Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a chronic, progressive, and frequently occurring health condition characterized by airflow obstruction caused by chronic bronchitis or. According to the World Health Organization, COPD ranks fourth as the leading cause of death globally, with approximately 3.5 million deaths caused by this disease. The purpose of writing this scientific paper is to provide nursing care to patients with COPD. The research method used in this Scientific Paper is a case study method that includes death assessment, death diagnosis, death intervention, death implementation, death evaluation and death documentation. The results of the study found 4 poisoning problems that appeared in the patient, namely Airway Clearance, Ineffective Breathing Pattern, Activity Intolerance, and Sleep Pattern Disorders. The results of the case study obtained Interventions that were arranged included effective coughing exercises, airway management, activity therapy, and sleep support. Based on the evaluation of accommodation for 4 days, it showed that the patient had improved as indicated by reduced shortness of breath, was able to cough effectively, activity intolerance had improved and sleep patterns had improved. Suggestions from this study can improve professional skills in medical-surgical care, enable patients and their families to receive care and adopt a healthy lifestyle, enable hospitals to serve as a reference in providing surgical care, and enable educational institutions to utilize the study as learning material for students regarding COPD.

Abstract

Acute gastroenteritis (GEA) remains a common health problem with a high morbidity rate, both globally and nationally. Based on World Health Organization (WHO) data, gastroenteritis causes millions of cases each year and can lead to serious complications such as dehydration and electrolyte imbalances. In Indonesia, the prevalence of acute gastroenteritis is approximately 6.8%, while in West Sumatra it reaches 8.5%. Data from Aisyiyah Pariaman Hospital indicate that gastroenteritis is among the top ten diseases, with an increasing trend in cases from 43 cases in 2023 to 90 cases in 2025. This condition shows that gastroenteritis remains a significant health problem requiring optimal management, particularly related to hygiene, dietary patterns, and environmental factors.The purpose of this case study is to understand and apply comprehensive nursing care for patients with gastroenteritis. The method used is a case study approach based on the nursing process, including assessment, diagnosis, intervention, implementation, and evaluation of patients in the Muzdalifah Ward at Aisyiyah Pariaman Hospital.The results show that patients with gastroenteritis generally experience diarrhea, nausea, vomiting, abdominal pain, and signs of dehydration such as decreased skin turgor and dry mucous membranes. The main nursing diagnoses include diarrhea, hypovolemia, hyperthermia, and risk of electrolyte imbalance. Interventions implemented, such as diarrhea management, fluid monitoring, rehydration therapy, and health education, have been shown to improve the patient’s condition.In conclusion, appropriate and comprehensive nursing care is essential in managing patients with gastroenteritis to prevent complications. It is recommended that healthcare providers enhance education on hygiene and healthy dietary habits, as well as strengthen patient monitoring to reduce the incidence of gastroenteritis.

Abstract

One of the non-communicable diseases (NCDs) is Chronic Kidney Disease. CKD is a global health problem with an increasing prevalence and contributes significantly to high morbidity and mortality rates. According to the World Health Organization, CKD is one of the leading causes of death worldwide. In Indonesia, the prevalence of CKD based on the 2023 Indonesian Health Survey (SKI) was 0.18% or approximately 638,178 people, while in West Sumatra it reached 0.23% or 13,042 cases. At the local level, RSUD Prof. H. Muhammad Yamin, S.H Pariaman recorded an increase in CKD cases in the internal medicine ward, with 118 cases in 2023, 227 cases in 2024, and 327 cases in 2025. This study aimed to describe medical-surgical nursing care for patients with CKD. The method used was a case study with a descriptive approach involving one patient during five days of hospitalization. Data collection techniques included interviews, observations, physical examinations, and documentation studies. The results identified seven nursing diagnoses: impaired gas exchange, ineffective peripheral perfusion, hypervolemia, acute pain, sleep pattern disturbance, impaired skin integrity, and risk of decreased cardiac output. Nursing interventions provided included positioning, blood transfusion, circulation care, hypervolemia management, pain management, sleep support, skin integrity care, and cardiac care. The evaluation showed gradual improvement in the patient’s condition. It can be concluded that comprehensive, individualized, and continuous nursing care is effective in improving the condition of patients with CKD.

Abstract

Hypertension is often dubbed the "silent killer" as it is frequently asymptomatic yet carries a high risk of serious complications such as heart disease, stroke, and kidney failure. According to the WHO (2025), hypertension is the leading cause of premature death worldwide, with an estimated 1.4 billion adults suffering from the condition. Based on the 2023 Indonesian Health Survey (SKI), the prevalence of hypertension in Indonesia reached 30.8% among residents aged >18 years, while in West Sumatra, it was recorded at 25.16%. In Pariaman City (2024), there were 8,164 hypertension patients who received health services. Medical record data at Prof. H. M. Yamin, S.H. Hospital, Pariaman, showed that hypertension patients experiencing complications reached 99.01% (602 patients) of the total cases in 2025.The purpose of this case study is to provide comprehensive nursing care for Mrs. E with hypertension in the Internal Medicine Ward of Prof. H. M. Yamin, S.H. Hospital, Pariaman. The method used is a descriptive case study conducted over five days, from February 4th to 9th, 2026, involving one research subject.The results of the initial assessment revealed extreme blood pressure of 210/117 mmHg and a pain scale of 7. Based on data analysis, four nursing problems were established: acute pain, risk of ineffective cerebral perfusion, disturbed sleep patterns, and risk of nutritional deficits, with acute pain as the top priority. The evaluation over five days showed that all nursing problems were resolved, marked by a decrease in blood pressure to 165/97 mmHg, the disappearance of pain (scale 0), and improved sleep patterns and nutritional intake, allowing the patient to be discharged. It is expected that the patient and their family will ensure that they adhere to a healthy lifestyle, a low-salt diet, and regular health checks to prevent complications.

Abstract

COPD is a condition characterized by narrowing of the airways. Based on World Health Organization data (2024), Chronic Obstructive Pulmonary Disease (COPD) is the third leading cause of death globally, with mortality rates reaching 3.5 million. The Indonesian Ministry of Health (2023) states that COPD is a significant health issue in Indonesia with a prevalence of 16 million sufferers, while West Sumatra recorded 4,284 cases (West Sumatra Health Office, 2024). Medical record data at Prof. Dr. H. Muhammad Yamin, S.H. Pariaman Regional Hospital shows that the number of COPD patients in 2026 was recorded at 122 patients. The objective of this case study is to describe the application of nursing care for patients with COPD in the Pulmonary Ward of Prof. Dr. H. Muhammad Yamin Hospital, Pariaman. The method used is a case study conducted from February 6th to 10th, 2026, at the Pulmonary Ward of Prof. Dr. H. Muhammad Yamin, S.H. Pariaman Hospital on one subject, Mrs. N, with COPD. The results identified five nursing diagnoses: ineffective airway clearance, impaired gas exchange, ineffective breathing patterns, disturbed sleep patterns, and activity intolerance. Interventions included effective cough training, respiratory monitoring, airway management, sleep support, and activity management. Evaluation over five days showed that all nursing problems—ineffective airway clearance, impaired gas exchange, ineffective breathing patterns, disturbed sleep patterns, and activity intolerance—were resolved. It is expected that the patient will increase awareness in avoiding triggers of breathlessness and maintaining a healthy lifestyle to prevent recurrent exacerbations.

Abstract

Femoral fracture is an emergency condition that can cause acute pain, impaired physical mobility, sleep pattern disturbances, and risk of infection. The main problem in patients with femoral fracture is the disruption of basic needs fulfillment due to physical limitations and pain responses that affect the healing process. This scientific paper aims to describe the comprehensive implementation of nursing care for patients with femoral fracture in order to improve the quality of nursing services and accelerate patient recovery. The method used was a descriptive case study with a nursing process approach, including assessment, nursing diagnosis, intervention planning, implementation, and evaluation. The research subject was Mrs. R with a medical diagnosis of left femoral fracture who was treated in the Malibo Inpatient Ward at RSUD Padang Pariaman. Data were collected through interviews, direct observation, physical examination, medical documentation, and relevant literature studies. The analysis results showed that the main nursing problems identified were acute pain, impaired physical mobility, sleep pattern disturbance, and risk of infection. Nursing implementation was carried out through pain management, mobility support, sleep support, and infection prevention. The case discussion showed that appropriate and continuous nursing interventions were able to reduce pain scale, improve mobility, enhance sleep patterns, and reduce the risk of infection.
