Abstract

ABSTRACT The radiology installation is a medical support unit with a high risk of ergonomic hazards due to radiographers’ work activities involving non-ergonomic postures, repetitive movements, and patient manual handling activities. These conditions have the potential to cause musculoskeletal disorders and reduce service quality. Based on conditions at the Radiology Installation of RSUD dr. Soeselo, the implementation of ergonomics in radiographers’ work activities has not been optimal; therefore, this study aimed to identify potential ergonomic hazards and their contributing factors. This study used a descriptive qualitative method conducted from October 2025 to April 2026 with research subjects consisting of 11 radiographers, the head of the installation, and the head of the unit. Data were collected through observation, interviews, documentation, and literature studies. The results showed that the main ergonomic hazards included bending postures, prolonged standing, and repetitive movements that caused complaints of lower back pain, shoulder pain, muscle soreness, and fatigue. The main contributing factors included the absence of ergonomic training, limited patient transfer aids, the unavailability of ergonomic standard operating procedures (SOPs), the use of heavy conventional equipment, and high workloads with shift systems. The implementation of ergonomics in the radiology installation has also not been carried out in a structured manner and has not been supported by routine evaluations. Therefore, it is necessary to develop ergonomic SOPs, conduct regular training, provide supporting equipment, and carry out routine evaluations to improve radiographers’ occupational safety and health. ABSTRAK Instalasi radiologi merupakan unit penunjang medis yang memiliki risiko hazard ergonomi tinggi akibat aktivitas kerja radiografer yang melibatkan postur tidak ergonomis, gerakan berulang, serta aktivitas manual handling pasien. Kondisi tersebut berpotensi menimbulkan gangguan muskuloskeletal dan menurunkan kualitas pelayanan. Berdasarkan kondisi di Instalasi Radiologi RSUD dr. Soeselo Kabupaten Tegal, penerapan ergonomi pada aktivitas kerja radiografer belum optimal sehingga penelitian ini bertujuan mengidentifikasi potensi hazard ergonomi dan faktor penyebabnya. Penelitian menggunakan metode deskriptif kualitatif yang dilaksanakan pada Oktober 2025–April 2026 dengan subjek penelitian sebanyak 11 radiografer, kepala instalasi, dan kepala ruang. Pengumpulan data dilakukan melalui observasi, wawancara, dokumentasi, dan studi kepustakaan. Hasil penelitian menunjukkan bahwa potensi hazard ergonomi utama meliputi postur membungkuk, berdiri terlalu lama, dan gerakan berulang yang menyebabkan keluhan nyeri pinggang, nyeri bahu, pegal, dan kelelahan. Faktor penyebab utama meliputi belum adanya pelatihan ergonomi, keterbatasan alat bantu pemindahan pasien, belum tersedianya SOP ergonomi, penggunaan alat konvensional yang berat, serta beban kerja tinggi dengan sistem shift. Penerapan ergonomi di instalasi radiologi juga belum dilakukan secara terstruktur dan belum didukung evaluasi rutin. Oleh karena itu, diperlukan penyusunan SOP ergonomi, pelatihan berkala, penyediaan alat bantu, dan evaluasi rutin untuk meningkatkan keselamatan dan kesehatan kerja radiografer.

Abstract

Latar Belakang: Pemeriksaan colon in loop merupakan prosedur radiografi penting dalam evaluasi kinis Carcinoma colon. di Instalasi Radiologi RS PKU Muhammadiyah Gombong, terdapat perbedaan dalam pelaksanaan prosedur, mulai dari persiapan pasien, alat dan bahan yang digunakan, teknik pemasukan media kontras, hingga pemilihan proyeksi. Penelitian ini bertujuan untuk mengetahui prosedur pemeriksaan, alasan modifikasi tahapan, dan fungsi masing-masing proyeksi. Metode: Penelitian menggunakan metode kualitatif deskriptif dengan pendekatan studi kasus, dilaksanakan pada Agustus–Juli 2025. Subjek penelitian adalah tiga radiografer dan satu dokter spesialis radiologi. Data diperoleh melalui observasi, wawancara, dokumentasi, dan studi pustaka, lalu dianalisis melalui tahapan reduksi, penyajian, dan penarikan kesimpulan Hasil : Hasil penelitian menunjukkan bahwa persiapan pasien terdiri dari persiapan umum dan khusus, persiapan alat dan bahan tidak menggunakan alat fluoroscopy. Teknik pemeriksaan diawali dengan proyeksi Anteroposterior (AP) abdomen polos, dilanjutkan pemasukan media kontras barium menggunakan proyeksi anteroposterior post kontras. Kemudian dilakukan teknik full filing dengan campuran 30cc media kontras positif dan 30cc negatif dimasukkan secara bersamaan, menggunakan proyeksi anteroposterior dan left posterior oblique (LPO). Modifikasi prosedur dilakukan untuk memperluas lumen usus, memastikan ada tidaknya sumbatan oleh tumor, serta meningkatkan efisiensi waktu. Proyeksi anteroposterior digunakan untuk mendapatkan gambaran menyeluruh colon, sedangkan oblique untuk memperjelas lekukan colon pada flexura hepatika dan lienalis. Kesimpulan : Prosedur di RS PKU Muhammadiyah Gombong tidak menggunakan fluoroscopy, serta menggunakan kombinasi proyeksi anterioposterior dan oblique. Disarankan penggunaan modalitas sinar-X dengan fluoroscopy dan teknik double kontras serta proyeksi post evakuasi untuk hasil diagnostik yang lebih optimal.

Abstract

ABSTRACT BNO-Intravenous Pyelography (BNO-IVP) in hydronephrosis requires appropriate projection and examination timing to obtain optimal diagnostic information. Procedural modifications may be necessary based on radiographic findings and anatomical conditions. This study aimed to describe the BNO-IVP procedure in hydronephrosis cases and analyze the considerations for using the Left Posterior Oblique (LPO) projection and extending examination time. This descriptive qualitative study used an observational case study approach at the Radiology Department of RS PKU Muhammadiyah Wonosobo. Data were collected through observation, semi-structured interviews, and documentation involving two patients with hydronephrosis and four informants consisting of three radiographers and one radiologist. Data were analyzed descriptively by comparing observations, interviews, documentation, and relevant theory. The BNO-IVP procedure included plain abdominal radiography, contrast administration, sequential radiographs, and post-micturition imaging. In one patient, LPO projection was performed at 15 minutes due to suspected double collecting system. The projection helped reduce ureteral superimposition and improve visualization of the urinary tract anatomy. In the other patient, examination was extended to 120 minutes due to delayed contrast excretion. These findings indicate that projection and examination-time modifications were adjusted based on radiographic findings and patient conditions to obtain more optimal diagnostic information. LPO may be used as an additional projection in suspected double collecting system or specific anatomical abnormalities, while extending the examination time may be performed in cases of delayed contrast excretion. ABSTRAK Pemeriksaan BNO-Intravenous Pyelography (BNO-IVP) pada kasus hidronefrosis memerlukan pemilihan proyeksi dan waktu pemeriksaan yang tepat untuk memperoleh informasi diagnostik yang optimal. Modifikasi prosedur dapat diperlukan berdasarkan temuan radiograf dan kondisi anatomi pasien. Penelitian ini bertujuan mendeskripsikan prosedur pemeriksaan BNO-IVP pada kasus hidronefrosis serta menganalisis pertimbangan penggunaan proyeksi Left Posterior Oblique (LPO) dan penambahan waktu pemeriksaan. Penelitian kualitatif deskriptif dengan pendekatan studi kasus observasional dilakukan di Instalasi Radiologi RS PKU Muhammadiyah Wonosobo. Data diperoleh melalui observasi, wawancara semi-terstruktur, dan dokumentasi terhadap dua pasien hidronefrosis dengan empat informan yang terdiri atas tiga radiografer dan satu dokter spesialis radiologi. Data dianalisis secara deskriptif melalui perbandingan hasil observasi, wawancara, dokumentasi, dan teori yang relevan. Hasil penelitian menunjukkan prosedur BNO-IVP meliputi foto polos abdomen, pemberian media kontras, pengambilan radiograf secara bertahap, dan pemeriksaan post-miksi. Pada satu pasien, proyeksi LPO dilakukan pada menit ke-15 karena dugaan double collecting system. Proyeksi tersebut mengurangi superposisi ureter dan memperjelas anatomi sistem urinaria. Pada pasien lainnya, pemeriksaan diperpanjang hingga menit ke-120 karena keterlambatan ekskresi media kontras. Modifikasi proyeksi dan waktu pemeriksaan dapat disesuaikan dengan temuan radiograf dan kebutuhan diagnostik pasien. Proyeksi LPO dapat dipertimbangkan sebagai proyeksi tambahan pada dugaan double collecting system atau kelainan anatomi tertentu untuk meningkatkan informasi diagnostik.

Abstract

ABSTRACT Proximal one-third femoral fractures are bone injuries that require operative management, one of which is Open Reduction Internal Fixation (ORIF). Postoperative radiographic examination is necessary to evaluate implant position, bone alignment, and the healing process. Limited patient mobility after surgery requires adjustments to the examination technique to maintain diagnostic image quality. This study aimed to determine the examination procedure, rationale for the technique, and its advantages and disadvantages in radiographic examination of proximal one-third femur post ORIF at the Radiology Installation of Salatiga City Hospital. This qualitative study employed a case study approach and was conducted from November 2025 to April 2026. Data were obtained through observation, interviews, and documentation involving three radiographers, an orthopedic specialist, and a radiologist. Data analysis included data collection, processing, presentation, and conclusion drawing. Post-ORIF femur radiography was performed without special preparation using Anteroposterior (AP) and lateral projections with a modified horizontal central ray angled 30° caudad in the lateromedial direction according to the patient's condition. For the lateral projection, the patient's position was maintained as comfortably as possible to reduce pain and the risk of complications. The results showed that the 30° caudad horizontal central ray modification produced a sufficiently diagnostic lateral image in post-ORIF patients with limited mobility. This modification reduced anatomical superimposition and improved visualization of the fracture area and implant without truncation. Therefore, adjustments to patient positioning and exposure technique can produce optimal diagnostic images while improving patient comfort. This technique maintains image quality, allowing optimal evaluation of the femoral structure and implant. ABSTRAK Fraktur femur 1/3 proksimal merupakan cedera tulang yang memerlukan penanganan operatif, salah satunya Open Reduction Internal Fixation (ORIF). Pemeriksaan radiografi pascaoperasi diperlukan untuk mengevaluasi posisi implant, keselarasan tulang, dan proses penyembuhan. Keterbatasan mobilitas pasien pascaoperasi memerlukan penyesuaian teknik pemeriksaan agar tetap menghasilkan gambaran diagnostik. Penelitian ini bertujuan mengetahui prosedur pemeriksaan, alasan penggunaan teknik, serta kelebihan dan kekurangan pemeriksaan radiografi femur 1/3 proksimal post ORIF di Instalasi Radiologi RSUD Kota Salatiga. Penelitian menggunakan metode kualitatif dengan pendekatan studi kasus yang dilaksanakan pada November 2025 sampai April 2026. Data diperoleh melalui observasi, wawancara, dan dokumentasi terhadap 3 radiografer, dokter spesialis ortopedi, dan dokter spesialis radiologi. Analisis data meliputi pengumpulan, pengolahan, penyajian data, dan penarikan kesimpulan. Pemeriksaan os femur post ORIF dilakukan tanpa persiapan khusus menggunakan proyeksi Anteroposterior (AP) dan lateral dengan modifikasi central ray horizontal menyudut 30° caudad arah latero-medial sesuai kondisi pasien. Pada proyeksi lateral, posisi pasien dipertahankan senyaman mungkin untuk mengurangi nyeri dan risiko komplikasi. Hasil penelitian menunjukkan bahwa modifikasi central ray horizontal 30° caudad mampu memberikan gambaran lateral yang cukup diagnostik pada pasien post ORIF dengan keterbatasan mobilitas. Modifikasi tersebut mengurangi superposisi struktur anatomi dan memperjelas area fraktur serta implant tanpa terpotong. Dengan demikian, penyesuaian posisi dan teknik penyinaran dapat menghasilkan gambaran diagnostik yang optimal sekaligus meningkatkan kenyamanan pasien. Teknik ini mempertahankan kualitas citra sehingga evaluasi struktur femur dan implant dapat dilakukan secara optimal.

Abstract

Microcephaly is a condition in which a child's head circumference is smaller than the average for age and gender. The diagnosis can be made using a CT Scan, which should be adjusted to the patient's age according to the IAEA protocol (2012), with an ideal effective dose of 1.8–3.8 mSv for a tube current of 200 mAs. However, at the Radiology Installation of Roemani Muhammadiyah Hospital Semarang, the examination was carried out without differentiating age, resulting in an excess dose of 9.317 mSv. This study aims to evaluate the procedures, diagnostic information, and radiation protection in plain head CT Scans of pediatric patients with microcephaly. This study used a qualitative approach through observation, interviews, and documentation of 3 radiographers, 1 PPR, and 1 radiologist. The results showed no protection for patients and only basic protection for companions. CT Scans showed decreased brain volume and ventriculomegaly. It is recommended that the CT Scan protocol be adjusted to the child's age to reduce the radiation dose, through the setting of exposure factors, slice thickness, and pitch.
