Abstract

Tumor pada jalan napas bisa menyebabkan masalah dan bisa juga tidak pada manajemen jalan napas, hal ini tergantung pada lokasi, ukuran, jenis, dan karakteristik tumor tersebut. Kesulitan dalam jalan napas akan sering dihadapi oleh setiap dokter anestesia. American Society of Anestesiologists (ASA) telah membuat algoritma manajemen jalan napas sulit untuk memfasilitasi dokter anestesia dalam menghadapi kondisi ini. Laporan kasus ini membahas tentang pasien wanita berusia 56 tahun dengan tumor hipofaring yang akan menjalani operasi biopsi tumor, kemudian terjadi perdarahan saat akan dilakukan intubasi hingga diperlukan tindakan trakeostomi darurat. Pasien akhirnya dapat diselamatkan dan kembali beraktifitas normal. Perencanaan yang matang dan pelaksanaan yang tepat selama perioperatif terutama berkaitan dengan manajemen jalan napas akan menghasilkan hasil yang baik bagi kesehatan dan keselamatan pasien. Kata kunci : American Society of Anestesiologists (ASA), kesulitan jalan napas, tumor hipofaring   Management of Difficult Airway in Patient with Hypopharyngeal Tumor  Tumors in the airway may or may not cause problems on the airway management. The magnitude of the problems depend on location, size, type, and characteristics of the tumor. Difficulties in the airway are frequently encountered by every anesthesiologist. ASA and DAS have made algorithm of difficult airway management to facilitate anesthesiologist facing these conditions. This case report discusses a 56 years old female patient with hypopharyngeal tumor undergoing a biopsy surgery. Profuse bleeding was encountered during intubation attempt indicating necessary for emergency tracheostomy. Eventually patients can be saved and returned to normal activity. Careful planning and proper execution of every perioperative aspects especially airway management during perioperative period will produce good results for the health and safety of the patients. Abstract

Latar Belakang. Teknik multimodal analgesia dengan menggunakan dua atau lebih obat analgesik yang berkerja pada dua atau lebih jalur nyeri yang berbeda merupakan rekomendasi teknik manajemen nyeri akut pasca seksio sesarea. Teknik yang paling sering dipakai adalah dengan menggunakan analgesik intravena diikuti dengan penggunaan analgesik oral. Penelitian ini bertujuan membandingkan efektifitas analgesik oral controlled release oksikodon 10 mg dan parasetamol 1000 mg dengan tramadol 50 mg dan parasetamol 1000 mg untuk mengatasi nyeri pascaseksio sesarea dalam 24 jam pertama. Metode. Penelitian ini menggunakan metode uji klinis prospektif acak tersamar ganda pada 58 pasien yang menjalani operasi seksio sesarea dengan anestesi spinal. Pasien dibagi menjadi dua kelompok sama besar untuk penanganan nyeri pascabedah. Kelompok oksikodon sebanyak 29 orang diberikan oksikodon controlled release 10 mg oral setiap 12 jam dan parasetamol 1000 mg oral setiap 8 jam sedangkan kelompok tramadol sebanyak 29 orang diberikan terapi tramadol 50 mg oral tiap 6 jam dan parasetamol 1000 mg oral tiap 8 jam. Penilaian nyeri pascabedah menggunakan Numerical Rating Scale (NRS) dalam posisi duduk, istirahat, menarik napas, dan interval waktu yang berbeda yaitu jam ke-1, 6, 12, 18, dan 24 pascapemberian obat pertama. Data yang terkumpul dianalisa lebih lanjut dengan secara statistik. Hasil. Oksikodon controlled release 10 mg dan parasetamol 1000 mg lebih efektif dibandingkan tramadol 50 mg dan parasetamol 1000 mg dalam mengatasi nyeri pascaseksio sesarea dengan NRS kelompok oksikodon lebih rendah secara signifikan dibandingkan kelompok tramadol pada tiap posisi pengukuran (P < 0,001). Kesimpulan. Oksikodon 10 mg CR oral ditambah dengan parasetamol 1000 mg lebih efektif untuk mengatasi nyeri pascaseksio sesarea dibandingkan dengan tramadol 50 mg oral ditambah dengan parasetamol 1000 mg.

Abstract

Background: Nasotracheal intubation is a widely used airway management method, especially in oral surgeries. Various devices were found to perform intubation techniques, such as video laryngoscopes. C-MAC® video laryngoscope enables better glottis visualization compared to the Machintosh conventional laryngoscope. C-MAC® demonstrates higher success rates of orotracheal intubation especially in difficult airway cases. However, this device has not been commonly used in nasotracheal intubation yet. Methods: A single blinded randomized clinical trial study of 86 subjects were done to compare the use of C-MAC® video laryngoscope and Macintosh conventional laryngoscope with reference to their success rates of intubation and duration of nasotracheal intubation in adult Malay patients. Patients with difficult airway, pregnancy, acute ischemic heart disease, heart failure, second- or third-degree block, uncontrolled hypertension, Guillen Barre syndrome, Myasthenia Gravis, and contraindications to nasotracheal intubation were excluded. Results: C-MAC® demonstrated a higher success rate at first attempt of intubation (RR 1,265, 95% CI (1,084-1,475)) and required a shorter duration of intubation (p value <0.001) compared to the Macintosh conventional laryngoscopes in adult Malay. Conclusion: In adult Malay patients, nasotracheal intubation is better performed with the C-MAC® video laryngoscope compared to the Macintosh conventional laryngoscope. High rate of successful first attempt and shorter duration of the process indicate more convenient and easier intubation.

Abstract

Latar belakang. Emergence delirium (ED) adalah suatu kondisi yang umum terjadi, pada pasien anak-anak yang menjalani pembedahan, dimana anak menjadi sangat agitasi, memberontak, dan sulit untuk diredakan serta berpotensi membahayakan dirinya sendiri maupun orang lain dengan insidensi di RSCM sebesar 39,7%. Etiologi, faktor resiko, dan patofisiologi diperkirakan multifaktorial. Berbagai upaya dan strategi telah diusahakan untuk mencegah kejadian tidak menyenangkan ini. Metode. Penelitian uji klinik acak tersamar ganda pada anak usia 1-8 tahun yang menjalani operasi mata dengan anestesia umum di OK Kirana FKUI-RSCM pada bulan Januari-Maret 2020. Sebanyak 108 subjek didapatkan dengan metode konsekutif yang dirandomisasi menjadi dua kelompok. Kelompok klonidin (n = 54) mendapat klonidin 2 mcg/kgbb bolus IV lambat saat induksi anestesia, sedangkan kontrol (n = 54) mendapat NaCl 0,9%. Kejadian ED, waktu pulih, waktu pindah, derajat nyeri, dan efek samping hipotensi dan bradikardia selama dan pascaoperasi dicatat. ED dinilai dengan Pediatric Anesthesia Emergence Delirium (PAED). Hasil. Kejadian ED pada kelompok klonidin sebesar 29,6% sedangkan kontrol 31,5% (IK 95% 0,481-2,475; p=0,835). Waktu pulih kelompok klonidin memiliki nilai rerata 6 menit dibandingkan kelompok kontrol selama 5 menit (p=0,998). Nyeri sedang dirasakan pada 3,7% kelompok klonidin berbanding 0% paqda kelompok kontrol. Hipotensi dialami pada 1 pasien di kelompok klonidin dan 1 pasien di kelompok kontrol, sedangkan bradikardia ditemukan pada 2 pasien di kelompok klonidin dan 3 pasien di kelompok kontrol Simpulan. Pemberian klonidin 2 mcg/kgbb bolus IV lambat di awal induksi tidak lebih efektif dibanding plasebo dalam mencegah kejadian ED pada pasien anak yang menjalani operasi mata

Abstract

Latar Belakang: Dua puluh persen dari operasi tumor tulang membutuhkan transfusi darah packed red cell (PRC) intraoperatif, dengan volume transfusi rata-rata 1200 ml. Kekurangan dalam jumlah PRC yang disediakan dapat menimbulkan perburukan kondisi pasien, sedangkan kelebihan permintaan darah dapat menimbulkan kerugian biaya.Tujuan: Penelitian ini bertujuan membuat model prediksi jumlah kebutuhan transfusi PRC pada operasi tumor tulang berdasarkan faktor-faktor letak, ukuran, karakteristik keganasan tumor, nilai Hb prabedah dan nilai ASA prabedah. Metode: Penelitian kohort retrospektif ini dilakukan pada pasien dewasa, ASA 1-3 yang menjalani pembedahan tumor tulang tahun 2015- 2018 dan setelah mendapat ijin dari Komite Etik Penelitian Kesehatan FKUI RSUPN Dr. Ciptomangunkusumo. Pasien dengan tindakan core biopsy dan data rekam medis yang tidak lengkap dikeluarkan dari penelitian. Data lokasi, ukuran, dan karakteristik malignansi tumor, konsentrasi Hb preoperatif serta jumlah PRC yang ditransfusikan dicatata dan dianalisis. Analisis bivariat dan multivariat dilakukan dengan menggunakan statistical package for the social sciences (SPSS) versi 21.Hasil: Analisis dilakukan pada 82 data yang didapat dari rekam medis. Uji bivariat menunjukkan letak tumor, ukuran tumor, karakteristik keganasan tumor, nilai Hb prabedah dan nilai ASA prabedah memiliki hubungan bermakna terhadap kebutuhan transfusi PRC perioperatif. Analisis multivariat regresi linier menunjukan hanya letak tumor dan nilai Hb prabedah yang dapat memprediksi jumlah kebutuhan transfusi PRC perioperatif pada operasi bedah tulang. Kesimpulan: Letak tumor dan nilai Hb dapat memprediksi memprediksi jumlah kebutuhan transfusi PRC perioperatif pada operasi bedah tulang.

Abstract

Thoracic surgery in patients with severe pulmonary disease poses significant anesthetic challenges. General anesthesia with positive pressure ventilation may exacerbate ventilation–perfusion mismatch and worsen air leakage in the presence of a bronchopleural fistula. Regional anesthesia techniques that preserve spontaneous ventilation may offer a safer alternative in selected high-risk patients. We report a case of a 68-year-old male with tuberculosis-related right-sided pyopneumothorax complicated by bronchopleural fistula, severe chronic obstructive pulmonary disease with a restrictive component, atrial fibrillation with normal ventricular response, and hepatic insufficiency (ASA physical status III), who was scheduled for an open pleural window procedure. Given the high risk of general anesthesia and mechanical ventilation, ultrasound-guided unilateral thoracic paravertebral block (TPVB) was selected as the primary anesthetic technique, combined with dexmedetomidine sedation. The block was performed at T5–T7 using ropivacaine, achieving adequate dermatomal anesthesia. Sedation was titrated to preserve spontaneous ventilation and patient cooperation. The procedure was completed without conversion to general anesthesia. The patient remained hemodynamically stable, with preserved spontaneous breathing and adequate oxygenation throughout the surgery. No vasopressors, rescue opioids, or airway interventions were required. Postoperative pain was minimal, and no block-related or respiratory complications occurred. Thoracic paravertebral block combined with dexmedetomidine sedation provided effective anesthesia and favorable perioperative stability for open pleural window surgery in a high-risk pulmonary patient. This case highlights the potential role of regional anesthesia-based strategies as an alternative to general anesthesia in carefully selected thoracic surgical patients where preservation of spontaneous ventilation is desirable.

Abstract

Introduction: Radiofrequency ablation (RFA) is an established interventional treatment for trigeminal neuralgia refractory to pharmacotherapy. Conventional protocols often escalate the temperature stepwise to 70–80°C; however, higher temperatures increase the risk of sensory complications. Whether selected patients can achieve pain control without escalation above 60°C remains clinically relevant. Case Description: We report two patients with idiopathic trigeminal neuralgia of the mandibular (V3) division treated using a response-guided, low-temperature, fluoroscopy-guided RFA strategy. A 58-year-old man (baseline Numeric Rating Scale [NRS] 8) received two lesions at 50°C for 60 seconds each; a 63-year-old woman (baseline NRS 10) received two lesions at 60°C for 60 seconds each. Both had persistent pain despite paracetamol and pregabalin. Needle placement through the foramen ovale toward Meckel's cave was confirmed fluoroscopically, with sensory and motor testing. Both patients achieved complete pain relief immediately after the procedure (NRS 0), sustained at the three-month follow-up, without corneal complications, dysesthesia, motor deficits, or other adverse events. Conclusion: Response-guided low-temperature RFA without escalation above 60°C may provide effective short-term pain relief in selected patients with idiopathic trigeminal neuralgia while minimizing unnecessary thermal exposure to healthy tissues. Prospective studies are needed to define the optimal patient selection, temperature, and long-term durability.
