Abstract

Traditional fishermen face a high risk of skin health disorders due to extreme exposure to ultraviolet radiation and sea salinity. This study aims to capture the occupational safety culture through the practice of Personal Protective Equipment (PPE) usage and the prevalence of skin disorders among traditional fishermen in Tanjung Balai Asahan. The research method used was quantitative with a cross-sectional design. A total of 110 fishermen were selected as samples using the total sampling technique. The results showed that PPE usage practices were still low and partial, with 54.5% of fishermen only wearing two types of PPE while working. The majority of fishermen prioritized the use of boots (68.2%) and hats (62.7%), but ignored long-sleeved clothing (54.5% did not use) and gloves (52.7% did not use). The prevalence of skin disorders was found to be very significant at 64.5%, with the dominant types being dermatitis (32.7%), followed by scabies (18.2%) and tinea corporis (13.6%). The conclusion of this study indicates a correlation between low comprehensive PPE compliance and high skin morbidity rates. Occupational safety education interventions focusing on changes in risk perception and improvement of personal hygiene are needed to reduce morbidity rates in the fishing community. AbstrakNelayan tradisional menghadapi risiko tinggi gangguan kesehatan kulit akibat pajanan radiasi ultraviolet dan salinitas air laut yang ekstrem. Penelitian ini bertujuan untuk memotret budaya keselamatan kerja melalui praktik penggunaan Alat Pelindung Diri (APD) dan prevalensi gangguan kulit pada nelayan tradisional di Tanjung Balai Asahan. Metode penelitian yang digunakan adalah kuantitatif dengan desain potong lintang (cross-sectional). Sampel penelitian sebanyak 110 nelayan diambil menggunakan teknik total sampling. Hasil penelitian menunjukkan bahwa praktik penggunaan APD masih rendah dan bersifat parsial, di mana 54,5% nelayan hanya menggunakan dua jenis APD selama bekerja. Mayoritas nelayan memprioritaskan penggunaan sepatu boot (68,2%) dan topi (62,7%), namun mengabaikan pakaian lengan panjang (54,5% tidak menggunakan) dan sarung tangan (52,7% tidak menggunakan). Prevalensi gangguan kulit ditemukan sangat signifikan sebesar 64,5%, dengan jenis gangguan dominan berupa dermatitis (32,7%), diikuti skabies (18,2%) dan tinea corporis (13,6%). Kesimpulan penelitian ini menunjukkan adanya keterkaitan antara rendahnya kepatuhan APD yang komprehensif dengan tingginya angka kesakitan kulit. Diperlukan intervensi edukasi keselamatan kerja yang berfokus pada perubahan persepsi risiko dan peningkatan personal hygiene untuk menekan angka morbiditas pada komunitas nelayan.

Abstract

Technology adaption for healthcare services generally involves the implementation of electronic medical records. Management ini pharmaceutical facilities using Phyton can be achieved by building a CLI (Command Line Interface) or GUI-based data management systems. This system tracks patient history, diagnoses, medication lists, and dosage analysis for vertigo. One subsystem of a healthcare facility’s information system is the electronic medical record. This subsystem is connected to other subsystems within the healthcare facility. With the issuance of Minister of Health Regulation No. 24 of 2022 concering Medical Records, a medical records is defined as a document containing patient identity data, examinations, treatments, procedures, and other services provided to patients. This regulation replaces Ministers of Health Regulation No. 269 of 2008 concerning Medical Records. MENKES Regulation No. Law No. 24 of 2022 explains the purpose of electronics medical records : to improve the quality of healthcare services, provide legal certainty in the implementation and management of medical records, ensure the security, confidentiality, integrity, and availability of medical record data, and realize the implementation and management of digitally-based and integrated medical records. Under these conditions, the Minister of Health stipulates that service facilities are required to implement electronics medical records in their healthcare services. According to Rano Indriadi, an intellectual, an Electronic Medical Record (EMR) is defined as a medical record stored electronically, containing personal data, demographic data, clinical/medical data, and various clinicals events from the beginning of the service process to the end froam various data sources. Abstract

Access to healthcare services remains a major challenge in achieving equitable health outcomes in Indonesia, despite the implementation of the National Health Insurance (JKN) program. This study aimed to analyze the barriers to healthcare access experienced by communities as a basis for formulating responsive healthcare service policies. A qualitative descriptive approach was employed, involving six informants selected purposively from JKN participants who had experience accessing healthcare services. Data were collected through in-depth semi-structured interviews and analyzed using data reduction, data display, and conclusion-drawing techniques. Source triangulation was applied to ensure data validity. The findings revealed four major barriers to healthcare access: geographical, economic, administrative, and health literacy barriers. Geographical and transportation issues, including long distances to healthcare facilities, inadequate road conditions, limited transportation options, and mobility constraints among older adults, emerged as the most prominent challenges. Economic burdens related to transportation costs, complex referral procedures, lengthy waiting times, and limited understanding of healthcare procedures further hindered healthcare utilization. These interconnected barriers contribute to persistent inequalities in healthcare access. The study concludes that healthcare policies should be context-specific and evidence-based, focusing on improving transportation infrastructure, simplifying administrative procedures, strengthening community-based health education, and providing greater support for vulnerable populations. Such efforts are essential to promote equitable and accessible healthcare services for all communities. AbstrakAkses terhadap layanan kesehatan masih menjadi tantangan utama dalam mewujudkan pemerataan derajat kesehatan masyarakat di Indonesia meskipun Program Jaminan Kesehatan Nasional (JKN) telah diterapkan. Penelitian ini bertujuan untuk menganalisis kendala akses layanan kesehatan yang dialami masyarakat sebagai dasar perumusan kebijakan pelayanan kesehatan yang responsif. Penelitian menggunakan pendekatan kualitatif dengan desain deskriptif. Informan penelitian berjumlah enam orang yang dipilih secara purposive, yaitu masyarakat peserta JKN yang pernah mengakses layanan kesehatan. Data dikumpulkan melalui wawancara mendalam menggunakan pedoman semi-terstruktur dan dianalisis melalui tahapan reduksi data, penyajian data, serta penarikan kesimpulan. Keabsahan data dijamin melalui triangulasi sumber. Hasil penelitian menunjukkan adanya empat kendala utama dalam akses layanan kesehatan, yaitu kendala geografis, ekonomi, administratif, dan literasi kesehatan. Kendala geografis dan transportasi berupa jarak fasilitas kesehatan yang jauh, kondisi jalan yang kurang memadai, keterbatasan sarana transportasi, serta keterbatasan mobilitas pada kelompok lansia menjadi hambatan yang paling dominan. Selain itu, biaya transportasi, prosedur administrasi yang kompleks, antrean pelayanan yang panjang, serta rendahnya pemahaman masyarakat mengenai prosedur dan hak pelayanan kesehatan turut memengaruhi pemanfaatan layanan kesehatan. Berbagai kendala tersebut saling berkaitan dan memperkuat ketimpangan akses layanan kesehatan. Oleh karena itu, diperlukan kebijakan yang kontekstual dan berbasis kebutuhan masyarakat melalui peningkatan infrastruktur transportasi, penyederhanaan prosedur pelayanan, penguatan edukasi kesehatan berbasis komunitas, serta perhatian khusus terhadap kelompok rentan. Upaya tersebut penting untuk mewujudkan pelayanan kesehatan yang lebih adil, merata, dan mudah diakses oleh seluruh masyarakat.
