patients with hypertension who is joining chronic disease management program (PROLANIS) on Mitra Husada Clinics in Kendal.The study was conducted during November 2013–January 2014 using quasiexperimental design (pretest-posttest). Fifty five patients were randomly divided into the intervention group who received pharmacist counseling (28 patients) and control group (27 patients). The patients were followed for two months to observe the level of drug adherence using MMAS questionnaires and therapeutic outcomes (decreased blood pressure) before and after counseling. Pharmacist counseling caused a significant increase in the patient adherence on drug administration in intervention group (p<0.05) compared to control group. The reduction of systolic/diastolic blood pressure of hypertension patient in intervention group is higher than control group (SBP/DBP=10.7/8.2 mmHg). However, this reduction does not achieve a significant blood pressure reduction compared to control group.Keywords: Adherence of drug administration, blood pressure reduction, hypertension, pharmacist
