Abstract

Abstract

ABSTRACT BNO-Intravenous Pyelography (BNO-IVP) in hydronephrosis requires appropriate projection and examination timing to obtain optimal diagnostic information. Procedural modifications may be necessary based on radiographic findings and anatomical conditions. This study aimed to describe the BNO-IVP procedure in hydronephrosis cases and analyze the considerations for using the Left Posterior Oblique (LPO) projection and extending examination time. This descriptive qualitative study used an observational case study approach at the Radiology Department of RS PKU Muhammadiyah Wonosobo. Data were collected through observation, semi-structured interviews, and documentation involving two patients with hydronephrosis and four informants consisting of three radiographers and one radiologist. Data were analyzed descriptively by comparing observations, interviews, documentation, and relevant theory. The BNO-IVP procedure included plain abdominal radiography, contrast administration, sequential radiographs, and post-micturition imaging. In one patient, LPO projection was performed at 15 minutes due to suspected double collecting system. The projection helped reduce ureteral superimposition and improve visualization of the urinary tract anatomy. In the other patient, examination was extended to 120 minutes due to delayed contrast excretion. These findings indicate that projection and examination-time modifications were adjusted based on radiographic findings and patient conditions to obtain more optimal diagnostic information. LPO may be used as an additional projection in suspected double collecting system or specific anatomical abnormalities, while extending the examination time may be performed in cases of delayed contrast excretion.

Abstract

Chronic Otitis Media (COM) is an inflammation of the middle ear that can lead to intratemporal complications such as mastoiditis. Radiographic evaluation of the mastoid bone plays a crucial role in identifying structural damage and assessing the extent of infection. Theoretically, the Schüller method axiolateral projection involves a 45° head rotation and a 25°–30° caudal beam angulation. However, preliminary observations at the Radiology Department of 'Aisyiyah Ponorogo General Hospital revealed the use of a 40° caudal angulation. This study aimed to examine the mastoid examination procedure and the rationale behind the 40° caudal angulation at the Radiology Department of 'Aisyiyah Ponorogo General Hospital. A qualitative case study approach was employed at the facility between December 2025 and June 2026, involving interviews with three radiographers and one radiologist. Data collection was conducted through direct observation, interviews, and documentation. Data analysis was performed narratively—involving data reduction and presentation—followed by a comparison with theoretical standards and the drawing of conclusions. The results indicate that the mastoid examination is performed without special preparation, other than the removal of metal objects from the head area. Patients are positioned erect in a true lateral position, with the side of the head being examined placed against the detector. A bilateral Schüller method axiolateral projection is used, with a centering point 2–3 cm posterior-superior to the external auditory meatus (EAM), a 40° caudal beam angulation, and exposure factors of 70–80 kV, 200 mA, 16 mAs, and an FFD of 100–120 cm. The 40° angulation was selected to produce an elongation effect on the mastoid, thereby minimizing superimposition with surrounding bony structures, allowing for clearer visualization of mastoid air cells, and facilitating a comparison of the bilateral mastoid conditions. The study concludes that the mastoid examination procedure for patients with chronic otitis media (COM) at 'Aisyiyah Ponorogo General Hospital utilizes the bilateral axiolateral projection (Schüller method) with a 40° caudal angulation and no special preparation. This angulation aims to achieve optimal elongation of mastoid structures, minimize superimposition, and enhance the diagnostic accuracy for COM.

Abstract

A noncompliant patient is one who cannot adhere to instructions or collaborate with healthcare professionals throughout the assessment. A fixation aid is employed to help the radiographer in attaining appropriate positioning of the patient and the cassette, thus guaranteeing optimal radiographic outcomes. This study employed an experimental methodology through the research and development strategy. The cassette holder fixation device was evaluated on five radiographers. Data collection was conducted through functional and performance assessments of the cassette holder at RSUD (Regional General Hospital) Muntilan and Dr. Soeharso Hospital in Surakarta from December 2024 to August 2025. The evaluation employed a checklist questionnaire for radiographers. The cassette holder fixation device was designed and modified into a U-shape, serving as a cassette support, with the base providing stability for the holder. The cassette holder fixation device features a locking mechanism and includes an adjustable left-hand cassette support to accommodate various cassette sizes. The cassette holder fixation device is applicable for lateral projection studies of cervical vertebrae. The cassette holder fixation device is constructed from 0.5 cm thick acrylic. The outcomes of functional testing indicated an 86% success rate, above the minimum feasibility threshold of 75%. This illustrates that the cassette holder fixation device is efficient in stabilizing the cassette, reducing radiation exposure to the radiographer, patient, and family, and optimizing cervical spine examination duration. The cassette holder fixation device enhances stability for both the cassette and the examination object, thereby reducing the probability of repeat imaging and preventing patients from receiving double radiation exposure.
