Abstract

Cylinder head is a critical component in internal combustion engines that operates under extreme thermal and mechanical conditions, making it highly susceptible to failure. One of the most common failure modes is cracking, which is primarily associated with overheating and thermal fatigue. However, existing studies often analyze failures descriptively without integrating causal relationships and quantitative risk assessment. This study aims to analyze the failure mechanism of a cylinder head in unit BMGS1740 using an Failure Mode and Effects Analysis (FMEA) approach. FMEA is applied to evaluate and prioritize risks based on Severity (S), Occurrence (O), and Detection (D), resulting in the Risk Priority Number (RPN). The analysis is based on operational data, inspection results, and thermal performance indicators. The results show that cylinder head failure is driven by a progressive thermal mechanism initiated by cooling system degradation, leading to overheating, thermal stress, and ultimately crack formation. The highest risk factors identified are radiator fouling, water pump failure, and thermal fatigue, each with RPN values ≥ 300, indicating their dominant contribution to failure. Medium-risk factors include thermostat malfunction, loose fan belt, and poor atomization, while overfueling is categorized as a low-risk contributing factor. The findings indicate that cooling system performance is the primary triggering factor, while thermal fatigue acts as the main damage propagation mechanism. Therefore, preventive strategies should focus on improving cooling system efficiency and optimizing combustion processes to minimize thermal loading. This integrated approach provides a more comprehensive framework for failure analysis and supports risk-based maintenance decision-making

Abstract

This research focuses on the design, fabrication, and performance evaluation of a cassava and banana cutting machine prototype aimed at improving the uniformity of slice thickness and the efficiency of the chip production process. The design stages were carried out systematically using AutoCAD and SolidWorks software to produce geometric configurations and cutting mechanisms that suit the characteristics of the tuber and fruit materials. Next, the virtually validated design was realized through a fabrication process into a functional prototype. Machine performance testing was carried out using cassava test material with an average diameter of 50 mm. Each test condition was repeated five times (n = 5) to ensure data repeatability. The performance parameters analyzed included production rate (throughput) in kg/hour, slice thickness in millimeters which was statistically analyzed using the mean and standard deviation values, and electrical power consumption during machine operation. The test results showed that the machine was capable of producing an average throughput of 50 ± 3 kg/hour with a slice thickness of 2.0 ± 0.15 mm and a coefficient of variation of 7.5%, which indicates a good level of slice uniformity. Power consumption was recorded at 3.1 kW under standard operating conditions. Compared to manual cutting methods, the use of this machine reduced cutting time per kilogram of material by approximately 60%. Based on these results, it can be concluded that the developed prototype is effective in improving process efficiency and sliced quality. Further research is recommended to optimize the material feed system and test the machine's performance on a wider variety of raw materials Keywords: Cutting machine; cassava; banana; slice uniformity; mechanical design

Abstract

The development of the manufacturing industry requires companies to improve the efficiency of the production process and maintain product quality stability. PT XYZ, a zipper manufacturer, faces a problem with a high Tape Jam Alarm ratio on its CM7 chain making machine, which has an impact on decreasing productivity. Based on three months of production data collection, it is known that the Tape Jam Alarm ratio reaches 8 times in every 1000 mD. This condition indicates an uncontrollable process and the need for an in-depth analysis of the causes of the disturbance. This study aims to identify the root cause of the Tape Jam Alarm and formulate corrective actions that can reduce the alarm frequency. The analysis method used is a Fishbone diagram to trace the causal factors originating from the aspects of the machine, material, method, and work environment. The results of the study indicate that the Tape Guide component on the CM7 machine has a gap that is too narrow and the surface is still rough, causing excessive friction and triggering tape jams. Based on these findings, corrective actions were taken by replacing the Tape Guide with a design that has a wider gap and a smoother surface. After the implementation of the improvements, the Tape Jam Alarm frequency was successfully reduced from 8 times to 2 times in every 1000 mD. Research on reducing tape jam alarms through modifications to the tape jam guide design on the machine is still limited. This research provides a contribution in the form of recommendations for repairing critical components on the CM7 engine and can be used as a reference in efforts to increase the efficiency and reliability of the production process at PT XYZ Keywords: Corrective actions; production; CM7

Abstract

The armrest cover on the chair is produced using the vacuum forming method and so far the line cutting and hole making process is done manually using a hand grinder, thus potentially causing quality inconsistencies and low productivity. The armrest cover made of ABS plastic has a hole dimension of 37 mm × 22 mm, a thickness of 3 mm, and a circumference of 119.27 mm, with a shear strength value of 67 MPa based on Proto XYZ data. Calculations show that the minimum force required for the punching process is 8.46 tons. This study aims to design and manufacture piercing dies for the armrest cover punching process to obtain precise cutting results and increase production output. The dies were made using a CNC milling machine with the help of Autodesk Fusion 360 software, then the dies were tested using a 15-ton capacity press machine. The test results showed that the resulting holes met the specified dimensional standards. The implementation of these piercing dies significantly increased production capacity, from 90 sets per shift in the manual process to 1,920 sets per shift using piercing dies. Thus, the use of piercing dies has proven effective in increasing the precision and efficiency of the plastic armrest cover manufacturing process.

Abstract

Improving machine productivity and effectiveness is a crucial factor in maintaining the competitiveness of manufacturing companies. One approach to improving equipment performance is Total Productive Maintenance (TPM), with Overall Equipment Effectiveness (OEE) as the primary indicator. This study aims to analyze the effectiveness of the Flow Mixer machine at PT ASI's Mold Shop Department based on its OEE value, which comprises availability, performance, and quality. It also identifies the dominant factors causing low machine effectiveness using the Six Big Losses approach and a fishbone diagram. The research data were obtained from production data, planned and unplanned downtime, the number of defective products, and machine operating time during September–November 2025. The calculation results show that the Flow Mixer machine's OEE values were 66% (September), 68% (October), and 68% (November), respectively, which are still below the world-class OEE standard of 85%. The main factors influencing the low OEE are breakdowns and process defect loss. Improvement proposals are based on the eight pillars of TPM, which focus on improving machine maintenance, operator skills, and standardizing work procedures. Implementation of the proposed improvements in December resulted in an increase in the OEE value to 72%. The results of this study demonstrate that consistent TPM implementation can improve the effectiveness of the Flow Mixer machine and the productivity of the production process.
