Abstract

ntroduction: Traditional postpartum care practices remain an integral aspect of maternal and child health in indigenous communities. Mararang tradition is a postpartum care tradition that continues to be widely practiced among Batak postpartum mothers, as it is believed to accelerate maternal and infant recovery through heat therapy. . This study aims to examine the historical evolution of the mararang tradition, its cultural significance, and its implications for maternal and infant health. Methods: A qualitative phenomenological approach was employed to explore the experiences of postpartum mothers practicing the mararang tradition. Data were collected through in-depth interviews with postpartum mothers, their husbands, traditional leaders, and healthcare providers in North Sumatra Province. Source triangulation, methodological triangulation, and data analysis were applied in this research. Thematic and content analyses were conducted to explore key dimensions of health beliefs, communication, and the transformation of the mararang tradition over time. Results: Participants ascribed thermal balance, comfort, and social support to mararang, while acknowledging potential risks from heat and smoke. Communication between families and midwives produced adaptive forms (e.g., moderated heat, improved ventilation, time?limited sessions), balancing cultural continuity with safety. We synthesise a conceptual model showing how explanatory beliefs, family authority, and professional guidance co?produce negotiated care. Conclusion: Mararang persists through culturally safe adaptations facilitated by respectful dialogue; practical safeguards (distance from heat source, ventilation, exclusion post?caesarean) can be embedded within routine postpartum care. Programmes should integrate culturally anchored counselling and risk?mitigation protocols, enabling context?sensitive maternal and newborn care.

Abstract

Introduction: In pluralistic societies like Indonesia, religious moderation is a critical public health and social cohesion imperative. Mosques and their imams serve as pivotal institutions and leaders in shaping community attitudes. This study explores how mosque imams in the Muslim-minority regions of Dairy and West Pakpak, North Sumatra, leverage local wisdom to foster religious moderation and social harmony. Objective: This research aims to analyze the practices and impact of mosque imams in promoting religious moderation by integrating Islamic teachings with indigenous local values, specifically the Sikata One (unity and solidarity) and Marga Silima (balance and harmony) principles of the Pakpak community. Methods: A qualitative study was conducted in Dairy and West Pakpak Regencies. Data were collected through in-depth interviews with mosque imams and administrators (n=40) and supplemented by content analysis of relevant documents, including local manuscripts and literature on religious moderation. A thematic analysis was employed to identify key patterns in the imams' strategies for integrating local wisdom with religious teachings. Results: The findings reveal a high prevalence of moderate attitudes among community members influenced by the imams. Key results include: 70% acceptance of friends from different religions; 75% rejection of coercing others to conform to one's religious will; and 90% comfort in sitting next to individuals of different faiths at events. Thematic analysis identified that imams actively incorporate the principles of Sikata One and Marga Silima into sermons and community guidance, framing religious coexistence as an extension of traditional cultural values. Conclusion: Mosque imams play an instrumental role as agents of religious moderation by effectively syncretizing Islamic doctrine with deeply held local wisdom. This culturally-grounded approach, championed by religious leaders, is a significant factor in the sustained interfaith harmony observed in these regions. The study underscores the importance of leveraging cultural assets and religious leadership in public health and policy initiatives aimed at building resilient, tolerant, and cohesive societies. Abstract

Stunting remains a critical public health concern in Indonesia, contributing toelevated morbidity rates and undermining the quality of human capital. While primary healthcare services are central to preventive efforts, their direct association with stunting at the national scale remains inadequately explored. This study investigates the relationship between primary healthcare utilization and stunting incidence among children aged 0-59 months in Indonesia, using data from the 2023 Indonesia Health Survey (SKI). A cross-sectional analytical design was employed, involving 79,141 children after data cleaning. Nutritional status was measured via the height-for-age (H/A) indicator based on WHO Z-scores. Independent variables included child's sex, maternal education level, maternal employment status, residential area, availability of primary healthcare facilities, and health insurance ownership. Statistical analysis encompassed univariate, bivariate (chi-square test), and multivariate (binary logistic regression) methods. Significant associations were found for sex, maternal education, place of residence, and availability of healthcare facilities (p < 0.05), while maternal employment and health insurance showed no significant relationship. Multivariate analysis identified maternal education as the dominant determinant (p = 0.023; Exp(B) = 1.040; 95% CI: 1.006-1.077). Children of mothers with lower educational attainment faceda heightened risk of stunting compared to those whose mothers had higher education. Utilization of primary healthcare services was associated with stunting among children, particularly through maternal education as a key determinant. Findings underscore the importance of strengthening family-based interventions and enhancing maternal health literacy within the framework of primary healthcare to address stunting more effectively Keywords: Stunting, Children, Primary healthcare services, Indonesia

Abstract

Backgrounds: Indonesia is the second-largest cigarette market in the world, with an increased smoking rate among young people aged 10-18 years. The latest data show that smoking is increasingly common in youth. Tobacco control will reduce public demand for tobacco products, especially if the supply of tobacco can be limited. Methods: This study, conducted in Tebing Tinggi City, North Sumatra Province, aimed to collect data on the density of cigarette sellers around schools and their impact on students' smoking behavior. The research involved 1914 students from 10 schools, examining factors such as smoking laws, age at first smoking, daily cigarette consumption, location, and advertisements. The study used a cross-sectional design, GPS coordinates, QGIS 3.0 analysis, and in-depth interviews with a Tebing Tinggi City Health Office representative. The findings will be analyzed using the JASP computer program. Results: The study shows that 8.2% of adolescents in Tebing Tinggi City have never smoked, with mixed views on the smoking law. The majority do not smoke, with varying smoking rates and exposure to cigarette advertisements. The study also found a high density of cigarette sellers in junior and senior high schools, indicating a lack of awareness about smoking. Conclusions: Most adolescents in Kota Tebing Tinggi do not smoke, with mixed views on the smoking law. They are exposed to cigarette advertisements in school and home environments, with vendors providing easy access. Increased awareness and effective law enforcement are needed to control cigarette advertising and sales.
